This visit was made to conduct a follow-up for a fire complaint investigation. Facility has made all the necessary repairs. The roof has been repaired, all ceiling tiles have been replaced and the hood system is in operation. This visit was made to conduct a fire complaint investigation. Fire Marshal notified Alexandria Health Department of a fire due to a grease fire. Smoke damage occurred out into the dining room. Facility voluntarily closed and foods were being discarded at times of inspection. The following must be corrected before facility can reopen: - Discard all open food in the kitchen, prep area and dining room - Discard all single service/to-go items throughout facility including dry storage area - Discard grease in fryers - Repair roof and hood system - Replace all missing ceiling tiles - Wash, rinse and sanitize all walls and surfaces including cook line, equipment, counter tops/prep tables, refrigerators/prep units ** Please contact A. Observation: Observed food employees using improper handwashing procedures (observed two employees not using proper handwashing procedure: did not wash for a full 20 sec, did not use paper towel to turn off faucet) Proper handwashing was demonstrated
Correction: ALL food employees shall wash their hands in the following manner: 1) remove and discard used gloves, 2) turn on warm water at the hand sink, 3) rinse hands/arms with warm water, 4) lather hands/arms with soap and scrub for at least 20 seconds, 5) rinse hands/arms with warm water, and 6) dry hands/arms with paper towels and then turn off the water at the hand sink using the paper towel. Effective handwashing will aid in the reduction of foodborne illness and is an important part of the food employee training program.

Observation: Observed the lack of an approved backflow prevention device on the water supply system (backflow prevention device under constant pressure with hose valve attachment).
Correction: A plumbing system shall be installed to preclude backflow of a solid, liquid, or gas contaminant into the water supply system at each point of use at the food establishment, including on a hose bib if a hose is attached or on a hose bib if a hose is not attached and backflow prevention is required by Law by installing an approved backflow prevention device as specified under 5-202.14.

Observation: The following food item(s) were measured at improper temperatures: country ham, sausage, turkey sausage, eggs, sliced cheese and sliced ham. The out of temperature hams, sliced cheese, eggs and turkey bacon were discarded and the salad dressing was relocated.
Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below.
- Equipment / Good Repair / Operation
Observation: The following mechanical refrigeration unit is not currently operating as required to cold hold food at a temperature of 41°F or less: The Trauslen upright refrigerator and the make table refrigerator at the cook line area not holding the required temperature. Use an ice bath under the PHFs or do not use refrigeration units for PHF items. Repair refrigeration units immediately.
Correction: Refrigerators shall be capable of holding foods at a temperature of 41°F or below. Repair the unit(s) immediately. Foods exposed to temperature abuse for over 2 hours shall be discarded, otherwise, the foods shall be stored in properly operating refrigerators.
- Controlling Pests by Routine Inspection of Premises
Observation: The premises is not being routinely inspected for the presence of insects, rodents, and other pests. Observed fruit flies in the rear storage area. Provide pest control to eliminate fruit flies.
Correction: Insects and other pests are capable of transmitting disease to people by contaminating food and food-contact surfaces. Routinely inspect the premises to control the presence of pests on the premises.
- Critical: Restricted Use Pesticides, 40 CFR Criteria
Observation: Restricted use pesticides do not meet the requirements in 40 CFR 152 Subpart 1 Classification of Pesticides. Raid was improperly stored in the food storage area. The Raid was relocated during the inspection.
Correction: Restricted use pesticides that do not meet the requirements of 40 CFR 152 may be toxic, which may result in the contamination of food. These pesticides shall not be used.

Possible one person foodborne illness due to meal eaten on Dec. 23, 2012 of waffles with whipped cream, strawberries and milk. Unconfirmed illness but sibling was diagnosed with E. coli 4 days prior to this person's illness. Spoke with the manager and no one reported ill during exposure period of 12/21 to 12/26/2012. Observation: An exterior door is not self-closing and tight-fitting. This may allow the entry of insects or rodents. Self-closing device needs to be adjusted.
Correction: All exterior doors must be sold and tight-fitting, and must be self-closing. Exterior doors used only as emergency exits do not need to be self-closing.

Observation: Drain cover not secure in the dish washing room.
