Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: cheese (51'F) in CIS walk-in cooler due to high ambient temperature of 53'F.
Correction: ALL ITEMS IN CIS WALK-IN COOLER WERE MOVED TO FORMAKOOL WALK IN COOLER. MAJORITY OF ITEMS IN CIS WALK-IN COOLER WAS IN THE PROCESS OF THAWING, THEREFORE THE FOOD ITEM TEMPERATURES WAS 41'F OR BELOW. The purpose of today's visit was to conduct a complaint investigation in conjunction with a routine inspection. The complaint was received by the Health Department on December 27, 2013 via the web. The complainant states their roommate purchased on December 24, 2013 a bucket of chicken from above facility and purchased complainant an individual box lunch. The roommate ate chicken on December 24 had symptoms of stomach pain, chest pain, diarrhea and felt very sick for two days. The complainant ate their individual box lunch on December 25, 2013 and had the same symptoms starting several hours later. Complainant ate a bucket of chicken from above facility the previous week and had no symptoms. Upon arrival EHS discussed complaint with manager. Manager was aware of complaint and shared that no other customers called or reported symptoms to facility. Manager shared that no employees have reported or have been observed ill for the last few weeks. Please see routine inspection report for additional observations. Observation: Observed the following food thawing using an improper method: -----------observed raw chicken in plastic packaging thawing in still water.
Correction: Potentially hazardous foods (time/temperature control for safety food) shall be thawed using one of the following methods: 1) under refrigeration that maintains the food temperature at 41°F or less, 2) completely submerged under running water at a water temperature of 70°F or below with a sufficient velocity to agitate and float off loose particles of food as long as held for period of time at a temperature of 41°F or less, or 3) as part of a cooking process if the food is cooked as specified by Law or thawed in a microwave oven and immediately transferred to a conventional cooking process. MANAGER TURNED ON COLD RUNNING WATER AND DRAINED THE WATER IN SINK BASIN.
- Critical: Quaternary Ammonium Compound Sanitizer Solution Concentration per Use Directions (corrected on site)
Observation: When tested, the quaternary ammonium compound-based sanitizing solution was observed to be at a concentration less than what is required for effective sanitization of food-contact surfaces of equipment and utensils. Observed the sanitizing solution in the sanitize basin and all the wiping cloth buckets with a concentration of less than 150 ppm total quaternary ammonium compound.
Correction: The quaternary ammonium sanitizer used in this facility shall have a minimum concentration of at least 200 ppm OR a concentration between 150-400ppm based on the manufacturer's use directions and using the appropriate test kit. MANAGER CHANGED THE SANITZER SOLUTION BAG SUPPLYING THE SANITIZE BASIN AND OBSERVED AT 200 PPM BEFORE END OF INSPECTION. BASIN AND ALL BUCKETS CHANGED.

The purpose of today’s visit was to conduct a routine inspection. A follow-up inspection will be conducted on Friday, May 3, 2013 to verify that the CIS walkin cooler is able to hold at 41F or below. MAINTENANCE: *Water Heater: AO Smith BT 800 300 which uses 75,100 BTU with posted recovery rate of 72.82 *Dish Machine: N/A, Sanitize basin and wiping cloth buckets observed with acceptable levels of Quat. Observed facility clean and organized.

Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth. MOVED TO CIS WALKIN FREEZER TO RAPIDLY CHILL AND THEN WILL BE MOVED TO FORMAKOOL WALKIN COOLER.

Observation: The following refrigeration unit is not operating properly and is unable to maintain cold food at or below 41°F: ------CIS walkin cooler observed at 50F.
Correction: Repair/adjust the unit so it is able to maintain foods at or below 41°F. Do not use the unit to store potentially hazardous foods while it is not operating properly. NOTE: UNIT HOLDS LIMITED ITEMS, MAJORITY OF MENU ITEMS HELD IN FORMAKOOL WALKIN COOLER OR CIS WALKIN FREEZER.
- Clean Ice Bins, Beverage Dispenser Nozzles, Etc. per Manufacturer Specs.
Observation: Surfaces of ice bins and enclosed components of equipment are not being cleaned as required.
Correction: Surfaces of utensils and equipment contacting food that is NOT potentially hazardous (time/temperature control for safety food) such as ice bins, beverage dispensing nozzles, and enclosed components of equipment such as ice makers, cooking oil storage tanks and distribution lines, beverage and syrup dispensing lines or tubes, coffee bean grinders, and water vending equipment at a frequency specified by the manufacturer or at a frequency necessary to preclude accumulation of soil or mold.
