Overall mixed follow up results. Food equipment working for good hot holding but processes for thick foods mashed potatoes and mac and cheese for hot well need to be examined and improved. Hot well foods such as green beans and gravy with good hot hold temperatures but mashed potatoes and mac and cheese both temperature checked at 130, 131 F < 2 hrs. Discussed with manager, reheat to 165 F then put in hot hold. Review process for both foods - mashed when made temperature checked at 136- 139 F, mac and cheese from the pan to dip up 157 F. Noted table top hot box with broken latch due for repair - suggest tie shut until repaired. Good handwashing with soap present. Employee health reviewed, good symptom with return knowledge, good disease knowledge - mixed knowledge on when to return from diagnosed reportable disease or close contact with reportable disease. Dish sink agitator on and working. **Lobby drink machines still with leaks - per manager drain lines in wall between lobby drink machines. **Contact EHS with planned repair time / date and process that will be used.

    Observation: *Lobby drink machines leaking - one machine with most significant leak out of order, other in use. Leak will require plumbing repair, per manager, drain line inside of wall between drink machines not draining properly.
    Correction: Contact EHS with plumbing information - what repair is needed, when it is planned, and plan to prevent contamination in facility. Dishmachine, grill oven leak, and freezer door repairs done.

    Observation: Drain system cleaned out by plumber. Report pending. Noted troughs with some deteriorated grouting and some broken tiles. Drain covers for drain line in middle troughs pending installation. Food debris present in troughs from previous evening.

Overall mixed inspection. Some equipment issues present that directly and indirectly impact food safety. Good management that maintenance already scheduled to arrive this morning. Most staff without required district food handler cards. Per shift manager, no general manager - and now no area coach. Discussed, all staff must have current district food handler cards. Original time frame given, extended through this week. Some temperature issues from procedural problems and some from equipment issues. Sanitizer present at 300 ppm quats at prep stations and used. Good chicken prep process. Good handwashing and glove use **But on arrival both handwashing sinks without handwashing soap - only hand sanitizer present. No refills present, bottles of soap placed during inspection. Employee health with mixed knowledge. Dish sink that normally constantly agitates water helping loosen food and clean dishes not working, directly impacting ability to keep cook ware and insert containers, utensils etc. clean and sanitized. Follow up in 3 days for food handler status and status of sink repair. Contact EHS when other equipment repair done.

    Correction: Reviewed 5 reportable diseases, and steps to take for diagnosis or close contact and return times. Flyer left.
  • Critical: Food - Potentially Hazardous Food - Hot Holding* (corrected on site)
    Observation: Foods placed on steam line before line was turned on and able to keep food hot tested at 121 for mashed potatoes and corn x 30 minutes. Chicken tenders in new hot hold bins at very front of tray tested in 120's, chicken in middle of bin hot holding adequately at 148 F. Mashed potatoes and gravy in to go containers in top section of large hot hold box checked at 134 - 146 F.
    Correction: Foods from steam line - mashed, corn, gravy, green beans, mac and cheese, reheated in microwave and checked for reheated over 165 F x 15 secs before replacing. Chicken tenders already using time for quality and when chicken changed, trays also changed, each hour so food safe by time. Mashed potatoes and gravy in top section of hot box already uses time for quality with max hold of 4 hours - all food in to go containers, so food already safe by time. Repairman looked and stated top of unit overstocked. New food will be after next batch put in place.

    Observation: The following equipment issues noted:
    *Dish machine motor with wires detached - not working.
    *Lobby drink machines leaking.
    *Grill Oven with drain problem
    *Lowboy freezer with door missing - replacement due to arrive this Thurs
    Hot box if temperature issues not corrected from storage.

    Correction: Repair equipment to operate as intended.
  • Equipment - Food Contact Surfaces, Nonfood Contact Surfaces, and Utensils
    Observation: Observed accumulations of dust, dirt, food residue or other debris on the following nonfood-food contact surfaces: Ventilation covers in lobby, light cover next to ventilation output, cabinets under lobby drink machine.
    Correction: Ensure ventilation system maintenance is routinely done to include ductwork, filters and covers. Clean cabinets.
  • Prohibitions - Single Service
    Observation: Extra single service lids stored in cabinets under lobby drink machines where drain lines are located.
    Correction: If lids or other single service items ie napkins, straws are stored in cabinets, place in durable plastic containers with lids, ie Rubbermaid etc. to prevent contamination of items.
  • Plumbing System Maintained in Good Repair
    Observation: Drain system in kitchen with floor troughs with drain line in middle of bottom. Troughs with metal grates at top, but drains without covers allowing pieces of food and debris into drains. Very foul smell at drains located by dish sink and in back room with chemical storage. Facility with history of drains with bad odor -during plumbing maintenance, drains with camera to check, found partial blockage with rotting debris and food - but not enough to back up drains. Pieces of raw chicken noted at top of drain line by dish sink.
    Correction: Per manager, drain covers have been placed repeatedly, but are either taken for scrap or discarded when cleaning. Suggest: Have drain lines cleaned out *especially back areas with very bad odor. Install drain covers that are screwed into place and must be removed with tools to deter theft or discarding. Please provide copy of plumbing report for cleaning drain lines to EHS for file and call when covers are in place.
  • Handwashing Cleanser - Availability (corrected on site)
    Observation: Soap was not provided at the hand washing lavatory in the front and middle sinks (sinks that are most frequently used).
    Correction: Dishsoap diluted and put in bottles at each sink until additional hand soap is received. Hand soap must be provided at all hand washing lavatories to encourage proper hand washing. Hand cleanser must always be present to aid in reducing microorganisms and particulate matter found on hands.

Sanitizer in place at 200 ppm quats and used. and mostly good employee health knowledge with some review of reportable diseases needed.     -Chicken walk in refrigerator with bottom of walls not sealed
    -New tabletop hot box door handle loose
    -Heat strip on walk in freezer door pending replacement - with door sometimes icing up and open 1-2 inches
    -Lobby Soda machine - leaking

    Correction: Repair or replace broken items.
  • Physical Facilities in Good Repair (repeated violation)
    Observation: Grouting deteriorated.
    Correction: Replace grouting / tiles in poor condition.
  • Pests - Controlling Pests* (repeated violation)
    Observation: 5-6 flies noted in facility, drive through window kept closed when not serving customers, facility clean and although trash is full, it is not overfilled. Noted Dumpster closed on arrival, when checked, unit just emptied, with rotted liquid in bottom and bottom back edge of dumpster rusted out and accessible to flies. Noted most drain covers for floor "troughs" missing allowing access to drain water by pests, and allowing food particles and other debris to go down drains possibly creating plumbing and pest issues.
    Correction: Have dumpster replaced right away. Unit replaced either spray with fly spray or pour bleach in bottom. Contact EHS when replaced - suggest in no less than 2 weeks. Replace missing drain covers. Contact pest control to see if any other measures can be taken to prevent entry in facility.

Overall mixed inspection. Facility and equipment issues noted that can cause significant problems, but immediate actions taken to ensure good food safety. Dry storeroom with dripping from ceiling, with contamination issues for some single service items, Walk in Refrigerator (Taco Bell / Cooks Box) with good unit temperature but some items cold holding above temperature noted, *Rethermalizer with low water level and food below temperature for reheat/hot hold. Discussed with Regional coach - new procedure in place that requires rethermalizer to be manually filled.* Good handwashing, good glove use, sanitizer present at 200 ppm Quats and used. Facility with written employee health policy, but poor knowledge by new staff. Ensure staff are trained, as well as filling out written policy. EHS will follow up on status of Walk in unit today. Several Staff with outdated or no district food handler cards. Obtain before the end of the month.
  • Critical: Food - Packaged and Unpackaged - Separation, Packaging, and Segregation* (corrected on site)
    Observation: Water dripping from ceiling in dry storeroom with two wet boxes noted and two more with drip marks on single service wrappers.
    Correction: Boxes voluntarily discarded. Place "tub" under leak until repair can be done - possibly AC related, both vent covers by 3 vat sink - but not above dishes and in unused washing machine area, with condensate and no discernible air flow. Ensure leak is repaired, contact EHS.
  • Critical: Reheating for Hot Holding* (corrected on site)
    Observation: Rethermolizer with very low water level and foods "reheating" 117 - 119" surface temps with infrared - and taco pizza sauce on prep line 70 stem temp - placed on line without reheating about 1&1/2 hrs.;Sauce reheated, Rethermolizer filled, and immediately temperature up to 199, food reheat temperature checked > 170 between bags by manger with calibrated thermocouple. Conduct training.

    Observation: Two pans of pork and beans 94 F x 1 hr 45 mins to 2 hrs - stacked on top of each other with lids on, and placed on top of box of bacon bits in walk in refrigerator
    Correction: Either reheat and hot hold or discard to remove cooling foods from unit with cold holding problems with some items, train staff on proper cooling techniques.
  • Critical: Food - Potentially Hazardous Food - Cold Holding* (corrected on site)
    Observation: 1) Walk in refrigerator with unit temperature of 41, but with food along rows, etc. cold holding at 44-45 for 4 pans of pot pie filling since last night, 44, for bottom large bin of slaw > 4 hrs, 49 - 1 pan of pot pie filling prepped (but not out of unit) pan of green beans, 46, 2 pans of pork and beans 90's prepared today ? time, leftover mac and cheese 60 F. All other time and temperature controlled foods temperature checked with thermocouple - internal temps calibrated at 32.0 EHS and 32.4 facility.
    2) Preparation line not functioning properly, freezing items with guacamole and sliced cheese double panned cold holding in 60's < 2 hours.

    Correction: All foods cold holding above 41-43 F > 4 hours voluntarily discarded. Regional mgr contacted by manager - repairmen scheduled to come at 3 pm for prep unit, suggested sooner if possible. All food in walk in unit now good until 3-4 PM using time and discussed with manager, if temperature checked again with probe temperatures in 2 hours, all within temperature good 4 hours. Contact EHS with unit / food status. Use time as a public health control for prep unit guacamole and sliced cheese.

    Observation: Floor in kitchen in poor repair, especially in front of fryers and grouting in other areas including coving.
    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.

    Observation: Some flies noted in facility with the following contributing factors observed:
    Dumpster door open, floor sinks without drain covers, open floor drain from removed buffet line. Spray can be used in dumpster area (never inside facility) and fly strips can be placed in management office, above mop sink and in unused washing machine room - only these places, must be changed at least 1/wk with handwashing.

    Correction: Close dumpster doors, obtain and put drain covers in floor sinks, seal open drain. EHS to facility to follow up for walk in refrigerator / food status. Walk in refrigerator repaired, sensor moved to touch line going to fan unit causing false temperature sensing, and cooling to go on and off and fluctuating unit temperatures. Unit repaired, per shift manager, previous manager checked all time and temperature controlled (TCS) foods at 5:00 PM with probe thermocouple - calibrated during initial inspection and all TCS foods within temperature. Shift manager believes problem creating dripping ceiling in dry storeroom repaired (associated with AC), but uncertain of cause / repair. Contact EHS with update when results learned.

Overall excellent sanitation. Observed excellent handwashing, good glove use, good food temperatures, storage and labeling. One food item, sliced cheese, cold holding above temperature, likely due to storage technique. Either change storage technique to keep food in temperature or use time as a public health control for that item. Sanitizer in place at 200 ppm quats and used. Good management practices checking and recording food temperatures. Excellent employee health knowledge for symptoms and return time, but reportable diseases and close exposure to reportable diseases not known. Flyer given, review entire employee health policy with all staff. Discussed eight major allergens, flyer left. Discussed upcoming requirement to post a statement that inspection reports can be viewed on Virginia Department of Health website. Suggest next to the Health Permit. Noted, ice tea machine out of order, tea must be made manually. Review process with all staff making tea. Buffet closed and pending removal.

    2) Walk in freezer door will not close completely due to ice build up.

    Correction: 1) Ensure wall panels are sealed closed and will not allow water / any liquid from raw chicken to splash behind wall coverings.
    2) Repair unit to prevent ice build up.

    Observation: Double backflow device missing from faucet with hose with nozzle attached.
    Correction: If device cannot be found, detach hose when not in use and obtain double backflow device. Hose in use during inspection.

Overall excellent food safety practices with excellent handwashing and glove use, sanitizer in place and used at 200-300 ppm quats, clean food prep surfaces including clean and sanitized handles and "bump pads", good food temperatures, labeling and storage. Noted permit posted in kitchen area not in view of public. Please post in view of public. Buffet no longer in use, removal date unknown. Ensure unit removed in timely manner to prevent harborage for pests and cleaning impediment.

Clean well organized facility with sanitizer in place at 400 ppm quats and used. Dish sink originally at < 150 ppm quats changed to 200 ppm quats. Noted open drink, not in area but not where facility policy directs. Review policy. Please call when walk in refrigerator door repaired.

Overall excellent sanitation. Excellent handwashing, good glove and utensil use, clean well maintained facility sanitizer in place at 200-300 ppm quats and used, good food temperatures, good reheating practices, good labeling and food storage.
Note: Several staff members with outdated or no Western Tidewater Food handler cards. Ensure all staff members have current Western Tidewater Food Handler Cards right away.
Note: **Lights over the buffet line, some in poor repair with rust present and some broken pieces seen. All bulbs do not appear shatterproof. **Ensure lights are in good repair and bulbs are shatterproof.
Note: Facility previously with odor problem esp in the summer. Per manager, problem found with floor drains missing one of the drain covers, resulting in some drain blockage. New covers ordered, and drain clean outs will be done. No problems with back ups noted.
Note: Some debris noted on equipment handles. Ensure handles are cleaned and sanitized with other food contact surfaces.

No violation noted during this evaluation.
