    Observation: The following food item(s) in the True 2DR prep cooler (bottom portion) were observed cold holding at improper temperatures using a calibrated food temperature measuring device:
    1) Shredded mozarella cheese at 49F,
    2) Garlic in oil at 48F.

    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth. MOVED TO WALKIN COOLER TO CHILL.

The purpose of today's visit was to conduct a routine inspection.
  • Critical: Gloves/Use Limitation/No Re-Use/Use For Only One Task (corrected on site)
    Observation: Single-use gloves worn during multi-tasked food preparation.--------Observed food employee cutting raw chicken with gloved hands, then rinse off gloves at three compartment sink, then enter walkin cooler without finish removing gloves, washing hands and putting on a new pair of gloves.
    Correction: If used, single-use gloves shall be used for only ONE task and used for no other purposes. The single-use gloves shall be discarded when damaged or soiled or when interruptions occur in the operation. DISCUSSED WITH MANAGER, WHO DISCUSSED WITH FOOD EMPLOYEE.

The purpose of today's visit is to conduct a routine inspection.
**Monitor cold holding, cooking, receiving temperatures.
DO NOT USE 3DR prep refrigerator until it is capable of holding temperatures at 41F or below. Please call a technician to fix the refrigerator and fax and invoice to my attention within 10 days. Note: The owner adjusted the thermostat during inspection.

  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site)
    Observation: The following food items were observed cold holding at improper temperatures using a calibrated food temperature measuring device: Sliced tomatoes 47F, sliced provolone 50F, 44F, sliced american cheese 54F.--3DR prep top. Sour cream 53F, feta cheese 54F, chicken salad 52F--3DR prep refrigerator. Mozzarella cheese 46F--2DR prep refg. Note: All items discarded except sliced provolone at 44F and mozzarella cheese.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth.

    Observation: The ambient (air/water) temperature measuring device located in the following unit is not accurate in 15°F. The thermometer in 3DR prep refg was reading under 30F. Note: Placed new thermometer in the refrigerator.
    Correction: Ambient air and water temperature measuring devices that are scaled only in Fahrenheit shall be accurate to +/-3°F in the intended range of use.
  • Food Temperature Measuring Device Required & Readily Accessible
    Observation: Food temperature measuring device was not working, no battery.
    Correction: A metal-stemmed probe food thermometer shall be provided and readily accessible for use in ensuring attainment and maintenance of food temperatures.

    Observation: The following refrigeration unit is not operating properly and is unable to maintain cold food at or below 41°F: 3DR prep refrigerator 46F.
    Correction: Repair/adjust the unit so it is able to maintain foods at or below 41°F. Do not use the unit to store potentially hazardous foods while it is not operating properly.
  • Nonfood-Contact Surfaces Clean at Frequency to Preclude Accumulation
    Observation: The nonfood-contact surfaces of the following equipment had accumulations of grime and debris: Observed shelves above 3-vat sink, area around equipment, gasket for refrigerators, refrigerator door is in need of cleaning.
    Correction: Nonfood-contact surfaces of equipment and utensils shall be cleaned at a frequency necessary to preclude accumulation of soil residues.

    Observation: There is no Certified Food Manager present at the beginning of the inspection.
    Correction: It shall be unlawful to operate a food establishment unless it is under the immediate control of a certified food manager. It is highly suggested that you have more than one employee with the food manager's license. ORS Interactive, Inc. (see handout provided) issues the required photo identification card with proof of successful completion of a certified food manager's exam. Failure to have a certified food manager on site during ALL hours of operation including food preparation, food service, and cleaning/sanitizing of equipment/utensils will result in closure of the establishment.

This is a followup inspection to check on the clean up of a grease spill related to a complaint inspection which took place on 16 april, 2013. The grease has been cleaned up from the back of the facility. However, the facility still needs to place a grease catch pan below each grease barrel (two) behind the facility. When the grease catch pans are installed within 10 days, fax or email a copy of a picture showing their installation. Thank you

The purpose of this visit is a complaint inspection for a grease spill on the back parking lot. The complaint is confirmed. The manager agreed to clean the grease spill and to provide a grease catch pan for each grease barrel. A follow-up will be conducted within 10 days to determine compliance. Thank you.
  • Refuse / Cover Receptacles / Outdoors / Tight-Fitting Lids (corrected on site)
    Observation: Outside refuse container was uncovered.
    Correction: Receptacles and waste handling units for refuse, recyclables, and returnables shall be kept covered with tight fitting lids or doors if kept outside the food establishment.
  • Refuse / Area / Clean
    Observation: Grease was observed on the sides of the grease barrels and on the ground adjacent to the two grease barrels associated with this facility.
    Correction: A storage area and enclosure for refuse, recyclables, and returnables shall be maintained free of unnecessary items and kept clean. The manager has agreed to replace the two containers and to have grease pans installed below the grease barrels within 48 hours. In addition, the manager agreed to clean the grease spill off the parking lot within 48 hours. A follow-up will be conducted within 10 days to determine compliance.
