The purpose of this visit was to conduct a routine inspection. Quaternary ammonium sanitizer 3 vat sink: 400 ppm.
  • Critical: Equipment / Food-Contact / Visibly Clean (corrected on site)
    Observation: The following equipment/utensils were observed soiled to sight and touch: boxes used to wrap dispense food stored on basement shelving.
    Correction: Equipment food-contact surfaces and utensils shall be clean to the sight and touch. A surface that is soiled cannot be effectively sanitized. NOTE: Discussed wiht PIC who will keep all food contact wraps and boxes covered/protected.

  • Critical: Food Protection from Contamination / Package, Cover, Wrap (corrected on site)
    Observation: Unwrapped or uncovered food in the following location where the food is subject to contamination: RTE cookies stored uncovered in freezer.
    Correction: Foods shall be protected from contamination by being stored in packages, covered containers or wrappings unless being cooled. NOTE: PIC corrected during the inspection.
  • Critical: Raw Animal Foods Cooked to 155°F (corrected on site)
    Observation: The following comminuted or ground meat, fish, or game animal was not cooked to a sufficient temperature and/or time to eliminate pathogenic bacteria: beef burgers cooked on griddle were placed in hot hold tray with some pink color remaining (incomplete color change) and insufficient cooking temperature: 2x 153/151 degrees F
    Correction: Raw animal foods such as ratites, injected meats, comminuted meats, commercially raised game animals, and raw eggs not for immediate consumption shall be cooked to a minimum internal temperature of 155°F for at least 15 seconds. NOTE: Discussed with PIC. Burgers were returned to griddle for additional cooking, and hot hold tray washed and sanitized. Advised PIC to have timer for automatic griddle checked and/or proper procedure for automatic griddle in use. Cooked beef burger temperatures of >= 155 degrees F must be verified.
  • Handwashing Cleanser, Available at Handwashing Facilities (corrected on site)
    Observation: Observed that no hand soap was not provided in either hand soap dispenser in men's room.
    Correction: Provide hand soap at each hand sink to allow employees to properly wash their hands. NOTE: PIC had soap containers refilled.

    Observation: Surfaces of ice bins and enclosed components of equipment are not being cleaned as required.
    Correction: Surfaces of utensils and equipment contacting food that is NOT potentially hazardous (time/temperature control for safety food) such as ice bins, beverage dispensing nozzles, and enclosed components of equipment such as ice makers, cooking oil storage tanks and distribution lines, beverage and syrup dispensing lines or tubes, coffee bean grinders, and water vending equipment at a frequency specified by the manufacturer or at a frequency necessary to preclude accumulation of soil or mold. CFM DISCUSSED WITH EMPLOYEE.
  • Handwashing Sink / Used for Hand Wash Only (corrected on site)
    Observation: Observed the handsink at the ICE MACHINE being used TO DUMP ICE AND CLEAN EQUIPMENT.
    Correction: Handsinks are to be used only for washing hands to encourage proper hand washing and to prevent contamination of cleaned hands. CFM DISCUSSED WITH EMPLOYEES.
  • Critical: Plumbing / Maintained in Good Repair (repeated violation)
    Observation: Plumbing connections CARRYING WASTE WATER LOCATED OVER DRY STORAGE AREA IS LEAKING.
    Correction: A plumbing system shall be maintained in good repair. REPAIR THE LEAK WITHIN 10 DAYS FOR REINSPECTION.
  • Critical: Controlling Pests / Methods to Use When Pests Are Found
    Observation: Methods are not being used to control pests. OBSERVED FRUIT FLIES ON THE LEAKING WASTE WATER PIPE IN THE BASEMENT DRY STORAGE AREA.
    Correction: Insects and other pests are capable of transmitting disease to people by contaminating food and food-contact surfaces. If pests are found on the premises methods shall be used to control their presence. PROVIDE PEST CONTROL SERVICE REPORT TO THE HEALTH DEPARTMENT WITHIN 10 DAYS.

The purpose of today's visit was to conduct a routine inspection.
Fax repair invoice for the walk-in-cooler to the Health Department within 10 days.

  • Food Storage / Preventing Contamination (corrected on site)
    Observation: Food stored on the floor and/or food stored less than 6 inches off the floor. OBSERVED 3 BUCKETS OF ICED TEA ON THE WALK-IN-FLOOR.
    Correction: Food shall be protected from contamination by storing the food at least 6 inches off the floor on approved shelving units or dunnage racks. Milk crates, soda crates, or bread racks are not suitable for food storage. MOVED THE BUCKETS.
  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site)
    Observation: The following food items were observed cold holding at improper temperatures using a calibrated food temperature measuring device: (1) corn and bean salsa in the walk-in-cooler at 52F (2) smoothie base in the walk-in-cooler at 48F.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth. UNPACKAGED FOOD DISCARDED AND OTHER POTENTIALLY HAZARDOUS FOODS MOVED TO OTHER COOLERS AND FREEZERS.
  • Equipment / Good Repair / Operation
    Observation: The following refrigeration unit is not operating properly and is unable to maintain cold food at or below 41°F: WALK-IN-COOLER
    Correction: Repair/adjust the unit so it is able to maintain foods at or below 41°F. Do not use the unit to store potentially hazardous foods while it is not operating properly. CONTACTED TECHNICIAN FOR SERVICE.
  • Plumbing / Maintained in Good Repair
    Observation: Plumbing PIPE IN THE DRY STORAGE AREA DOWNSTAIRS IS LEAKING.
    Correction: A plumbing system shall be maintained in good repair. IN THE PROCESS OF BEING REPAIRED.

  • Critical: Responsibility of PIC to Require Food Employees to Report GI Symptoms or Cuts, Disease, Medical Condition or Exposure to PIC (repeated violation)
    Observation: Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy.
    Correction: A complete employee health policy must have the following elements:
    1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout)
  • Equip/Utensils/Linens/Single-Service/Storage, Location
    Observation: Observed that clean single service items downstairs in the storage room were stored in an manner that exposed the items to contamination.---EHS observed boxes of single service items, french fries and hash brown holders was uncovered.
    Correction: Store clean equipment, utensils, and linens in a clean, dry location where they are not exposed to splash, dust, or other contamination and at least 6 inches above the floor.
