    Observation: In use open drinking containers stored in a manner that may contaminate food, food contact surfaces or utensils.
    Correction: Employees may drink from a closed beverage container if the container is handled to prevent contamination of (1) The employee's hands (2) The container and (3) Exposed food clean equipment, utensils, and linens and unwrapped single-service and single-use articles.
  • Critical: Food-PHF/TCS-Cold Holding-shell eggs (corrected on site)
    Observation: Observed raw shell eggs not stored in refrigeration equipment that maintains ambient air temperature of 45°F or less. Observed garlic not stored at 58F..
    Correction: Store raw shell eggs in refrigerated equipment capable of maintaining ambient air temperatures of 45°F or less. Store garlic at 41F or below.
  • Handwashing - Using a Handwashing Lavatory (corrected on site)
    Observation: The handwashing facility located at the rear of the kitchen is blocked, preventing access by employees for easy handwashing.
    Correction: Access to the handwashing facility identified above is to be available during all hours of operation. Remove the trash can and hand tools preventing its use.

    Observation: Plumbing connections under the 3-compartment sink are leaking.
    Correction: Plumbing systems and components shall be maintained in good repair.

    Observation: Observed leaking dumpster outside the rear of the facility.
    Correction: Replace leaking refuse containers, they are an attractant for pests.
  • Handwashing Cleanser - Availability (corrected on site)
    Observation: Soap was not provided at the hand washing lavatory in the rear of the kitchen.
    Correction: Hand soap must be provided at all hand washing lavatories to encourage proper hand washing. Hand cleanser must always be present to aid in reducing microorganisms and particulate matter found on hands.
  • Hand Drying Provision (corrected on site)
    Observation: No disposable towels were provided at the hand washing lavatory in the rear of the kitchen.
    Correction: Hand drying devices such as individual disposable paper towels, a continuous towel system that supplies the user with a clean towel or heated air hand drying device must be provided at all hand washing lavatories to encourage proper hand washing and avoid employees to drying their hands on their clothing or other unclean materials

    Observation: Food preparation table shelf covered with cardboard and observed rusted noted in need of cleaning.
    Correction: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.

    Correction: Rapidly reheat the food to 165°F and maintain at 135°F or above through the hot holding period.
  • Temperature - Food Temperature Measuring Devices - Provided (corrected on site)
    Observation: The Person in Charge could not produce an appropriate food temperature measuring device to verify cooking temperatures of thin foods such as meat patties or fish filets.
    Correction: A temperature measuring device with a suitable small-diameter probe that is designed to measure the temperature of thin masses shall be provided and readily accessible to accurately measure the temperature in thin foods such as meat patties and fish filets.

    Observation: There was not hot water available at the handsink.
    Correction: Repair the handsink so that hot water is available.

-Small fire occurred in the BBQ cooker at the end of the cook line on 6-11-13. Overhead fire suppression system was not discharged, however a small handheld fire extinguisher was partially discharged.
-Fire suppression system observed inspected by Fire Safety and update.
-All fire suppressant observed cleaned from the facility.
-Please clean the hood system every 3 months.
- Phone conversation with Lee Simpkins (Radford Fire Department).
-Facility may reopen.

It is recommended that the person on charge complete an approved food safety class. Please fax a copy of the purchase invoice for replacement of the cutting board to the Montgomery Health Department within 30 days.
  • Food Storage Containers - Identified with Common Name of Food (corrected on site)
    Observation: Unlabeled food containers.
    Correction: Label working containers with the common name of its contents. Consumers may be allergic to certain foods or ingredients. The mistaken use of an ingredient when the consumer has requested that it not be used may result in severe medical consequences. Liquid foods and granular foods may resemble cleaning compounds. The mistaken use of food from an unlabeled container could result in chemical poisoning.
  • Cooling Methods (corrected on site)
    Observation: Cooked chicken observed cooling in large rectangular container in the walk-in cooler.
    Correction: Cool foods by the following methods: 1. Placing food in shallow pans 2. Separating food into smaller thinner portions 3. Using rapid chill cooling equipment 4. Stirring the food in a container placed in an ice water bath 5. Using containers that facilitate heat transfer 6. Adding ice as an ingredient 7. Any other method that can effectively accomplish cooling.
  • Equipment - Cutting Surfaces
    Observation: The cutting board(s) along the preparation table are heavily scratched and scored. The food contact surface is no longer easily cleaned and sanitized due to condition.
    Correction: Resurface or discard and replace the cutting boards to provide a food contact surface that is smooth, easily cleaned and sanitized.
  • Handwashing Cleanser - Availability (corrected on site)
