    Observation: Employees observed working in the food service area without proper hair restraints - beard nets.
    Correction: Ensure all employees wear hair restraints such as hats, hair coverings or nets, beard restraints, and clothing that covers body hair, that are designed and worn to effectively keep their hair from contacting exposed food, clean equipment, utensils and linens, and unwrapped single-service and single-use articles. This section does not apply to food employees such as counter staff who only serve beverages and wrapped or packaged foods, hostesses, and wait staff if they present a minimal risk of contaminating exposed food

    Observation: Raw foods of animal origin stored over/on ready-to-eat (RTE) food in the refrigeration unit: raw hamburger on container of potato salad, roll of raw hamburger adjacent to container of spinach dip, raw shell eggs over cooked chicken and adjacent to cream cheese.
    Correction: Separate raw foods of animal origin during storage, preparation, holding, and display from raw RTE food including foods such as vegetables and cooked RTE food.

    Observation: Dispensing utensils improperly stored between uses - the dipper well did not have sufficient velocity to flush particulates to the drain.
    Correction: Store in-use utensils or dispensing utensils in one of the following manners: 1) In the food with their handles above the top of the food and the container. 2) In food that is not potentially hazardous with their handles above the top of the food within containers or equipment that can be closed. 3) On a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food-contact surface of the food preparation table or cooking equipment are cleaned and sanitized every 4 hours. 4) In running water of sufficient velocity to flush particulates to the drain. 5) In a clean, protected location if the utensils are used only with a food that is not potentially hazardous. 6) In a container of water if the water is maintained at a temperature of at least 135°F.

    Observation: @FOOD@ cold holding at improper temperatures in the Salad refrigeration unit.
    Correction: Discard the food and ensure that potentially hazardous foods for cold holding are maintained at 41°F or below to inhibit the growth of harmful bacteria.

    Observation: The prepared ready-to-eat (RTE) @FOOD@ in the refrigeration unit is not properly dated for disposition.
    Correction: Mark the name and "consume by" date on the container of RTE foods at the time of preparation if the food is to be held for more than 24 hours. If the food is held at 41°F or below the food shall be served or sold within 7 calendar days. Some harmful bacteria continue to grow even at refrigeration temperatures so limiting the amount of time in storage limits the amount of growth allowed for these bacteria.

    Observation: The food contact surface of the racks in the refrigeration unit is not smooth and it contains cracks, chips, or pits that can not be easily cleaned - corroded racks in the refrigeration units.
    Correction: Replace the @EQUIPMENT@ to provide a food contact surface that is smooth and easily cleanable, free of breaks, open seams, cracks, chips, pits, sharp internal angles, corners, and crevices to prevent build-up of food debris.

    Observation: The ambient (air/water) temperature measuring device (degrees F) located in the @EQUIPMENT@ is not accurate or missing.
    Correction: Provide a temperature measuring device that is accurate within +/- 3 degrees Fahrenheit in the intended range of use in all refrigeration units.

    Observation: The following equipment food-contact surfaces were observed soiled to sight and touch: the refrigerated units (boards) for food preparation - the lids, sides of the unit where food touches, taco shells are placed in an open container with dust hanging from the rack above, dipper well and inside of refrigeration units.
    Correction: Clean and sanitize these surfaces for food contact. Cleaning of theses areas were started and some finished at time of inspection.

    Observation: Observed accumulations of dust, dirt, food residue or other debris on the following nonfood-food contact surfaces: shelves, all racks (for storage and refrigeration), outside of equipment where needed.
    Correction: Maintain nonfood-contact surfaces of equipment clean.

    Correction: Clean food-contact surfaces of @EQUIPMENT@ no less than every 4 hours to prevent the growth microorganisms on those surfaces.

    Observation: Grease on pavement next to recyclable oil container outside the facility.
    Correction: The refuse/recyclable container storage area and grounds adjacent to the container are to be maintained clean and sanitary.

    Observation: The following physical structures were noted in need of cleaning: floors, walls, and attached equipment, downstairs storage area and walk-in.
    Correction: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.

    Observation: A food employee failed to wash his or her hands before engaging in food preparation, after touching bare human body parts, after coughing, sneezing, eating, after handling soiled utensils or after engaging in any activity which may have contaminated his/her hands - not washing hands before donning new gloves.
    Correction: Instruct food employees to clean their hands and exposed portions of their arms immediately before engaging in food preparation including working with exposed food, clean equipment and utensils, and unwrapped single-service and single-use articles and during food preparation, as often as necessary to remove soil and contamination and to prevent cross contamination when changing task.

    Correction: Relocate food to a refrigeration unit that is capable of maintaining food storage at 41°F or below. Repairman was called to repair and he affirmed that it was corrected.

    Observation: @EQUIPMENT@ was observed in a state of disrepair and damaged - one of the fryers was not working, and 1 of the 2 bulbs on the heat lamp was not working (not due to a bulb outage).
    Correction: Repair the @EQUIPMENT@ to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the @EQUIPMENT@, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.
