This visit was made in order to conduct a follow up inspection. Manager has obtained their Northern Virginia FPM card.
  • Refuse / Cover Receptacles / Outdoors / Tight-Fitting Lids (repeated violation)
    Observation: Outside refuse container was uncovered because one of the lids was broken off.
    Correction: Receptacles and waste handling units for refuse, recyclables, and returnables shall be kept covered with tight fitting lids or doors if kept outside the food establishment.

This visit was made to conduct a routine food safety evaluation. The following issue requires attention:
1. Obtain a Northern VA FPM card within 10 days and email or fax to the Alexandria Health Department.

  • Certified Food Manager: Certificate Process (repeated violation)
    Observation: The Person in Charge (PIC) has a valid certificate of successful completion of an accredited Certified Food Manager (CFM) exam, but does not possess the Northern Virginia CFM card.
    Correction: Obtain a Northern Virginia CFM card from ORS Interactive, Inc. within 10 business days. A copy of the Northern Virginia CFM card can be faxed to the Environmental Health Division at 703.746.4919. Failure to provide a copy of the photo identification card may result in further enforcement action.
  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site)
    Observation: The following food item(s) were measured at improper temperatures: bean sprouts in 2 door prep top.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below. (Bean sprouts were just taken out of the walk-in, and were moved to another prep top unit).

    Observation: Observed that paper towels were not provided at the handwashing sink next to the ice machine.
    Correction: Provide paper towels at each hand sink to allow employees to properly dry their hands after handwashing. (Paer towles were provided during inspection)

    Observation: Employees observed wearing jewelry on their arms and hands while preparing food (watches, bracelets).
    Correction: Food employees shall limit their jewelry to a plain wedding ring while preparing food. Food employees may not wear bracelets, watches, etc. because they can inhibit proper hand washing.
  • Critical: Food-Contact Surface Sanitized before Use after Cleaning (corrected on site)
    Observation: The food-contact surfaces of the following equipment were not observed sanitized: dishes, equipment and utensils.The hot water sanitizing dishmachine is not oerating properly.
    Correction: In conjunction with Section 4-703.11, the food-contact surfaces of equipment and utensils shall be sanitized before use and after cleaning.Use 3 comp sink to sanitize until repaired.

    Observation: Wiping cloths improperly used for the following activity: towel uunder cutting board.
    Correction: Cloths shall not be used for any other purposes. For example, cloths shall not be placed under cutting boards to prevent the boards from sliding while in use.
  • Cooling Methods (corrected on site)
    Observation: The following methods used for cooling would not have been adequate to facilitate cooling of the product from ambient to 41°F within 4 hours: cut tomatoes in tightly covered pans.
    Correction: Cooling foods shall be accomplished as follows: 1) placing the food in shallow pans, 2) separating the food into smaller or thinner portions, 3) using rapid cooling equipment, 4) stirring the food in a container placed in an ice water bath, 5) using containers that facilitate heat transfer, 6) adding ice as an ingredient, or 7) other effective methods.(foods that are cooling shall be uncovered)
  • Plumbing / Maintained in Good Repair
    Observation: Plumbing connections under the 3 comp sink piping are leaking (pipe was disconnected).
    Correction: A plumbing system shall be maintained in good repair. Recommend bracing or connector to keep pipe stable.

This visit was made to conduct a risk factor assessment. The following observations require attention:
1. Make sure hand sink is supplied with soap and paper towels at all times to promote proper hand washing. Proper hand washing is one of the most important control measures in the prevention foodborne illnesses.
2. Make sure food employees know to wash their hands after contamination such as handling trash, using the rest-room, handling raw meats, handling soiled equipment and utensils, and before putting on clean gloves.
3. If food employees drink while preparing food, make sure they drink from a container that is covered and consumed through a straw and stored in a manner that will prevent contamination to food, equipment, utensils, linens, and single service items.
--Repeat observation are subject to civil penalties.
Note: Food Safety Information binder provided to food establishment during the risk factor assessment. Discussed with the FPM major changes with the new 2013 Food Code to include employee health, clean-up of vomiting and diarrheal events, food allergens, responsibilities of permit holder (sticker provided), and increases in civil penalties. In addition, the 2013 Food Code training presentation in English was given to the FPM.

  • Critical: Hands and Arms / Cleaning Procedure (corrected on site)
    Observation: Observed food employees using improper handwashing procedures. Food employee washed her hands with soap but did not use a paper towel to properly dry off her hands.
    Correction: ALL food employees shall wash their hands in the following manner: 1) remove and discard used gloves, 2) turn on warm water at the hand sink, 3) rinse hands/arms with warm water, 4) lather hands/arms with soap and scrub for at least 20 seconds, 5) rinse hands/arms with warm water, and 6) dry hands/arms with paper towels and then turn off the water at the hand sink using the paper towel. Effective handwashing will aid in the reduction of foodborne illness and is an important part of the food employee training program. Corrected by discussing proper hand washing procedures with the food employee and Food Protection Manager (FPM).
  • Critical: Handwashing / When to wash hands (corrected on site)
    Observation: A food employee failed to wash his or her hands after handling soiled utensils and before handling clean utensils.
    Correction: ALL food employees shall be washing their hands OFTEN especially after handling trash, using the restroom, handling raw meats, handling soiled equipment and utensils, BEFORE putting on clean gloves, etc. Proper handwashing frequencies will aid in the reduction foodborne illness. Corrected by discussing when to properly wash hands with the food employee and the Food Protection Manager (FPM).
  • Eating, Drinking, or Tobacco Use / Proper Food Employee Drinking Containers (corrected on site)
    Observation: Open drinking containers stored in a manner that may contaminate food, food contact surfaces or utensils. Observed a plastic cup without a lid stored on the shelf above a prep table in the kitchen.
    Correction: Food employees may have beverages in the kitchen areas as long as they are covered and consumed through a straw and stored in a manner that will prevent contamination to food, equipment, utensils, linens, and single service items. Corrected by moving the open beverage container to the dish room.
  • Hand Drying Provision / Individual, Disposable Towels (corrected on site)
    Observation: Observed that paper towels were not provided at the hand sink adjacent to the ice machine.
    Correction: Provide paper towels at each hand sink to allow employees to properly dry their hands after handwashing. Corrected by providing paper towels for the hand sink adjacent to the ice machine.
