    Observation: Observed that paper towels were not provided at each handwashing sink.
    Correction: Provide paper towels at each hand sink to allow employees to properly dry their hands after handwashing. Person in charge provided towels at the handsink nearest to the 3-vat sink.

  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site) (repeated violation)
    Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: 1) ham 49F, turkey 48F, american cheese 49F in 3dr preptop cooler sandwich, 2) meat sauce 45F and mozzarella cheese 45F in walkin cooler
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth. All potentially hazardous food items in 3dr preptop cooler were placed in pizza cooler and walkin cooler. Walkin cooler was in "defrost" mode and air temperature was able to get down to 41F and below and food temps were also dropping
  • Temp Meas Devices for Cold & Hot Hold Equipment, Present (repeated violation)
    Observation: There was no temperature measuring device located in the following cold or hot holding equipment: superior 2dr glass sliding cooler
    Correction: Cold or hot holding equipment used for the storage of potentially hazardous foods (time/temperature control for safety food) shall be designed to include and shall be equipped with at least one integral or permanently affixed temperature measuring device that is located to allow easy viewing of the device's temperature display.
  • Equipment / Good Repair / Operation
    Observation: The following refrigeration unit is not operating properly and is unable to maintain cold food at or below 41°F: 3dr preptop cooler sandwich
    Correction: Repair/adjust the unit so it is able to maintain foods at or below 41°F. Do not use the unit to store potentially hazardous foods while it is not operating properly.
  • Preset Tableware / Protected by Wrapping, Covering, or Inverting (corrected on site)
    Observation: Preset tableware was observed unprotected from contamination.
    Correction: Tableware that is preset shall be protected from contamination by being wrapped, covered, or inverted.

    Observation: The handwashing facility located next to the 3-vat is blocked, preventing access by employees for easy handwashing.
    Correction: A hand washing sink shall be maintained so that it is accessible at all times for employee use.
  • Outer Openings, Protected / Screen Requirements (repeated violation)
    Observation: Openings to the exterior of the building are present along the back door
    Correction: Fill or close holes and other gaps along floors, walls, and ceilings to protect against the entry of insects and rodents.
  • Cleaning Frequency for Physical Facilities
    Observation: Observed that the wall by the cookline is in need of cleaning.
    Correction: Clean the physical facilities at a frequency necessary to ensure the protection and safe preparation of food.

The purpose of today's visit is to conduct a routine inspection.
Please fax following items to my attention within 10 days:
1) Copy of CFM card, provided handout for ORS Interactive.
2) Copy of invoice for replacing cutting board at the pizza prep line.
REMINDERs:
Monitor datemarking, hand washing, no bare hand contact, receiving temperatures, cooling methods and holding temperatures.
**USE THE HAND SINK located near 3-vat sink. DO NOT BLOCK IT and provide soap, hot water and paper towel.

  • Critical: Responsibility of PIC to Require Food Employees to Report GI Symptoms or Cuts, Disease, Medical Condition or Exposure to PIC (corrected on site) (repeated violation)
    Observation: Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy." Note: New employee (the manager) has to sign the form.
    Correction: A complete employee health policy must have the following elements:
    1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout)
  • Critical: Eating, Drinking, or Tobacco Use / Proper Food Employee Drinking Containers (corrected on site)
    Observation: A food employee was observed eating in areas where they may contaminate food, clean equipment, utensils or other items needing protection.
    Correction: Food employees may only eat, drink, or use tobacco in a designated area like a dining room or office area. Employees shall not eat or drink in the kitchen areas, warewashing areas, or storage areas where items may become contaminated.
  • Food Storage Containers, Identified with Common Name of Food
    Observation: The following food items that are not easily identified by appearance were observed without a label: flour, oil, butter, sauce bottles and prep line/cook line.
    Correction: Unless a food item can be easily recognized, all foods and ingredients removed from their original containers and stored in working containers shall be labeled using the common name of the food. For example: cooking oils, salt, sugar, flour, spices, herbs, etc.
  • Cooling Methods (corrected on site) (repeated violation)
    Observation: The following methods used for cooling were not adequate to facilitate proper cooling: cooked chicken, cooked beef covered with lid in prep refrigerator.
    Correction: Cooling hot foods shall be accomplished as follows: 1) placing the food in shallow pans

    Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: cooked chicken 47F, cooked tandoori chicken 55F--2DR pizza prep top.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth. Note: Put inside the refrigerator. Do not overfill containers.
  • Nonfood-Contact Surface/Corrosion Resistant/Nonabsorbent
    Observation: The attachments and non food contact surfaces for the dough maker, outer surface of the flour container and blender base.
    Correction: Non-food contact surfaces of equipment that are exposed to splash, spillage, or other foods that require frequent cleaning shall be constructed of a corrosion-resistant, nonabsorbent, and smooth material.
  • Temp Meas Devices for Cold & Hot Hold Equipment, Present
    Observation: There was no temperature measuring device located in the following cold holding equipment: walk-in refrigerator (thermometer on door broken) and 3DR prep refrigerator missing, 2 chest freezers, 1DR freezer, and the thermometer inside the pizza prep refg is also broken.
    Correction: Cold or hot holding equipment used for the storage of potentially hazardous foods (time/temperature control for safety food) shall be designed to include and shall be equipped with at least one integral or permanently affixed temperature measuring device that is located to allow easy viewing of the device's temperature display.
  • Equipment / Good Repair / Components / Gaskets
    Observation: The door gaskets of the following unit is damaged: 2Dr pizza prep refg.
    Correction: Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications.
  • Single-Service and Single-Use Articles / No Reuse
    Observation: Manufacturer container of sour cream was observed reused for the storage of tzatziki sauce.
    Correction: Manufacturer containers may not be reused for secondary storage of other foods because the containers are not durable enough for multiple reuse and become uncleanable.
  • Critical: Equipment / Food-Contact / Visibly Clean (corrected on site) (repeated violation)
    Observation: The following equipment/utensils were observed soiled to sight and touch: can opener and cutting board at prep line (it has lots of cuts).
    Correction: Equipment food-contact surfaces and utensils shall be clean to the sight and touch. A surface that is soiled cannot be effectively sanitized.
  • Certified Food Manager/Certificate Process
    Observation: The person in charge (PIC) has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc.
    Correction: Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. (see handout provided) within 10 business days. A copy of the photo identification card can be faxed to the food safety section at (703) 385-9568. Failure to provide a copy of the photo identification card will result in further enforcement action.
  • Handwashing Sink / Accessible at All Times (repeated violation)
    Observation: The handwashing facility located at the 3-vat sink is blocked, preventing access by employees for easy handwashing. Per the manager they never use it because it is in the corner and there is a big metal rack to hold clean utensils.
    Correction: A handwashing sink shall be maintained so that it is accessible at all times for employee use. Note: Discussed the importance and reason of using the hand sink near 3-vat. They moved the rack a little bit.
  • Handwashing Sink / Used for Hand Wash Only (corrected on site)
    Observation: Observed the handsink at the front being used to fill the water jug.
    Correction: Handsinks are to be used only for washing hands to encourage proper hand washing and to prevent contamination of cleaned hands.

    Observation: A food employee was observed handling the following ready-to-eat food using their bare hands: while making sandwich the employee touched bread with bare hands. Note:The owner will take the sandwiches home.
    Correction: Except when washing fruits and vegetables, food employees may not contact ready-to-eat foods using their bare hands. Employees shall use suitable utensils such as deli tissue, spatulas, tongs, or clean disposable gloves to handle all ready-to-eat foods.

    Observation: The following food items were observed cold holding at improper temperatures using a calibrated food temperature measuring device: peeled boiled eggs 49F, sliced cheese 49F, sliced turkey 46F--3DR prep refg. Note: put all items inside.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth.
  • Critical: Date Marking of Refrigerated, Ready-to-Eat PHF (TCS Food) (corrected on site)
    Observation: The following refrigerated, ready-to-eat, potentially hazardous food that is prepared on site is not used or discarded within 24 hours of preparation and was not observed to be date marked: lasagna
    Correction: Refrigerated, ready-to-eat, potentially hazardous foods that are prepared on site and kept for more than 24 hours cannot have a shelf-life of more than 7 days, including the day of preparation. These food items must be marked with either the "consume-by" date and/or "preparation" date, color-coded labels, or another marking system/method that effectively indicates shelf-life that is clearly understood and properly used by the employees.

    Correction: Refrigerated, ready-to-eat, potentially hazardous commercially processed foods when opened and kept for more than 24 hours cannot have a shelf-life of more than 7 days, including the day of the original container was opened. These food items must be marked with either the "consume-by" date and/or "preparation" date, color-coded labels, or another marking system/method that effectively indicates shelf-life that is clearly understood and properly used by the employees. This date cannot exceed any manufacturer determined "use-by" date based on food safety.

    Observation: The following equipment is not installed in a manner that allows accessibility for cleaning, nor is it installed with closely abutting surfaces to minimize the need for cleaning: Handsink in women restroom needs new caulking.
    Correction: Equipment that is fixed because it is not easily movable shall be installed so that it is: 1) spaced to allow access for cleaning along the sides, behind, and above the equipment, 2) spaced from adjoining equipment, walls, and ceilings a distance of not more than 1 millimeter or one thirty-second inch, and 3) sealed to adjoining equipment or walls if the equipment is exposed to spillage or seepage.
  • Cutting Boards / Resurface / Discard
    Observation: The cutting boards along both the prep refrigerators and other cutting boards in kitchen are heavily scratched and scored. The food contact surface is no longer easily cleaned and sanitized due to condition.
    Correction: Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced if they can no longer be effectively cleaned and sanitized, or discarded if they are not capable of being resurfaced.

    Correction: Repair the physical facilities. Poor repair and maintenance compromises the functionality of the physical facilities.

    Observation: Observed that grease has accumulated in between equipment and is in need of cleaning. Also, observed that walls and ceiling is need of cleaning. Non food contact surfaces like outside of meat slicer, microwave, container for flour is in need of cleaning.
    Correction: Clean the physical facilities at a frequency necessary to ensure the protection and safe preparation of food.
  • Mops in Air-dry Position (repeated violation)
    Observation: Observed that mops are improperly stored between use.
    Correction: Store mops in a position that allows them to air dry without soiling walls, equipment, or supplies.
  • Critical: Conditions of Use/Presence & Use of Toxics Per Law (corrected on site)
    Observation: Pesticides labeled for residential use were observed in the facility and are being applied by the operator to control pests. Observed spray can of home defense pesticide spray. Note: Discarded.
    Correction: Remove residential use pesticides from the facility. When necessary for pest control, chemical pesticides must be applied by a certified pesticide applicator to prevent injury and contamination.
  • Critical: Sanitizers, 21 CFR Criteria for Food Contact (corrected on site)
    Observation: The concentration of the bleach solution was measured at more than 200 ppm. This concentration exceeds the approved limits for use with food-contact surfaces of equipment and utensils in a foodservice establishment.
    Correction: Maintain the concentration of bleach solution between 50 to 200 ppm. Verify concentration using the appropriate test kit.
