This was a follow-up inspection to verify compliance regarding the dumpster storage area. The area within the dumpster fence has been cleared and asphalt installed in place.
The establishment is in compliance. Thank you for cooperating. Continue to maintain this area in clean condition at ball times.

  • Refuse / Area / Clean
    Observation: Trash and litter were observed within the fenced area of the dumpster.
    Correction: A storage area and enclosure for refuse, recyclables, and returnables shall be maintained free of unnecessary items and kept clean.
  • Outdoor Refuse Areas, Curbed and Graded to Drain
    Observation: The outdoor refuse area is not curbed or graded to properly drain. The ground cover within the dumpster fence is absorbent and not easy to clean.
    Correction: Provide an area for refuse, recyclables, and/or returnables that is curbed and graded to drain to collect and dispose of liquid waste from the refuse and from cleaning the area and receptacles.
  • Cleaning Frequency for Physical Facilities
    Observation: Observed that the floor under the refriogerator,freezer, and stove is in need of cleaning.
    Correction: Clean the physical facilities at a frequency necessary to ensure the protection and safe preparation of food.

Today's visit was a complaint and routine inspection follow-up regarding unsanitary conditions and disrepair at the dumpster fenced storage area. During the original inspection, a follow-up inspection, was scheduled to take place 30 days from the date of the inspection. However, the Director called our office and requested to have additional time(30 days)and this was granted.
Today's visit is the scheduled follow-up after the additional 30 days and the violation has not been corrected. Additional enforcement will be taken and you will be notified.

    Correction: Receptacles and waste handling units for refuse, recyclables, and returnables shall be kept covered with tight fitting lids or doors if kept outside the food establishment.

    Correction: The food-contact surfaces of cooking equipment and pans shall be kept free of deposits or food residue. Clean the interior of the microwave oven often to prevent attracting insects.

The purpose of today's inspection was to perform a risk factor assessment.
  • Critical: Quaternary Ammonium Compound Sanitizer Solution Concentration per Use Directions (corrected on site)
    Observation: Observed 50 ppm quaternary ammonium in sanitize compartment of three compartment sink.
    Correction: Increase concentration of quaternary ammonium basin to 200 ppm and verify with test strips. NOTE: CFM stated that plates intended for service recently passed through sanitize basin. Plates were not used and resubmerged at proper concentration.
  • Certified Food Manager/Certificate Process
    Observation: CFM presented certificate, but no photo ID certificate.
    Correction: Send evidence of photo ID certificate by 11/17/14.

    Correction: It shall be unlawful to operate a food establishment unless it is under the immediate control of a certified food manager. It is highly suggested that you have more than one employee with the food manager's license. ORS Interactive, Inc. (see handout provided) issues the required photo identification card with proof of successful completion of a certified food manager's exam. Failure to have a certified food manager on site during ALL hours of operation including food preparation, food service, and cleaning/sanitizing of equipment/utensils will result in closure of the establishment. No Certified food managers present. EHS provided training to person in charge on food safety related issues. A follow-up will be conducted within 10 days to ensure that a certified food manager is present.

The purpose of today’s visit was to conduct a routine inspection. ***A Certified Food Manager shall be present during all operating hours. If a Certified Food Manager cannot be on site during operating hours, it is perfectly acceptable to ask parents to provide lunch for children for the day. Ensure that there is an adequate number of certified food managers at the facility to cover all operating hours, including the times when management has meetings and are off-site. A follow-up will be conducted within 10 days to ensure that a Certified Food Manager is present. Trainig was provided to the person in charge during today's visit.

  • Critical: Discard RTE, PHF(TCS) if Not Consumed Within 7 Days at 41°F (Unless Frozen) (corrected on site)
    Observation: Observed the following ready-to-eat, potentially hazardous food being held for more than 7 days at 41°F: American cheese in the True Upright reach in refirgerator dated 1/6.
    Correction: Foods shall be discarded if it: 1) exceeds 7 days or at 41°F (except the time it is frozen)
