The purpose of this visit was to conduct a routine inspection. Chlorine sanitizer dishmachine: 125 ppm. Provided sample TPHC logs for front display case.
  • Critical: Handwashing / When to wash hands (corrected on site)
    Observation: A food employee failed to wash his hands after touching raw pork. Employee wiped hands on apron and continued.
    Correction: ALL food employees shall wash their hands OFTEN especially after handling trash, using the restroom, handling raw meats, handling soiled equipment and utensils, BEFORE putting on clean gloves, etc. Proper handwashing frequencies will aid in the reduction of foodborne illness. NOTE

  • Critical: Responsibility of PIC to Require Food Employees to Report GI Symptoms or Cuts, Disease, Medical Condition or Exposure to PIC (repeated violation)
    Observation: Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy.
    Correction: A complete employee health policy must have the following elements:
    1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout)

    Observation: Raw animal food stored over ready-to-eat food in the walkin refrigeration units. Raw shrimp was stored above fried noodles in the walk-in near the woks. Raw chicken was stored above cooked noodles and vegetables in the walk-in refrigerator adjacent to the walk-in freezer.
    Correction: Raw foods like poultry, beef, pork, seafood, and eggs shall be stored properly inside refrigerators and freezers to prevent cross contamination. Raw foods shall be stored on shelving units BELOW or SEPARATED from all cooked foods, commercially processed foods, washed produce, leftover foods, etc. FOODS WERE REARRANGED TO PREVENT CROSS CONTAMINATION. RTE FOODS WERE ALREADY SECURELY COVERED. A HANDOUT DETAILING PROPER FOOD STORAGE IN REFRIGERATION UNITS WAS PROVIDED IN ENGLISH AND CHINESE LANGUAGES.
  • Critical: Food Protection from Contamination / Separate Types of Raw Animal Foods (corrected on site)
    Observation: Different types of raw animal foods stored in such a manner that may cause cross contamination as follows: Raw poultry was stored above raw pork and beef in the walk-in refrigerator adjacent to the walk-in freezer.
    Correction: Raw animal foods shall be sorted by their "type" and cooking temperatures while in storage inside a refrigerator or freezer. For example, if using ONE shelving unit for storage of ONLY raw foods, the foods shall be sorted in this manner: 1) raw poultry (chicken, turkey, or duck) shall be stored on the BOTTOM shelf (165°F cook temperature), 2) raw ground beef and ground pork shall be stored on the shelf above poultry (155°F cook temperature), and 3) raw pork (chops, ribs), seafood, and shell eggs shall be stored on the top shelf above beef (145°F cook temperature). FOODS WERE REARRANGED.
  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site) (repeated violation)
    Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: Bean sprouts (55 F), soft crabs (48 F), raw pork (48 F) in three door prep top cooler near prep area. Above temperature foods in the three door prep top cooler near prep area were recently removed from the cooler for food preparation according to the CFM, and the unit was maintaining an acceptable interior air temperature (35 F).
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below. METHODS AND PUBLIC HEALTH IMPORTANCE OF PROPER COLD HOLDING WERE DISCUSSED WITH CFM. FOODS WERE RELOCATED TO THE INTERIOR OF PREP COOLER TO MAINTAIN PROPER TEMPERATURE.
  • Critical: Equipment / Food-Contact / Visibly Clean (corrected on site) (repeated violation)
    Observation: Cleavers contaminated with meat juices were stored in a block beneath the cookline. A slicer was observed contaminated with dried-on vegetable pieces from the previous night. Clean utensils were stored inside a dirty bucket at the beverage station. Soiled food smoking hooks were stored in the food smoking area.
    Correction: Equipment food-contact surfaces and utensils shall be clean to the sight and touch. A surface that is soiled cannot be effectively sanitized. EQUIPMENT AND UTENSILS WERE CLEANED AND SANITIZED.
  • Handwashing Sink / Used for Hand Wash Only (corrected on site)
    Observation: Observed a strainer containing used beverage ice and straws in the handsink at the beverage station. Wet sponges were observed in several handsinks throughout the facility.
    Correction: Handsinks are to be used only for washing hands to encourage proper hand washing and to prevent contamination of cleaned hands. ITEMS NOT USED FOR HANDWASHING WERE REMOVED FROM HANDSINKS.
  • Hand Drying Provision / Individual, Disposable Towels (corrected on site) (repeated violation)
    Observation: Observed that paper towels were not provided at each handwashing sink. No paper towels were present at the handsink at the beverage station. A roll of paper towels still sealed in the manufacturers plastic wrap was observed at the handsink near the food smoking station.
    Correction: Provide paper towels at each hand sink to allow employees to properly dry their hands after handwashing. PAPER TOWELS WERE MADE AVAILABLE AT ALL HANDSINKS.
  • Critical: Sanitizers, 21 CFR Criteria for Food Contact (corrected on site)
    Observation: The concentration of the chlorine sanitizing solution was measured to exceed 200 ppm. This concentration exceeds the approved limits for use with food-contact surfaces of equipment and utensils in a foodservice establishment.
    Correction: Maintain the concentration of chlorine in the sanitizer solution between 50 and 150 ppm. Verify concentration frequently using the appropriate test kit. SOLUTION WAS DILUTED TO ACCEPTABLE CONCENTRATION.

    Observation: There is no Certified Food Manager present at the beginning of the inspection.
    Correction: It shall be unlawful to operate a food establishment unless it is under the immediate control of a certified food manager. It is highly suggested that you have more than one employee with the food manager's license. ORS Interactive, Inc. (see handout provided) issues the required photo identification card with proof of successful completion of a certified food manager's exam. Failure to have a certified food manager on site during ALL hours of operation including food preparation, food service, and cleaning/sanitizing of equipment/utensils will result in closure of the establishment. THE CFM ARRIVED WITHIN FIVE MINUTES OF THE BEGINNING OF THE INSPECTION.

EHS provided manager with employee health policy, hand washing signs, and cross-contamination handouts.

  • Critical: Responsibility of PIC to Require Food Employees to Report GI Symptoms or Cuts, Disease, Medical Condition or Exposure to PIC (corrected on site)
    Observation: Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy.
    Correction: A complete employee health policy must have the following elements:
    1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout)
  • Critical: Molluscan Shellstock/Maintaining ID/Tags Attached until Container Emptied (corrected on site)
    Observation: Tags missing from the molluscan shellfish containers.---Tag missing from the clam container.

    Observation: Raw animal food holding in a manner that may cause cross contamination of ready-to-eat food as follows: raw pork over sticky rice, raw fish over vegetables.
    Correction: Food shall be protected from cross contamination by separating raw animal foods like poultry, beef, pork, seafood, and eggs during holding from ALL other foods such as cooked foods, ready-to-eat foods, washed produce, etc.---Items above was moved above the raw pork and fish.
  • Utensils That are In-Use / Between-Use Storage (corrected on site)
    Observation: Dispensing and/or in-use utensils improperly stored between use as follows: cooking utensils in standing water.
    Correction: During pauses in food preparation or dispensing, food utensils shall be stored in one of the following manners: 1) in the food with their handles above the top of the food and the container, 2) in food that is not potentially hazardous (time/temperature control for safe food) with their handles above the top of the food within containers or equipment that can be closed, 3) on a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food contact surface of the food preparation table or cooking equipment are cleaned and sanitized, 4) in running water of sufficient velocity to flush particulates to the drain, 5) in a clean, protected location if the utensils are used only with a food that is NOT potentially hazardous (time/temperature control for safe food), or 6) in a container of water if the water is maintained at a temperature of at least 135°F and the container is cleaned frequently.---The standing water was poured out.

    Observation: Observed the following food thawing using an improper method: raw fish thawing in standing water.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be thawed using one of the following methods: 1) under refrigeration that maintains the food temperature at 41°F or less, 2) completely submerged under running water at a water temperature of 70°F or below with a sufficient velocity to agitate and float off loose particles of food as long as held for period of time at a temperature of 41°F or less, or 3) as part of a cooking process if the food is cooked as specified by Law or thawed in a microwave oven and immediately transferred to a conventional cooking process.

    Observation: There is no test kit located in the facility for monitoring the concentration of the chemical sanitizing solutions.

    Observation: There is no Certified Food Manager present at the beginning of the inspection.
    Correction: It shall be unlawful to operate a food establishment unless it is under the immediate control of a certified food manager. It is highly suggested that you have more than one employee with the food manager's license. ORS Interactive, Inc. (see handout provided) issues the required photo identification card with proof of successful completion of a certified food manager's exam. Failure to have a certified food manager on site during ALL hours of operation including food preparation, food service, and cleaning/sanitizing of equipment/utensils will result in closure of the establishment.

SCOPE OF RENOVATIONS:
*Expansion of an existing restaurant into an adjusted tenant space, adding extra dining room, 2 toilets, a walk-in refrigerator, dry goods storage room, a dancing space and waitress station.
INSPECTION:
1. Smooth, non absorbent ceiling finishes have not been provided at two rest rooms, dry goods storage, and waitress station area.
2. PA paper towel dispenser has not been installed at the waitress station hand washing sink.
3. A mop rack has not been installed at the mop sink.
4. Shelves have not been installed at the new walk-in refrigerator.
5. Shelves have not been installed at the dry goods storage room.
*Not approved for a Health Department Permit. Owner shall correct the above listed items and schedule for a final follow-up Health Department inspection.
