Note: Need to get maintenance to paint the wall beside the walk-in freezer during the summer or sometime not during operation. Also, there is one crate of milk stored directly on the floor in the walk-in cooler. Need to put milk on approved shelving at least six inches off of the floor or just put an empty milk crate between the floor and the crate of milk.

    Observation: The refuse container or dumpster used to store refuse/recyclables has no drain plug.
    Correction: Replace the drain plug to the refuse container or dumpster.

    Observation: The wash temperature of the high temperature sanitizing ware wash machine is not meeting the manufacturer's specifications. The minimum temperature required is 150°F while the actual temperature was 143°F.
    Correction: Need to have the booster heater on the dish machine serviced so that the minimum wash temperature per the manufacturer's specifications is met.

    Correction: Ensure wet wiping cloths are stored in a chemical sanitizer at the proper concentration between use.

    Observation: Observed the faucet at the hand wash sink in the kitchen has a leak which is in poor repair.
    Correction: Repair and maintain all plumbing components and fixtures.

    Observation: Observed a light bulb is out at the grill vent hood and there are cracks and holes in the wall near the walk-in freezer in the dry storage area which is not maintained in good repair

  • Critical: Hands - When to Wash* (corrected on site)
    Observation: A food employee failed to wash his or her hands after handling soiled utensils at the dishwasher or after engaging in any activity which may have contaminated his/her hands. Observed employee only drying hands with a paper towel.
    Correction: Instruct food employees to clean their hands and exposed portions of their arms immediately before engaging in food preparation including working with exposed food, clean equipment and utensils, and unwrapped single-service and single-use articles and during food preparation, as often as necessary to remove soil and contamination and to prevent cross contamination when changing task.
  • Equipment - Good Repair and Proper Adjustment
    Observation: Observed the thin tip probed thermometer is not working.
    Correction: Need to replace the battery.
  • Critical: Handwashing Lavatory*
    Observation: There are not an adequate number of handwash lavatories on site. (See 12 VAC5-421-2280)
    Correction: Install additional lavatories to increase the employee's opportunity to wash hands routinely at the dishwashing area. Prior to installing additional plumbing fixtures the permit holder must apply and gain approval through the necessary regulatory agencies.
  • Sink - Service Sink
    Observation: There is no utility sink or curbed cleaning facility designed for the cleaning of mops or similar wet floor cleaning tools and for the disposal of mop water and similar liquid wastes.

    Observation: Observed a hose attached to a faucet fixture. The hose extended below the flood rim level of the hot chad.
    Correction: Install an approved backflow prevention device (double check valve with atmospheric break) or alter the length of the hose to provide the necessary separation between the water supply and the flood rim level of the sink basin. The minimum allowable separation distance must be at least 2x the diameter of the water supply inlet and at least 1 inch.
  • Outer Openings - Protected
    Observation: Observed the back door of the establishment is not tightly fitted at the bottom.
    Correction: Need to repair by putting in a new sweep at the bottom of the door to prevent the entry of pests.
  • Lighting, Intensity
    Observation: Less than 10 foot candles of light was noted in the walk-in refrigerator.
    Correction: Provide at least 10 foot candles at a distance of 30 inches from the floor in the walk-in refrigeration units and dry storage areas and in other areas and rooms during periods of cleaning. Need to install an additional light.
  • Physical Facilities in Good Repair
    Observation: The screen on the back door is not maintained in good repair
    Correction: Maintain physical facilities in good repair by fixing or replacing the screen on the back door. Poor repair and maintenance compromises the functionality of the physical facilities.
