REMINDERS:
1) Make sure you mark start time and discard time for cooked pizza. It should be discarded after 4 hours.
2) Monitor employees for hand washing and sanitizing practice.
3) Check dish machine at least once a day to make sure it is sanitizing.
4) Check all refrigerator temperatures and food inside at least twice a day and write down the temperatures.
5) Do not overfill containers on prep top. Make sure pizza sauce, cheese and other PHFs are cooled to 41F or less before you put on prep top.
6) Fix the 3-vat sink faucet.
7) Clean the vent in walk-in refrigerator.

  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site) (repeated violation)
    Observation: The following food items were observed cold holding at improper temperatures using a calibrated food temperature measuring device: sausage 46F, pizza sauce 53F--3DR--top pizza prep refg.
    Correction: Potentially hazardous foods (time/temperature conrol for safety food) shall be held cold at a temperature of 41°F or below. Note: Put inisde the refrigerator.
  • Critical: Time as a Public Health Control/ Up to 4 Hours for Cold & Hot PHF(TCS) / Requirements (corrected on site) (repeated violation)
    Observation: The following food item(s) for which time rather than temperature is used as a control is not marked or otherwise identified to indicate the time that is 4 hours past the point in time when the food is removed from temperature control.
    Correction: If time only, rather than time in accordance with temperature control, up to a maximum of 4 hours, is used as the public health control the food shall: 1) have an initial temperature of 41°F or less if removed from cold holding temperature OR 135°F or above if removed from hot holding temperature control, 2) be marked or otherwise identified to indicate the time that is 4 hours past the point in time when the food is removed from temperature control, 3) be cooked and served, served if ready-to-eat, or discarded within 4 hours from the point in time when the food is removed from temperature control, and 4) be discarded if unmarked or marked to exceed a 4 hour limit.
  • Critical: Chlorine Sanitizer Solution Concentration based on pH and Water Temp (corrected on site)
    Observation: When tested, the low temperature, chemical sanitizing warewashing machine was observed with a sanitizing rinse concentration of less than 50 ppm chlorine solution. Observed at 10 ppm.
    Correction: Immediately discontinue use of the dishmachine. Until the machine is adequately repaired, set up the three-compartment sink to manually wash, rinse and sanitize food-contact surfaces using a sanitizing solution at the proper concentration of at least 50 to 100 ppm total chlorine. Note: Manually sanitized until it is checked. The manager called techinician.

REMINDERS and RECOMMENDATIONS:
1) Train employees for food safety procedures.
2) Monitor and maintain logs for holding, receiving, reheating, cooling and cooking temperatures.
3) Use timer for the pizza that is held as TPHC 4 hours. OR Maintain a sheet or use post it notes to mark time.
4) Adjust all refrigerators to 36F during summer.

  • Hair Restraint Effectiveness
    Observation: Employees observed working in the food service area without proper hair restraints.
    Correction: Food employees shall wear hair restraints such as hats, visors, hair nets or beard nets to effectively keep their hair from contacting exposed food, clean equipment, utensils, linens, and unwrapped single-service items. This section does not apply to food employees such as counter staff who ONLY serve beverages and wrapped or packaged foods, hostesses, and wait staff.
  • Utensils That are In-Use / Between-Use Storage (corrected on site)
    Observation: Dispensing and/or in-use utensils improperly stored between use as follows: Knives stored in between steam table and refrigerator. Also, plastic bowl used to scoop feta cheese.
    Correction: During pauses in food preparation or dispensing, food utensils shall be stored in one of the following manners: 1) in the food with their handles above the top of the food and the container, 2) in food that is not potentially hazardous (time/temperature control for safe food) with their handles above the top of the food within containers or equipment that can be closed, 3) on a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food contact surface of the food preparation table or cooking equipment are cleaned and sanitized, 4) in running water of sufficient velocity to flush particulates to the drain, 5) in a clean, protected location if the utensils are used only with a food that is NOT potentially hazardous (time/temperature control for safe food), or 6) in a container of water if the water is maintained at a temperature of at least 135°F and the container is cleaned frequently.
  • Critical: Reheat PHF(TCS) to 165°F for Hot Holding (corrected on site)
    Observation: The following food item was not reheated for hot holding to a sufficient temperature and/or time: Marinara sauce on right 107F. The employee did not reheat it before putting it on steam table.
    Correction: Potentially hazardous foods (time/temperature control for safety food) that is cooked, cooled, and reheated for hot holding shall be reheated to a minimum temperature of 165°F for 15 seconds. Note: Reheat on stovetop.
  • Cooling Methods (corrected on site)
    Observation: The following methods used for cooling were not adequate to facilitate proper cooling: Observed gyro meat cooling in deep (~6 inches)container with lid on.
    Correction: Cooling hot foods shall be accomplished as follows: 1) placing the food in shallow pans

    Observation: The following food items were found hot holding at improper temperatures using a calibrated food temperature measuring device: marinara sauce left 121F.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be hot held at a temperature of 135°F or above unless the permit holder is using "time as public health control" as specified under 3-501.19. Note: Reheat to 165F.
  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site)
    Observation: The following food items were observed cold holding at improper temperatures using a calibrated food temperature measuring device: pizza sauce 47F, shredded cheese 45F--3DR prep top. Ricotta cheese 53F, diced tomatoes 57F, garlic with oil 56F--3DR prep refg. Sliced provolone 48F, sliced ham 46F, garlic with oil 47F--2 drawer/1DR refg. sliced cheese--2 drawer/1DR prep top.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth. Note: Put all items in walk-in refrigerator.

    Correction: Repair/adjust the unit so it is able to maintain foods at or below 41°F. Do not use the unit to store potentially hazardous foods while it is not operating properly.

    Observation: Observed that hand soap was not provided at each handwashing sink near 3-vat sink.
    Correction: Provide hand soap at each hand sink to allow employees to properly wash their hands.

Observed the sanitizing solution in the wash cloth bucket with a concentration of 0.0 ppm total chlorine.
    Correction: A chlorine-based sanitizing solution shall have a minimum concentration of at least 50 to100 ppm total chlorine when tested using the appropriate test kit.

    Observation: The Person in Charge (PIC) has proof of successful completion of an approved Certified Food Manager (CFM) exam but does not possess the Northern Virginia Certified Food Manager certificate (photo card) issued by ORS Interactive, Inc.
    Correction: Obtain the photo identification card from ORS Interactive, Inc. (see handout provided) within 10 business days to avoid further enforcement action. A copy of the photo identification card can be faxed to my attention at (703) 385-9568.

The purpose of this visit is to conduct a routine inspection. Discontinue use of dish machine until it is able to maintain correct chemical sanitizer level. Wash, rinse and sanitize all dishware at the three compartment sink.

  • Critical: Eating, Drinking, or Tobacco Use / Proper Food Employee Drinking Containers (corrected on site)
    Observation: Open drinking containers stored in a manner that may contaminate food, food contact surfaces or utensils. OBSERVED OPEN BEVERAGES STORED AT THE SLICER, AND THE DRY STORAGE AREA.
    Correction: Food employees may have beverages in the kitchen areas as long as they are covered and consumed through a straw and stored in a manner that will prevent contamination to food, equipment, utensils, linens, and single service items. MANAGER DISCARDED BEVERAGES.

All outstanding violations are to be corrected within ten (10) days but no later than the time of your first routine inspection (approximately 30 days). Equipment additions/replacement/changes are not allowed without Health Department approval.**
Note the following recommendations:
It is recommended that more than one person on staff be a Certified Food Manager. A Certified Food Manager must be on the premises during ALL food preparation and service.
Recommendation is made to have exhaust hood cleaned at least every six months. Maintain records of hood cleaning for Health Department review. Hood was cleaned in December 2012
Recommendation is made to clean hood filters at least every two weeks.
It is recommended that the grease trap be cleaned at least every two months. Maintain records of grease trap cleaning for Health Department review.
It is recommended that a monthly pest control program be implemented. Maintain records of pest control treatments for Health Department review.

    Observation: The flat-top freezer has a buildup of ice along the front edge of the right-side door.
    Correction: Remove the ice
  • Physical Facilities Good Repair
    Observation: One floor panel in the walkin refrigerator is not secured to the floor
    Correction: the edge is up approximately 1/4".;Secure the floor panel to the underpanel. It is also recommended to seal the grooves(3) between the panels to allow for easier cleaning of the floor
