| ACR 36617 Caller indicates being in facility 9/1/14 and observed staff take trash out then return to condiment station without washing hands, and ceiling/roof leak in dining room. 9/9/14- Site visit for complaint inspection. Spoke to PIC/same person indicated in complaint. Copy of complaint provided, she was aware of handwashing concerns and has addressed. The leaks from the ceiling were 2 blocked condensation tubes.
