    Observation: Two food employees failed to wash his or her hands before touching bread after handling raw beef and raw chicken..
    Correction: ALL food employees shall wash their hands OFTEN especially after handling trash, using the restroom, handling raw meats, handling soiled equipment and utensils, BEFORE putting on clean gloves, etc. Proper handwashing frequencies will aid in the reduction of foodborne illness.
  • Wiping Cloths / Use Limitation /Dry
    Observation: Wiping cloths improperly used for the following activity: under cutting board
    Correction: Cloths shall not be used for any other purposes. For example, cloths shall not be placed under cutting boards to prevent the boards from sliding while in use. Note: Provide rubber mats for cutting boards.
  • Wiping Cloths/Use Limitation/Wet Towel Storage (corrected on site)
    Observation: Wet wiping cloths used for wiping counters and other equipment surfaces were observed stored on the counter.
    Correction: Wet wiping cloths are to be used for wiping counters and other equipment surfaces and are to be held in a chemical sanitizer solution at the appropriate concentration between uses.

    Observation: The following methods used for cooling were not adequate to facilitate proper cooling: sliced tomatoes cooling in 2 layers completely covered.
    Correction: Cooling hot foods shall be accomplished as follows: 1) placing the food in shallow pans

    Observation: The following food item(s) for which time rather than temperature is used as a control is not marked or otherwise identified to indicate the time that is 4 hours past the point in time when the food is removed from temperature control. Sliced tomatoes.
    Correction: If time only, rather than time in accordance with temperature control, up to a maximum of 4 hours, is used as the public health control the food shall: 1) have an initial temperature of 41°F or less if removed from cold holding temperature OR 135°F or above if removed from hot holding temperature control, 2) be marked or otherwise identified to indicate the time that is 4 hours past the point in time when the food is removed from temperature control, 3) be cooked and served, served if ready-to-eat, or discarded within 4 hours from the point in time when the food is removed from temperature control, and 4) be discarded if unmarked or marked to exceed a 4 hour limit. Observed the sanitizing solution in the sink with a concentration of less than 150 ppm total quaternary ammonium compound.
    Correction: The quaternary ammonium sanitizer used in this facility shall have a minimum concentration of at least 200 ppm OR a concentration between 150-400ppm based on the manufacturer's use directions and using the appropriate test kit.
  • Sanitizing Solution Test Kit Accurate & Used
    Observation: The operator is not testing the chemical sanitizing solution to ensure proper concentration of the solution.
    Correction: The operator shall be monitoring the concentration of the sanitizing solution using a test kit or other device.

As discussed in addition to the cited violations, corrective actions must be done in accordance with Code Section 4.501.11A, Where-by equipment must be in good working order.
Adjust the ambient temperature of the refrigerators to between 33-39F to have food temperatures below 42F, or with-in 10 days have the refrigeration unit repaired and fax the repair record to our office at phone # 703-653-9448 (LABEL FAX WITH NAME OF RESTAURANT & INSPECTOR, PLEASE). REGULAR TEMPERATURE CHECKS WILL PREVENT FUTURE OCCURRENCES.

FOOD SERVICE WORKER HANDLED MONEY TRANSATION AND PUT GLOVES ON WITH-OUT WASHING HANNDS.
    Correction: ALL food employees shall wash their hands OFTEN especially after handling trash, using the restroom, handling raw meats, handling soiled equipment and utensils, BEFORE putting on clean gloves, etc. Proper handwashing frequencies will aid in the reduction of foodborne illness.

    Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: SALSA 43-44F & PASTRAMI 44-45F @ #2.
    Correction: Potentially hazardous foods (time/temperature conrol for safety food) shall be held cold at a temperature of 41°F or below. MOVED PASTRAMI & SALSA.
  • Critical: Quaternary Ammonium Compound Sanitizer Solution Concentration per Use Directions (corrected on site)
    Observation: When tested, the quaternary ammonium compound-based sanitizing solution was observed to be at a concentration less than what is required for effective sanitization of food-contact surfaces of equipment and utensils. Observed the sanitizing solution in the wet wash cloth with a concentration of 0.0 ppm total quaternary ammonium compound.
    Correction: The quaternary ammonium sanitizer used in this facility shall have a minimum concentration of at least 200 ppm OR a concentration between 150-400ppm based on the manufacturer's use directions and using the appropriate test kit.     Observation: Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy." HAS INFORMATION BUT HAS NOT COMPLETED UPTODATE.
    Correction: A complete employee health policy must have the following elements:
    1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout)

    Observation: Observed that the continuous towel system used at the handwashing sinks are not providing clean towels and/or is not operational.
    Correction: Repair the continuous towel system. Continuous towel systems shall supply the user with a clean towel to allow employees to properly dry their hands after handwashing.
06/06/2014Routine
Because unit # 5 (Front cold well) open topped cooler tubs once again had warmer than required temperatures for cold holding, even though the unit itself was well under 32F, a Time as a Public Health Control Procedure was agreed to. The CFM has signed the agreement (now in the file) for cut tomatoes and lettuce, to handle these items.
  • Critical: Gloves/Use Limitation/No Re-Use/Use For Only One Task (corrected on site)
    Observation: Single-use gloves worn during multi-tasked food preparation. Observed worker handle raw steak and procede with sandwich making, including handling sub roll.
    Correction: If used, single-use gloves shall be used for only ONE task and used for no other purposes. The single-use gloves shall be discarded when damaged or soiled or when interruptions occur in the operation.
  • Critical: Quaternary Ammonium Compound Sanitizer Solution Concentration per Use Directions
    Observation: When tested, the quaternary ammonium compound-based sanitizing solution was observed to be at a concentration less than what is required for effective sanitization of food-contact surfaces of equipment and utensils. Observed the sanitizing solution in the 3 compartment sink and wash cloth buckets with a concentration of 75 ppm total quaternary ammonium compound.
    Correction: The quaternary ammonium sanitizer used in this facility shall have a minimum concentration of at least 200 ppm OR a concentration between 150-400ppm based on the manufacturer's use directions and using the appropriate test kit.

The purpose of this visit is a risk factor assessment. A suggestion is to ensure that bulk meats and cheeses are placed into containers and covered when being stored in the refrigeration units (not during cooling of the roast beef or tomatoes) and when not being used Also, please obtain a letter certifying that the beef roasts prepared in your facility are whole muscle intact beef roasts and fax a copy of this letter to ATTN: Lois at 703-385-9568 within 10 days.

    Correction: The quaternary ammonium sanitizer used in this facility shall have a concentration of 200 ppm based on the manufacturer's use directions.
  • Handwashing Signage at Handwashing Facilities Required
    Observation: Observed that sign or poster that notifies food employees to wash their hands is not provided at all handwashing sinks used by food employees.
    Correction: Provide a sign or poster at all handwashing sinks used by food employees that reminds food employees to wash their hands. The facility has been in operation for less than a week and per the manager, the handwashing plaques have not been hung yet. Hand the handwashing signs.
  • Critical: Storage of Toxics, Not Above Food, Linens, Equip., Single-Svc. (corrected on site)
    Observation: Observed that poisonous and toxic materials are not properly stored to prevent the contamination of food, equipment, utensils, linens or single service items. Observed spray bottles of cleaners hanging on a shelf above single service items
    Correction: All poisonous and toxic materials shall be stored below food, equipment, utensils, linens, and single service items to prevent contamination.
