The purpose of today's visit was to conduct a risk factor assessment inspection. Observed facility clean and organized during a preparation time. Observed that facility maintains equipment and food temperature logs as an active managerial control practice.

The purpose of today's visit was to conduct a routine inspection. Observed facility clean and organized. Please fax or email a letter from the supplier for the three fish species noted above confirming how they are raised and fed or how they are frozen for parasite destruction, no later than Friday, May 15, 2015.
  • Fish Served Raw RTE / Records Creation & Retention of Freezing For Parasite Destruction
    Observation: No records of freezing for parasite destruction are maintained for the following raw, raw marinated, partially cooked or marinated-partially cooked fish that are served or sold in ready-to-eat form:-------two types of shrimp & lobster.
    Correction: If raw or partially cooked fish are served or sold in ready-to-eat form, the person in charge shall record the freezing temperature and time to which the fish are subjected and shall retain the records for the frozen fish at the food establishment for 90 calendar days beyond the time of service or sale to a consumer. If the fish are frozen by a supplier, a letter the fish supplier stipulating that the fish is frozen as required shall be provided.
  • Farm Raised Fish Served Raw RTE / Written Statement from Supplier
    Observation: The following farm-raised fish served raw or partially cooked were observed without a written statement from the supplier stating that the fish were raised and fed according to 3-402.11(B)(3):-------salmon.
    Correction: Contact your fish supplier and ask for a written statement stating that the fish were raised in net pens if in open water OR land-based operations AND fed formulated feed that contains no live parasites. FAX a copy of this letter to your inspector and keep this statement on site at the food establishment for 90 days beyond the time of service of the fish.
  • Temp Meas Devices for Cold & Hot Hold Equipment, Present (corrected on site)
    Observation: There was no temperature measuring device located in the following cold holding equipment:
    1) True 2DR prep cooler # 1,
    2) True 2DR prep cooler # 2

    Correction: Cold or hot holding equipment used for the storage of potentially hazardous foods (time/temperature control for safety food) shall be designed to include and shall be equipped with at least one integral or permanently affixed temperature measuring device that is located to allow easy viewing of the device's temperature display.     Observation: The handwashing facility located adjacent to the dish machine was filled with wiping clothes, preventing access by employees for easy handwashing.
    Correction: A handwashing sink shall be maintained so that it is accessible at all times for employee use.

    Observation: Observed employee clothing and other possessions stored, on prep area in several places in the kitchen may contaminate food, food-contact surfaces of equipment, single-service items, or single-use items.
    Correction: Instruct employees to use the designated lockers and clothes hooks to prevent contamination of food, food equipment and utensils, and preparation surfaces.

    Observation: Observed that the floor/wall juncture around the kitchen is in need of cleaning.
    Correction: Clean the physical facilities at a frequency necessary to ensure the protection and safe preparation of food.

RECOMMENDATION:
Keep prep pans in the top of the prep refrigerators to assist in maintaining the TCS (time/temperature control for safety food) inside the refrigerator at 41F or below and not permitting the cold air from escape and the ambient air from entering the refrigerator.

    Observation: The following food item were observed cold holding at improper temperatures using a calibrated food temperature measuring device:Observed butterkaese and fontina cheese at 53F on the True 2 door prep top, salsa at 46F in the True 2 door prep refrigerator - MOVED TO A REFRIGERATOR CAPABLE OF MAINTAINING POTENTIALLY HAZARDOUS FOOD AT 41F OR BELOW
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth.

    Observation: A chemical vegetable wash solution is used in the establishment. No test kit is provided to measure the concentration of the chemical in the solution.
    Correction: A chemical vegetable wash is considered a food additive and must be maintained at the concentration recommended by the manufacturer in order to be safe. Provide the approved test kit.
  • Equipment / Good Repair / Operation (corrected on site)
    Observation: The following refrigeration unit is not operating properly and is unable to maintain cold food at or below 41°F:True 2 door prep refrigerator - TURNED DOWN THE TEMPERATURE OF THE UNIT
    Correction: Repair/adjust the unit so it is able to maintain foods at or below 41°F. Do not use the unit to store potentially hazardous foods while it is not operating properly.
  • Cutting Boards / Resurface / Discard
    Observation: The cutting board along the salad prep refrigerator and individual cutting boards are heavily scratched and scored. The food contact surface is no longer easily cleaned and sanitized due to condition.
    Correction: Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced if they can no longer be effectively cleaned and sanitized, or discarded if they are not capable of being resurfaced.
  • Handwashing Sink / Accessible at All Times (corrected on site)
    Observation: The handwashing facility located at the dishmachine and the 3-vat sink are blocked by utensils, towels and brooms, preventing access by employees for easy handwashing.
    Correction: A handwashing sink shall be maintained so that it is accessible at all times for employee use.

The purpose of today’s visit was to conduct a Risk Factor Assessment in conjunction with the investigation of a foodborne illness complaint received on February 15, 2013.
Additionally, please fax to my attention within 10 days, the parasite destruction information for the salmon and mahi mahi.

  • Critical: Responsibility of PIC to Require Food Employees to Report GI Symptoms or Cuts, Disease, Medical Condition or Exposure to PIC (repeated violation)
    Observation: Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy.
    Correction: A complete employee health policy must have the following elements:
    1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout)
