A FOLLOW UP inspection was conducted today to verify compliance with pest control measures and cleanliness of the facility. At today's inspection EHS was provided with documentation of pest control for extermination of cockroaches with latest service done on 8/19/2015. Facility will continue to have targeted pest control every two weeks till insects are eliminated. It was observed that facility has increased the cleaning frequency and specific staff assigned to a cleaning schedule rotation. It was observed that all noted violations have been corrected in this follow up..

  • Critical: Controlling Pests / Methods to Use When Pests Are Found
    Observation: Methods are not being used to cockroaches. OBSERVED COCKROACHES IN NYMPH STAGE ON THE WALL BEHIND THE RAM 280 UNIT.
    Correction: Cockroaches are capable of transmitting disease to people by contaminating food and food-contact surfaces. If cockroaches are found on the premises methods shall be used to control their presence. DISCUSSED WITH MANAGER , ECOLAB WAS CALLED DURING INSPECTION , FACILITY TO CONDUCT PEST EXERMINATION FOR COCKROACHES WITHIN 3 DAYS. FOLLOW UP WILL BE CONDUCTED IN 2 WEEKS TO VIEW DOCUMENTATION OF PEST CONTROL AND INSPECT FOR COCKROACHES.
  • Cleaning Frequency for Physical Facilities (corrected on site)
    Observation: Observed that the floor area along the edges ,behind the sink near the Ecolab fly trapper, near the back door and behind the ice machine and Ram 280 are in need of cleaning. OBSERVED DIRT, FOOD DEBRIS AND OTHER SEDIMENTS AT THESE AREAS WHICH CAN CAUSE POTENTIAL FOR INSECT AND OTHER VECTOR INFESTATION.
    Correction: Clean the physical facilities at a frequency necessary to ensure the protection and safe preparation of food. DISCUSSED WITH MANAEGR TO INCREASE THE CLEANING FREQUENCY OF THESE AREAS.
08/05/2015Routine
A Site visit was made today following a complaint received that the men's and women's restrooms were closed and waster was flowing on the floor of the mens restroom. Site visit revealed that both restrooms were clean, floors dry and the toilets were in working order. Hot and cold water faucets were working and there were no smells or odors. Manager informed me that the men's toilet drain was clogged with water flowing on to the floors. This was repaired within one and a half hours and the toilets and men's restroom then came back in service . Complaint is closed.

In addition to the following violations documented above, the following violations associated with foodborne illness risk factors and public health interventions were noted:
4-501.11(a) Observed the Carrier glass door counter top refrigerator unable to hold potentially hazardous food (PHF) at 41F or below. Do not use this refrigerator to hold PHF until it is repaired and capable of holding PHF at 41F or below.

The purpose of this visit was to respond to a complaint received by the Health Department on September 23, 2013. The complainant alleged she and another customer returned french fries and the french fries were put on the line for re-service. Additionally, the customer alleged the non-food contact surfaces of the soda area were dirty. Upon investigation, discussed with the certified food manager no food is to be re-served unless it is non-potentially hazardous in a closed container or non-potentially hazardous in an original unopened package that is in good condition.     Observation: Wet wiping cloths used for wiping counters and other equipment surfaces were observed stored on the counter.
    Correction: Wet wiping cloths are to be used for wiping counters and other equipment surfaces and are to be held in a chemical sanitizer solution at the appropriate concentration between uses.
  • Equip/Utensils/Linens/Single-Service/Original Package (corrected on site)
    Observation: Coffee filters were observed stored unprotected at the coffee machines.
    Correction: Single-service items shall be stored in a clean, dry location that is not exposed to splash, dust, or other contamination and at least 6 inches off the floor on approved shelving units or dunnage racks. In addition, the single-service items shall be kept in the original protective package or stored by using other means that afford protection from contamination until used.
  • Handwashing Signage at Handwashing Facilities Required
    Observation: Observed that sign or poster that notifies food employees to wash their hands is not provided at all handwashing sinks used by food employees - women's and men's restroom
    Correction: Provide a sign or poster at all handwashing sinks used by food employees that reminds food employees to wash their hands.
