The purpose of this visit was to conduct a risk factor assessment. Good cooking temperatures observed as well as good cold-holding temperatures and good employee health policy records.
Discussed hand washing, sanitizer, and sanitizer bucket availability at all stations during today's visit. Thank you for your time today.

  • Critical: Handwashing / When to wash hands (corrected on site)
    Observation: A food employee failed to wash hands after handling money, removing items from the floor, and before slicing and boxing pizza. Discussed with person in charge and employee. After the discussion, the employee washed hands before resuming food handling.
    Correction: ALL food employees shall wash their hands OFTEN especially after handling trash, using the restroom, handling raw meats, handling soiled equipment and utensils, BEFORE putting on clean gloves, etc. Proper handwashing frequencies will aid in the reduction of foodborne illness.
  • Handwashing Sink / Used for Hand Wash Only (corrected on site)
    Observation: Observed the handsink across the 3-vat sink being used to add water to containers of food prep.
    Correction: Handsinks are to be used only for washing hands to encourage proper hand washing and to prevent contamination of cleaned hands. DISCUSSED WITH PERSON IN CHARGE.

The purpose of this visit was to conduct a routine inspection. Ensure QAC sanitizer is dispensing at appropriate concentrations. Walk-in refrigerator was maintained open during cleaning process as observed and discussed with food employee. Minimize time that the door is kept open to maintain temperatures at 41F or below.

  • Wiping Cloths/Use Limitation/Wet Towel Storage (repeated violation)
    Observation: Wet wiping cloths used for wiping counters and other equipment surfaces were observed stored on the counter.
    Correction: Wet wiping cloths are to be used for wiping counters and other equipment surfaces and are to be held in a chemical sanitizer solution at the appropriate concentration between uses.
  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site) (repeated violation)
    Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: garlic oil on top of 3DR prep refrigerator at 68F. American cheese in 2DR prep refrigerator (right) at 45F.
    Correction: Potentially hazardous foods (time/temperature conrol for safety food) shall be held cold at a temperature of 41°F or below. Garlic oil was discarded. Discussed time/temperature control with person in charge and food employees. American cheese was stored above fill line in prep refrigerator. After discussion, food employee placed American cheese in bottom of the 2DR prep refrigerator to cool to 41F or below.
  • Critical: Quaternary Ammonium Compound Sanitizer Solution Concentration per Use Directions
    Observation: When tested, the quaternary ammonium compound-based sanitizing solution was observed to be at a concentration less than what is required for effective sanitization of food-contact surfaces of equipment and utensils. Observed the sanitizing solution dispensing from mop sink and 3-vat sink with a concentration of 0 ppm total quaternary ammonium compound.
    Correction: The quaternary ammonium sanitizer used in this facility shall have a minimum concentration of at least 200 ppm OR a concentration between 150-400ppm based on the manufacturer's use directions and using the appropriate test kit. BLEACH SANITIZER SOLUTION PROVIDED AT 50 PPM. ENSURE QAC SANITIZER IS CALIBRATED TO DISPENSE AT APPROPRIATE CONCENTRATIONS AS INDICATED.

Note:
The manager has agreed to use Time as a Public Health Control 4 hours for portioned to go containers for marinara sauce. The manager will have a sheet which will be use to note time everyday. Also, the chef and manager decided to use cold holding instead of time as a public health control 4 hours for garlic sauce/garlic w/butter.
REMINDERS:

**Do not overfill containers on prep top.

  • Jewelry Prohibition
    Observation: Employees observed wearing watch on their arms while preparing food.
    Correction: Food employees shall limit their jewelry to a plain wedding ring while preparing food. Food employees may not wear bracelets, watches, etc. because they can inhibit proper hand washing.
  • Critical: Food Protection from Contamination / Package, Cover, Wrap (corrected on site)
    Observation: Unwrapped or uncovered food in the following location where the food is subject to contamination: gyro meat in prep refrigerator and several items in 1DR upright freezer.
    Correction: Foods shall be protected from contamination by being stored in packages, covered containers or wrappings unless being cooled.
  • Food Storage Containers, Identified with Common Name of Food (repeated violation)
    Observation: Unlabeled food containers with the following food items that are not easily identified by appearance: squeeze bottles with oil, dressings, water at cookline.
    Correction: ALL food and ingredients removed from their original container and stored in other food containers, squeeze bottles or "shakers" shall be labeled properly using the common name of the food (if the food or ingredient is not easily distinguished). For example, all cooking oils, salt, sugar, flour, starch, spices, herbs, etc. shall be labeled.

    Correction: Food shall be protected from contamination by storing the food at least 6 inches off the floor on approved shelving units or dunnage racks. Milk crates, soda crates, or bread racks are not suitable for food storage.
  • Critical: Hot Holding of Potentially Hazardous Foods at 135°F or More (corrected on site)
    Observation: The following food item was observed hot holding at improper temperatures using a calibrated food temperature measuring device: portioned to go containers of marinara sauce--in a tray above pizza oven.
    Correction: Potentially hazardous foods (time/temperature conrol for safety food) shall be held hot at a temperature of 135°F or above unless the permit holder is using "time as the public health control" as specified under 3-501.19. Note: Put in prep refrigerator.
  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site) (repeated violation)
    Observation: The following food items were observed cold holding at improper temperatures using a calibrated food temperature measuring device: garlic sauce with butter 66F--pizza prep table
    Correction: feta cheese 47F and Sliced provolone cheese 45F, sliced tomatoes 48F, tzatziki sauce 50F--True 2DR prep refrigerator left.
  • Critical: Date Marking of Refrigerated, Ready-to-Eat PHF (TCS Food) (corrected on site)
    Observation: The following refrigerated, ready-to-eat, potentially hazardous food that is prepared on site is not used or discarded within 24 hours of preparation and was not observed to be date marked: meat sauce--walk-in refrigerator. alfredo sauce and tuna salad--prep refrigerator.
    Correction: Refrigerated, ready-to-eat, potentially hazardous foods that are prepared on site and kept for more than 24 hours cannot have a shelf-life of more than 7 days, including the day of preparation. These food items must be marked with either the "consume-by" date and/or "preparation" date, color-coded labels, or another marking system/method that effectively indicates shelf-life that is clearly understood and properly used by the employees.
  • Food Contact Surface/Smooth, Easily Cleanable (corrected on site)
    Observation: Food contact surfaces of equipment and utensils are not finished to have a smooth, cleanable surface. Observed metal banded brush used for butter and broken brush for garlic sauce at cookline.
    Correction: Materials that are used in the construction of utensils and food contact surfaces of equipment may not allow the migration of deleterious substances or impart colors, odors, or tastes to food and under normal use conditions shall be finished to have a smooth, easily cleanable surface. Note: Discarded brush.
  • Temp Meas Devices for Cold & Hot Hold Equipment, Present (repeated violation)
    Observation: There was no temperature measuring device located in the following cold or hot holding equipment: 3 prep refrigerators
    Correction: Cold or hot holding equipment used for the storage of potentially hazardous foods (time/temperature control for safety food) shall be designed to include and shall be equipped with at least one integral or permanently affixed temperature measuring device that is located to allow easy viewing of the device's temperature display.
  • Sanitizer Test Kit Required (repeated violation)
    Observation: There is no test kit located in the facility for monitoring the concentration of the chemical sanitizing solutions.
    Correction: Obtain a QUATERNARY AMMONIA test kit.
  • Equipment / Fixed / Space / Seal
    Observation: The following equipment is not installed in a manner that allows accessibility for cleaning, nor is it installed with closely abutting surfaces to minimize the need for cleaning: hand sink in kitchen needs new caulking
    Correction: Equipment that is fixed because it is not easily movable shall be installed so that it is: 1) spaced to allow access for cleaning along the sides, behind, and above the equipment, 2) spaced from adjoining equipment, walls, and ceilings a distance of not more than 1 millimeter or one thirty-second inch, and 3) sealed to adjoining equipment or walls if the equipment is exposed to spillage or seepage.
  • Equipment / Good Repair / Components / Gaskets
    Observation: The door gaskets of the following unit is damaged: True 3DR pizza prep refrigerator.
    Correction: Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications.
  • Cutting Boards / Resurface / Discard
    Observation: The cutting boards along the cookline are heavily scratched and scored. The food contact surface is no longer easily cleaned and sanitized due to condition.
    Correction: Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced if they can no longer be effectively cleaned and sanitized, or discarded if they are not capable of being resurfaced.
  • Certified Food Manager/Certificate Process
    Observation: The person in charge (PIC) has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc.
    Correction: Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. (see handout provided) within 10 business days. A copy of the photo identification card can be faxed to the food safety section at (703) 385-9568. Failure to provide a copy of the photo identification card will result in further enforcement action.
  • Critical: Sanitizers, 21 CFR Criteria for Food Contact (corrected on site)
    Observation: The concentration of the quaternary ammonia solution was measured at more than 400 ppm. This concentration exceeds the approved limits for use with food-contact surfaces of equipment and utensils in a foodservice establishment.
    Correction: Maintain the concentration of quaternary ammonia solution at 200 ppm. Verify concentration using the appropriate test kit. Note: Diluted with water.

    Observation: The following food item for which time rather than temperature is used as a control is not marked or otherwise identified to indicate the time that is 4 hours past the point in time when the food is removed from temperature control. Garlic mixed with oil is not marked.

    Observation: The following food items from damaged packaging were found offered for sale or service: Observed dented cans of pizza sauce and marinara sauce in dry storage area.
    Correction: Food packages such as canned goods shall be in good condition and protect the integrity of the contents so that the food is not exposed to adulteration or potential contaminants. Return all dented and damaged canned goods to your food supplier for a credit.
  • Food Storage Containers, Identified with Common Name of Food
    Observation: The following food items that are not easily identified by appearance were observed without a label: Butter and olive oil
    Correction: Unless a food item can be easily recognized, all foods and ingredients removed from their original containers and stored in working containers shall be labeled using the common name of the food. For example: cooking oils, salt, sugar, flour, spices, herbs, etc.

    Observation: Grease is noted to be accumulating on the ceiling and on the metal surface at the unit. This appears to be due to insufficient ventilation of the hood system. Note: Per the manager the accumulation on the outer surfafce of hood was build up from past 24 to 48 hours.
    Correction: Ventilation hood systems and devices shall operate properly and be sufficient in number and capacity to prevent grease or condensation from collecting on walls and ceilings.
  • Food Temperature Measuring Device Required & Readily Accessible
    Observation: The certified food manager could not provide a food temperature measuring device.
    Correction: A metal-stemmed probe food thermometer shall be provided and readily accessible for use in ensuring attainment and maintenance of food temperatures.
  • Sanitizer Test Kit Required
    Observation: There is no test kit located in the facility for monitoring the concentration of the chemical sanitizing solutions.
    Correction: Obtain a BLEACH test kit.
  • Critical: Food-Contact Surface Sanitized before Use after Cleaning (corrected on site)
    Observation: The food-contact surfaces of the following equipment were not observed sanitized: There was no sanitizing solution in the sanitizing sink while the employee washed all the dishes. Note: Added bleach in the water at 100 ppm.
    Correction: All utensils and food-contact surfaces of equipment shall be sanitized before use and after cleaning.
  • Ventilation Systems, Shall Be Clean
    Observation: Observed that the hood filters had heavy grease accumulation, which could contaminate food. Per manager they have the filters replaced every 2 weeks.
    Correction: Clean intake and exhaust air ducts at regularly intervals to prevent them from becoming sources of contamination.

    Observation: Working containers of poisonous and toxic materials are not properly labeled with a common name. Degreaser not labeled.
