This complaint was conducted in conjunction with a Routine Inspection.
Complainant alleges Roaches and Flies in Kitchen, Good Repair of stove with only one functioning burner, and overall Cleaning practices. Discussed complaint with CFM and facility Director, aware of Complaint.
Pest Control Services performed once a month. Unable to view previous month's pest control reports at time of inspection. **Please send copies of Pest Control Invoices to Health Department by 8/5/15.
New shelving delivered for installation today. All shelving units are to be replaced with new units. Work Order placed prior to inspection for new Stove unit. Currently only stove burner is functional and area where previous non-functioning stove burner is open with interior wires exposed. Observed cavity in need of cleaning, however unable to be cleaned due to Electrical Safety. Per CFM, no reports of Sparks or electrical malfunctions have been noted or observed. **Establishment is to send copy of Service Order and notify Health Department when new stove arrives for replacement. Thank you.
Kitchen in process of being cleaned with mop during time of Inspection. Flooring is swept and cleaned daily. Observed areas under Refrigeration unit and Dry Storage areas. Flooring by Dry Storage area in need of thorough cleaning and will be cleaned once shelves are removed and to be replaced later today. Observed all shelving and refrigeration units cleaned. Counter-tops observed clean, with minor coffee staining. All cleaning is performed daily with sanitizer.

No violation noted during this evaluation.

This visit was conducted in conjunction with a Complaint Investigation noted in a separate report.

    Observation: The following refrigerated, ready-to-eat, potentially hazardous food that is commercially processed is not used or discarded within 24 hours of when the original container was opened and is not observed to be date marked: 1) Cheese
    Correction: Refrigerated, ready-to-eat, potentially hazardous commercially processed foods when opened and kept for more than 24 hours cannot have a shelf-life of more than 7 days, including the day of the original container was opened. These food items must be marked with either the "consume-by" date and/or "preparation" date, color-coded labels, or another marking system/method that effectively indicates shelf-life that is clearly understood and properly used by the employees. This date cannot exceed any manufacturer determined "use-by" date based on food safety. PRODUCTS WERE DISCARDED.

    Observation: Observed the following to be in disrepair:
    1) Shelving in the food prep/dry storage area

    Correction: The above mentioned items shall be maintained in good repair.
  • Nonfood-Contact Surfaces Clean at Frequency to Preclude Accumulation (repeated violation)
    Observation: The nonfood-contact surfaces of the following equipment had accumulations of grime and debris:
    1) Shelving,
    2) Exterior of equipment

    Correction: Nonfood-contact surfaces of equipment and utensils shall be cleaned at a frequency necessary to preclude accumulation of soil residues.
  • Physical Facilities Good Repair (repeated violation)
    Observation: Observed that the following is not maintained in good repair:
    1) Floor tiles,
    2) Walls

    Correction: Repair the physical facilities. Poor repair and maintenance compromises the functionality of the physical facilities.

    Observation: Observed that the following is in need of cleaning:
    1) Floor/wall junctures underneath and behind equipment and shelving,
    2) Walls,

PLEASE FAX/EMAIL THE REPAIR INVOICE UPON RECEIPT FOR THE WATER HEATER TO VERIFY THAT THE PROPER HOT WATER TEMPERATURES HAVE BEEN MET.
  • Food Storage / Preventing Contamination
    Observation: Food stored on the floor and/or food stored less than 6 inches off the floor.
    Correction: Food shall be protected from contamination by storing the food at least 6 inches off the floor on approved shelving units or dunnage racks. Milk crates, soda crates, or bread racks are not suitable for food storage.
  • Food Temperature Measuring Devices, F Scale, Accurate within 2°F (corrected on site)
    Observation: The food temperature measuring device located in the following cold or hot hold unit is not accurate in °F: Observed 40F in a 32F ice bath
    Correction: Food temperature measuring devices that are scaled only in Fahrenheit shall be accurate to +/- 2°F in the intended range of use. DEMONSTRATED HOW TO CALIBRATE PROBE THERMOMETER.
  • Equipment / Good Repair / Operation (repeated violation)
    Observation: Observed the following to be in disrepair:
    1) Shelving

    Correction: The above mentioned items shall be maintained in good repair.
  • Manual Warewashing Wash Solution Temperature 110°F or per Manufacturer label
    Observation: Observed the three compartment sink at 72F.
    Correction: The temperature of the wash solution in manual warewashing equipment shall be maintained at not less than 110°F or the temperature specified on the cleaning agent manufacturer's label instructions.
  • Plumbing / Maintained in Good Repair
    Observation: Observed the following to be in disrepair:
    1) Handsink (leaking),
    2) Three compartment sink (leaking)

    Correction: A plumbing system shall be maintained in good repair.
  • Refuse / Cover Receptacles / Outdoors / Tight-Fitting Lids (repeated violation)
    Observation: Outside refuse container was uncovered.
    Correction: Receptacles and waste handling units for refuse, recyclables, and returnables shall be kept covered with tight fitting lids or doors if kept outside the food establishment.
  • Refuse / Area / Clean
    Observation: Trash and litter were observed adjacent to the refuse container outside the facility.
    Correction: A storage area and enclosure for refuse, recyclables, and returnables shall be maintained free of unnecessary items and kept clean.

A separate training was provided on Norovirus and Influenza in addition to the risk factor assessment.
  • Certified Food Manager/Certificate Process
    Observation: The person in charge (PIC) has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc.
    Correction: Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. (see handout provided) within 10 business days. A copy of the photo identification card can be faxed to the food safety section at (703) 385-9568. Failure to provide a copy of the photo identification card will result in further enforcement action.
  • Hand Drying Provision / Individual, Disposable Towels (corrected on site)
    Observation: Observed that paper towels were not provided at each handwashing sink.
    Correction: Provide paper towels at each hand sink to allow employees to properly dry their hands after handwashing.
  • Critical: Refrigerated Medicines / Container Requirements (corrected on site)
    Observation: Refrigerated medicines for employees or children are not being stored in leakproof container.
    Correction: Refrigerated medicines shall be stored in leak proof containers to prevent food contamination. MEDICATIONS WERE REMOVED TO AN EMPLOYEE USE REACHIN COOLER.

    Correction: Food packages such as canned goods shall be in good condition and protect the integrity of the contents so that the food is not exposed to adulteration or potential contaminants. Return all dented and damaged canned goods to your food supplier for a credit.
