Discussed employee health with the person in charge. Please get some thermometers for inside all the refrigeration units.
  • Critical: Food - Packaged and Unpackaged - Separation, Packaging, and Segregation* (corrected on site)
    Observation: Raw foods (shell eggs) of animal origin stored over ready-to-eat (RTE) food in the refrigeration unit.
    Correction: Separate raw foods of animal origin during storage, preparation, holding, and display from raw RTE food including foods such as fish for sushi or molluscan shellfish, or other raw RTE food such as vegetables, and cooked RTE food.
  • Critical: Food - Potentially Hazardous Food - Cold Holding*
    Observation: The waffle batter was observed at 69 degrees.
    Correction: Discard the food and ensure that potentially hazardous foods for cold holding are maintained at 41°F or below to inhibit the growth of harmful bacteria. The waffle batter was voluntarily discarded.
  • Sanitizing Solutions, Testing Devices
    Observation: There is no properly working test kit provided in the facility for monitoring the concentration of the chemical sanitizing solutions.
    Correction: Obtain a quatarney test kit so that employees can accurately monitor the concentration of the sanitizing solutions and maintain them at a safe and effective level.
  • Handwashing - Using a Handwashing Lavatory - No Other Purpose
    Observation: The handwash station at the front of the shop is being used as a dump station.
    Correction: The handwash facility identified above is to be used for washing hands only
  • Critical: Plumbing System Maintained in Good Repair*
    Observation: A recent repair to plumbing system components of the front handsink is missing the right handle and is not in accordance with law.
    Correction: All repairs to plumbing equipment and lines are to be in accordance with law. Properly repair the equipment. The person in charge agreed to have this fixed.
  • Floors, Walls, and Ceilings - Cleanability
    Observation: There is a hole in the back wall. The area needs to be smooth and easily cleanable.
    Correction: Please patch the hole and repaint the area.
  • Handwashing Cleanser - Availability (corrected on site)
    Observation: Soap was not provided at the hand washing lavatory in the back handsink.
    Correction: Hand soap must be provided at all hand washing lavatories to encourage proper hand washing. Hand cleanser must always be present to aid in reducing microorganisms and particulate matter found on hands.
  • Hand Drying Provision (corrected on site)
    Observation: No disposable towels were provided at the hand washing lavatory in the at the back handsink.
    Correction: Hand drying devices such as individual disposable paper towels, a continuous towel system that supplies the user with a clean towel or heated air hand drying device must be provided at all hand washing lavatories to encourage proper hand washing and avoid employees to drying their hands on their clothing or other unclean materials

    Observation: Mops not hung up to air dry.
    Correction: Wet use maintenance items such as mops must be hung in a position that allows them to air dry so they are not allowed to grow contamination which can then be spread through the establishment at the next use.

Reviewed 5 major risk factors. Reviewed noncritical items and discussed employee health policy. Left information for management regarding employee health policy / standards.
  • Critical: Date Marking - Ready-to-Eat - Potentially Hazardous Food - Date Marking* (repeated violation)
    Observation: The ready-to-eat (RTE) commercially processed sliced ham in the refrigeration unit was not properly dated for disposition after opening.
    Correction: Mark a "consume by" date on commercially processed RTE foods at the time the original container is opened. If the food is held at 41°F or below the food shall be served or sold within 7 calendar days. Some harmful bacteria continue to grow even at refrigeration temperatures so limiting the amount of time in storage limits the amount of growth allowed for these bacteria. Owner agreed to correct.
  • Critical: Handwashing Lavatory*
    Observation: There is no handwash lavatory in the dish cleaning area, preventing routine handwashing by food workers. Hand sink had been disconnected from the wall.
    Correction: Re-install hand sink in this area to allow convenient use by food workers. Prior to installing additional plumbing fixtures the permit holder must apply and gain approval through the necessary regulatory agencies. 1) Owner of facility will be called by the local health department to emphasize the importance of having this fixture replaced, 2) recommend training staff to wash hands at the remaining front employee hand sink. 10/25/13 Owner was called. Owner agreed to re-install sink within the next 6-7 days.
  • Physical Facilities - Cleaning Frequency and Restrictions (repeated violation)
    Observation: Ceiling vent above the ice machine noted in need of cleaning. Heavy dust residues exist on this this vent.
    Correction: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner. Person-In-Charge agreed to clean.
  • Critical: Toxics - Common Name/working Containers of Toxics* (corrected on site) (repeated violation)
    Observation: Working containers of purple cleaner are not properly labeled.
    Correction: Working containers of toxic items are to be properly labeled with contents. Identification of these containers with the common name of the material helps prevent the dangerous misuse of the contents.
