EHS to facility to follow up for three issues: Wall behind mop sink that was rotted - and bad odor noted, with flies in facility, shelving in walk in refrigerator in very poor condition, and lowboy refrigerator at front counter area that did not close properly. Work started directly after last inspection and wall behind mop sink replaced with pictures viewed on district managers phone, also drain behind washing machine checked as odor noted in that area and cleaned out. One fly seen during food delivery. Walk in refrigerator shelving replaced with new shelving. Lowboy refrigerator taken off line until new doors received. Per manager, sufficient refrigeration present to avoid cold holding problems. Noted in discussion with District Manager and General Manger - lighting for walk in refrigerator poor and forgotten in last report. Discussed installing additional lighting to ensure entire unit easy to see. GM will respond to EHS after determination made for additional lighting.

    Observation: Walk in freezer severely overcrowded so that entire floor space is taken up with storage and whole lines of food rollers must be pulled out to access food on shelves. Freezer space is not sufficient in number or capacity to meet the food storage demands of the establishment.
    Correction: Provide additional freezer space necessary to maintain food items at frozen solid state and be able to access food without pulling items out at room temperature. Overstocked equipment does not allow for proper air circulation and/or heat transfer. Improper food storage temperatures are a major contributing factor to foodborne illness.

    Observation: Small hot hold bin timers and bun steamer repaired. Receipt of parts for lowboy pending for repair.
    Correction: Repair lowboy so that doors will close and seal properly.

Overall good sanitation. Good handwashing, good glove use, good food temperatures and storage. Overall good cleanliness with one area impacted by building maintenance. Sanitizer in place and tested at 200 ppm Cl. Good employee health knowledge for symptoms, but staff uncertain of reportable diseases, written policy in place. New flyer given - review with all staff and ensure they understand actions to take and return times. *Wall in back area (with plumbing for mop sink) rotted behind loose tiles with foul odor. Flies present at least 10-15 with no easy to see source, so may be from wall with rotted smell. Follow up within 2 weeks *Wall area, shelving in walk in refrigerator in very poor condition that could cause contamination issues, and lowboy refrigerator that doors do not close all the way with gap present *also Fillet steamer currently out of order.

    Correction: Replace immediately. Protect food from miscellaneous sources of contamination.

    Observation: The following equipment with repair issues: *Small hot hold bins - "cabinets" with some timers that do not work so that staff have to go to other side of prep line to set timers that effect time / temperature control for food items. Lowboy door will not close. Fillet steamer not working.
    Correction: *Repair timers right away will follow up for repaired during 2 week follow up. Repair lowboy so that doors will close and seal properly - top holding milk good temperatures bottom items small creamers and small butters with temps in the mid 40's but hard to get good temperatures as items warm quickly. Repair all damaged or out of order equipment.
  • Handwashing Cleanser - Availability (corrected on site)
    Observation: Soap was not provided at the hand washing lavatory in the front prep area.
    Correction: Hand soap must be provided at all hand washing lavatories to encourage proper hand washing. Hand cleanser must always be present to aid in reducing microorganisms and particulate matter found on hands.
  • Pests - Controlling Pests* (repeated violation)
    Observation: Flies present with the following contributing factors: ***Wall in mop sink area rotted inside - may be breeding site. Trash can liners not used. Trash area and cleaning good.
    Correction: *Have wall checked immediately and repair all damaged areas when open if any signs of breeding site - infestation present have pest control done. Suggest trash liners to keep cans clean and reduce attraction to pests.

Good job of employee hand washing, good glove use, and sanitizer in place. Good overall temperatures except for the breakfast reach-in refrigerator.
  • Critical: Food - Potentially Hazardous Food - Cold Holding* (corrected on site)
    Observation: EHS observed Folded eggs, canadian bacon, sliced tomatoes, and shell eggs cold holding at improper temperatures
    Correction: Discard the food and ensure that potentially hazardous foods for cold holding are maintained at 41°F or below to inhibit the growth of harmful bacteria. Manager discarded and chilled other items.
  • Food Contact Surfaces - Cleanability*
    Observation: The food contact surface of the sheet pans and small hot holding bins are not designed or constructed to be smooth and it contains cracks, chips, or pits that can not be easily cleaned. EHS observed damage to the holdong bins and carbon build up on the sheet pans.
    Correction: Replace the sheet pans and holding bins to provide a food contact surface that is smooth and easily cleanable, free of breaks, open seams, cracks, chips, pits, sharp internal angles, corners, and crevices to prevent build-up of food debris.
  • Equipment - Good Repair and Proper Adjustment (repeated violation)
    Observation: The following equipment was observed in a state of disrepair and damaged: ***Reach-in refrigerator, walk-in refrigerator shelving rusting, grill drip pans leaking. Some tea dispensers bent.
    Correction: Repair the to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability.
  • Handwashing Lavatory, Water Temperature, and Flow
    Observation: Water from both handwashing sink has no cold water
    Correction: Make necessary adjustments at both handsinks..
  • Plumbing System Maintained in Good Repair (repeated violation)
    Observation: 3 compartment sink in poor repair. EHS observed a leak at the three compartment sink.
    Correction: Repair and maintain all plumbing components ans fixtures.
  • Pests - Controlling Pests* (repeated violation)
    Observation: Harborage conditions exist. EHS observed an infestation of flies and nats.
    Correction: Eliminate harborage conditions. Insects and other pests are capable of transmitting disease to man by contaminating food and food-contact surfaces. Please keep drains and areas around drink machines clean.
08/17/2015Routine
Overall excellent sanitation seen. Observed good handwashing, good glove and utensil use, good food temperatures, and storage. Labeling with some issues seen, old labels not always removed with new labels not placed. General manager will work with staff preparing items for labeling practices. Will assess overall labeling during next visit. Discussed, labeling requirements are an important part of preventing the growth of Listeria in leftovers. Good employee heath knowledge. Noted dumpster open on arrival and corrected. Shake machine out of order today and called in for maintenance. ***New McDonalds policy if car at window waiting for service, keep window open as customers think staff are being rude. Suggest placing a sign - in the window that is easily visible - explaining window are closed when not speaking to keep flies out. Flies just beginning to become active and if present in the area will be attracted to food smells and get in kitchen - causing large contamination risk.

    Observation: Shelving in the walk in refrigerator rusty with the potential for rust to fall into food - especially if food is cooling and uncovered until cooled - the correct method for cooling.
    Correction: Replace shelving right away. Protect food from miscellaneous sources of contamination.
  • Equipment - Good Repair and Proper Adjustment
    Observation: Grill "Grease trap" or drip pan with hole present resulting in grease pooling on the floor under the grill.
    Correction: Floor cleaned each day, however, replacing or repairing trap to prevent grease from pooling on the floor will improve cleanliness, decrease attraction to pests and improve prevention of injuries. Please repair or replace trap right away.
04/29/2015Routine
Overall much improved. Discussed with GM / DM grill and drive through drain both repaired 29 Oct 14, the day after the initial inspection. Temperature log listed sausage - that was cooked on the right grill - at 167 F from this morning. Drive through area with no water on floor, drain working properly. Cleaning much improved with some deep cleaning still needed in a few areas. Complete deep cleaning for areas in need of additional cleaning. Noted grill with grease under unit - drip pan just emptied. Have pan / and holder checked for flaws, replace if necessary. A few Some flies noted in facility, all food covered and containers correctly stored.

    Observation: Mop sink area in poor repair, deteriorated grouting, tiles etc.
    Correction: Repair / replace deteriorated tiles and grouting.
11/14/2014Follow-up
Overall mixed inspection. Good handwashing, glove use, and sanitizer in place for use and used. Good labeling both for leftovers and time. Cooking temperatures not adequate for right grill, burgers visibly not cooked as well as middle grill, otherwise good food temperatures. Employee health policy knowledge mixed, most symptoms known, but reportable diseases not known - review policy with staff, symptoms, return time, and reportable diseases, especially discussing close contact with reportable diseases, ie family member with a disease, flyer left. Two open packages of cheese returned to reach in refrigerator, where they may be subject to contamination, or more likely bare hand contact. Flies present in facility create a serious contamination risk for both food and food containers. Most single service containers properly stored with some eating surfaces up. Ensure that all containers are properly stored to protect from contamination and all food out for preparation is covered when not actually being used to prepare meals. Contact EHS when grill and drain are repaired. Follow up in about 2 weeks for cleaning / flies.
  • Critical: Food - Packaged and Unpackaged - Separation, Packaging, and Segregation* (corrected on site)
    Observation: Tray of uncovered bacon and cheese on prep line, flies present including once on tray.
    Correction: Food on tray covered with sandwich wrappers. Protect food from cross contamination by storing food in packages, covered containers, or wrappings.
  • Critical: Temperature, Cooking - Raw Animal Foods* (corrected on site)
    Observation: Hamburgers cooked on right grill with inconsistent final cook temperatures with initial check of small patties from 133-144 F and for 2 large patties 162 and 150-151 F (for 15 seconds each). Patties bloody looking on top some more than others.
    Correction: Undercooked hamburgers discarded (instead of finished cooking for QA purposes of facility). *Discontinue use of right grill until maintenance is done and reliable consistent cooking temperatures using McDonalds set cooking times can be achieved. Right grill taken off line during inspection.
  • Plumbing System Maintained in Good Repair
    Observation: Noted fast dripping water from front of drive through ice bin. When checked drain line completely filled with water, and water flowing onto floor.
    Correction: Discontinue use of drive through ice bin. Have drain line cleared. Clean and sanitize bin, scoop etc. Ensure gap is present when bin returned to holder over drain lines to prevent backflow into bin.
  • Lighting, Intensity
    Observation: Two lights out in walk in freezer.
    Correction: Replace lights. If lights continue to burn out, have maintenance done.
  • Physical Facilities in Good Repair
    Observation: Mop sink area in poor repair, deteriorated grouting, tiles etc.
    Correction: Repair / replace deteriorated tiles and grouting.
  • Pests - Controlling Pests*
    Observation: Several flies present in facility. The following contributing factors noted:
    -Drive through window opened approximated 10 minutes from arrival of EHS until inspection started.
    -Non food contact surfaces soiled with food debris/grease etc.
    -Areas under the grill, ice machine and drive through drink area with debris etc. on the floor and around the floor drain.
    -Drain cover missing for floor drain under the drive through drink machine.
    -Black garbage transport cart by back door filled with garbage / with flies and gnats present.
    -Recycle container has had garbage with old food or other substance placed in container with substance spilled out on container.
    -Dumpster with bad odor.
    -Dumpster area with debris.
    -Brooms in facility heavily soiled.
    -Trash lines not used in some cans.

    Correction: Clean continually all areas that will not create contamination risk while food, containers etc. are out. Ensure garbage is emptied often, containers are kept clean, and liners are used. Keep transport cart emptied, and cleaned. Clean dumpsters and area. Ensure garbage is completely contained in bags, if bags prone to break, double or change bags. Open garbage attracts flies for a breeding area, and results in many flies by/in the facility. When facility is closed, and no food or containers are out, kill flies (manually / with fly swatter etc.) clean and sanitize. Clean or replace brooms. Do not spray unless a staff member is a certified pest control operator. Contact pest control for additional suggestions / treatment. Because flies are attracted to and breed in rotting organic material / fecal matter, they physically "pick up" harmful bacteria then walk on food surfaces or food inside the facility depositing the bacteria that may cause food borne illness.

    Observation: Open container of fryer cleaner left on shelf above fryers.
    Correction: Cleaner stored in back area location. Ensure chemicals are stored away / under food / food containers and food prep areas.
10/28/2014Routine
Overall outstanding. Best overall sanitation in 5 years. Good food temperatures, good overall handwashing, good glove use, sanitizer in place at 200 ppm quats and used, good food storage, good labeling, good employee health knowledge, good cleanliness. New general manager in place discussed the following: Grill blue gloves used to handle raw meats / eggs then removed do not replace handwashing. Train staff to remove gloves and wash hands after handling any raw meats/eggs and before touching anything else. Facility with new prep line refrigerator. Per GM all McDonalds cheese with testing to allow tempering for good melting on sandwiches. Please send copy of information to EHS. Please send copies of current certified food managers certificates for new GM and Mgr L. R. Some facility issues like replacing damaged tiles grouting etc. including loose tiles on outside walkway. Please correct as soon as possible.
No violation noted during this evaluation.
04/18/2014Risk Factor
Follow up inspection done with the following results: Overall much improved.
0160: Very good
3270: Much improved, 2 flies noted after at facility for 2 hours.
1770 C/3180: Much improved. Cup holders remain soiled and cleaned during follow up inspection. Area under grill with grease dripping down. Area initially cleaned during previous inspection and clearly cleaned afterwards. Check system for flaws allowing for grease dripping down under unit instead of into drip pans to empty.
0470: All food storage good.
3170: Mop sink, plan to fix in September.
2 door breakfast reach in refrigerator, cheese 39 stem temperature.

No violation noted during this evaluation.
10/02/2013Follow-up
Overall improvements needed. Noted bag of frozen chicken in chest freezer with obvious signs of thawing and refreezing. When asked if they had noticed the chicken and what action should be taken, staff had noticed chicken, stated they would inform the manager and returned the chicken to the freezer for use. Creamer unit out of order with real creamer present. Discussed both issues with district manager and manager. Both the chicken and the cream were discarded before EHS left facility. Most significant cleaning issues corrected before EHS left facility, ie ice bins. Ensure staff training is done. Noted good food temperatures and good glove use and changing on the sandwich make line.
  • Critical: Hands - When to Wash* (corrected on site)
    Observation: Mixed handwashing, with good handwashing when arriving to work, changing tasks etc, however, hands not washed until asked by EHS after touching face, itching head, touching glasses, handling open bags of raw chicken, after putting on blue gloves and placing raw meats on grill and before handling sanitized utensils, and after picking container up from floor and taking to dish sink and before resuming making sandwiches.
    Correction: Discussed with manager and staff handwashing done. Discussed, reviewing "hand awareness" being aware of what hands are touching from the time workers enter the kitchen until leaving kitchen.
  • Critical: Food - Packaged and Unpackaged - Separation, Packaging, and Segregation* (corrected on site) (repeated violation)
    Observation: The following cross contamination issues seen: 1) Raw eggs stored over and very close to ready to eat foods. 2) Hamburger chest freezer left open with open bags of hamburger patties for much of the inspection and flies present in facility. 3) Food bins dropped on the floor, picked up, placed on the sandwich make line, then taken to the dish sink, and worker resumed making sandwiches. 4) Open bags of raw frozen chicken stored in chest freezer with ready to eat, pre-cooked mcgriddles and eggs. 5) Uncovered tray of bacon present.
    Correction: 1. Raw eggs moved and stored below and separate from ready to eat foods. 2. Chest freezer closed. 3. Sandwich make line sanitized. 4. Pre-cooked breakfast items removed for lunch period, and per DM, was not supposed to remain in unit. 5. Bacon covered. Discussed, food should remain covered (when no being prepared) to protect from contamination at all times, and keeping food protected from contamination is especially important if any flies are present.

    Observation: Walk in refrigerator door handle broken.
    Correction: Repair.
  • Equipment - Food Contact Surfaces, Nonfood Contact Surfaces, and Utensils
    Observation: Noted insides and outsides of equipment soiled. Insides and outsides of ice bins soiled. Floors especially under equipment such as grill, shelving, and drink machine at drive through window soiled. Accumulated debris in some floor drains. Drain covers missing for some drains. Food cart and cup holders soiled. Ceiling above make table soiled.
    Correction: Thoroughly clean all noted areas. Check cleaning schedule to ensure schedule is sufficient and if so ensure schedule is followed. Unclean areas provide an attractant to pests and can become a contributing factor to food contamination.

    Observation: Mop sink tiles broken, grouting in poor condition, and drain cover missing.
    Correction: Repair.
  • Critical: Pests-Controlling Pests* (repeated violation)
    Observation: 7-8 flies noted on cords above the make table. The following issues noted that contribute to flies inside the facility: Soiled brooms stored in the back room. Cords above make table with grease and food splatter. Drive through window when not serving customers. Cleaning issues noted in facility. (Dumpster and dumpster area ok).
    Correction: Train staff to keep windows closed when not actively serving customers. Thoroughly clean facility, including cleaning cords above make table. If brooms are not able to be cleaned, replace. Contact pest control operator for evaluation and further suggestions. The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by using methods such as trapping devices or other methods as required.

Overall excellent sanitation, with good handwashing, good glove use, good use of time tapes to track time for public health control, good food temperatures, clean food contact surfaces and sanitizer in place at 400 ppm quats and used. Some deep cleaning needed in bottom cabinets, shelving etc. Noted, drive through window not working properly, repair ordered and pending. Noted one staff drink stored with facility food. Refresh training where staff may store personal drinks - below and separate from all facility food, suggest labeling a shelf with a tray.

    Observation: Some staff with outdated Western Tidewater food handler cards.
    Correction: Ensure all staff have current Western TIdewater Health District food handler cards.

    Observation: Two staff handling utensils, equipment handles etc. with food gloves contaminated with raw meat at the grill area, both with and without blue gloves.
    Correction: All contaminated areas sanitized immediately during inspection. Normal policy, use blue gloves for raw meat, take off, wash hands, use regular food handler gloves. Suggest using a utensil, with handle up - never touching raw meats - placed in a deep container in the freezer with the meat. Workers touch the clean handle, place meat on grill, and replace tongs without contaminating gloves. Or other method to reduce risk of contamination.
