This visit was made to conduct a routine food safety evaluation. There was not food prep going on a the time of inspection.
A Big 6 poster was given and discussed.
Great job with the repairs and painting of the floors, walls, ceiling in the main kitchen.
A hand washing sink has been installed, but is not connected to the water yet. This will have to be connected in order for the kitchen to be approved to be rented out to another establishment. The floor will also need to be repaired to be smooth and easily cleanable.
If you wish to use the satellite kitchen, a hand washing sink will need to be installed as well. And, the floor, walls, and ceiling will need to be repaired to be smooth and easily cleanable.
Please contact me or Rachel when you wish to have an inspection completed to approve the kitchen to be rented out to another establishment.

  • Physical Facilities Good Repair (repeated violation)
    Observation: Observed that the floor in the main kitchen and near the fridge in the satellite kitchen is not maintained in good repair.
    Correction: Repair the physical facilities. Poor repair and maintenance compromises the functionality of the physical facilities.

  • Critical: Hands and Arms / Cleaning Procedure (corrected on site)
    Observation: Food handlers did not wash for a full 20 seconds or use paper to turn off faucet after washing hands.
    Correction: -ALL food employees shall wash their hands in the following manner:
    1. remove and discard used gloves,
    2. turn on warm water at the hand sink,
    3. rinse hands/arms with warm water,
    4. lather hands/arms with soap and scrub for at least 20 seconds,
    5. rinse hands/arms with warm water, and
    6. dry hands/arms with paper towels and then turn off the water at the hand sink using the paper towel.
  • Critical: Food Protection from Contamination / Separate Raw from RTE (corrected on site)
    Observation: Raw animal food holding in a manner that may cause cross contamination of ready-to-eat food as follows: raw shell eggs over sour cream tubs in refri.
    Correction: Food shall be protected from cross contamination by separating raw animal foods like poultry, beef, pork, seafood, and eggs during holding from ALL other foods such as cooked foods, ready-to-eat foods, washed produce, etc.
  • Physical Facilities Good Repair (repeated violation)
    Observation: Observed that the ceiling and floor tiles is not maintained in good repair (prep table under damaged ceiling was moved)
    Correction: Repair the physical facilities. Poor repair and maintenance compromises the functionality of the physical facilities.
  • Drying Mops (corrected on site)
    Observation: Mops and brooms are not hung up to air dry.
    Correction: Wet use maintenance items such as mops must be hung in a position that allows them to air dry so they are not allowed to grow contamination which can then be spread through the establishment at the next use.

  • Food Storage Containers, Identified with Common Name of Food
    Observation: Unlabeled food containers with the following food items that are not easily identified by appearance: flours, sugar in bulk containers
    Correction: ALL food and ingredients removed from their original container and stored in other food containers, squeeze bottles or "shakers" shall be labeled properly using the common name of the food (if the food or ingredient is not easily distinguished). For example, all cooking oils, salt, sugar, flour, starch, spices, herbs, etc. shall be labeled.
  • Date Marking of RTE, PHF(TCS) Prepared On-Site, Stored at 41°F or less (corrected on site)
    Observation: The following ready-to-eat, potentially hazardous food in the refrigeration unit is not used within 24 hours of preparation and is not properly dated for disposition within 7 days at 41°F: some foods in reach in that were prepared saturday
    Correction: Refrigerated, ready-to-eat, potentially hazardous foods (time/temperature control for safety food) that are prepared on site and held at 41°For below shall be clearly labeled and date marked with a "USE BY" not exceeding 7 TOTAL DAYS. If the food is held for only 24 hours, no date marking is required. For example, if foods like leftover soups, chili, tuna salad, potato salad, pasta salad, etc. are prepared on 12/1 they shall be date marked with a "use by" date not exceeding 12/7.

    Observation: Observed that the floor tiles and ceiling are not maintained in good repair.
    Correction: Repair the physical facilities. Poor repair and maintenance compromises the functionality of the physical facilities.

This visit was conducted to complete a routine food safety evaluation, and to discuss the requirements and renovations necessary to allow the business model to change to allow it to be rented by the hour.
Very limited food preparation was occurring at the time of the inspection, temperatures taken were noted to be correct and food handlers had an adeqaute knowledge of key food safety issues. As discussed, your risk priority rating will be changed to reflect the types of food that you produce.
A separate report has been generated regarding the upgrades that would be necessary to facility the change in business model.

No violation noted during this evaluation.

Any person or entity using your facility as a commissary must obtain a permit from the Alexandria Health Department. Keaving Kabobs is operating illegally and must obtain a permit immediately.
Follow the Fire Marshal's guide lines for exhaust and ventilation.

  • Single-Service and Single-Use Articles / No Reuse
    Observation: Single-service items were observed reused for the storage of muffins. Cardboard boxes can not be used to muffins.
    Correction: Single-service items may not be reused.
  • Critical: Food-Contact Surface Sanitized before Use after Cleaning
    Observation: The food-contact surfaces of the following equipment were not observed sanitized: muffin pans. Equipment must goo through all 3 steps of the dish washing process..
    Correction: In conjunction with Section 4-703.11, the food-contact surfaces of equipment and utensils shall be sanitized before use and after cleaning.
  • Toilet Rooms, Enclosed / Self-Closing Door
    Observation: The toilet room is not provided with a self-closing door.
    Correction: Provide a self-closer for the toilet room door. Completely enclosed toilet facilities minimize the potential for the spread of disease by the movement of flies and other insects between the toilet facility and food preparation areas.
  • Physical Facilities Good Repair
    Observation: Observed that the floors and the ceiling are not maintained in good repair. Repair floors and walls in 30 days.
    Correction: Repair the physical facilities. Poor repair and maintenance compromises the functionality of the physical facilities.
  • Critical: Restricted Use Pesticides, 40 CFR Criteria
    Observation: Restricted use pesticides do not meet the requirements in 40 CFR 152 Subpart 1 Classification of Pesticides. Raid is not approved to be used in this estiblishment.
    Correction: Restricted use pesticides that do not meet the requirements of 40 CFR 152 may be toxic, which may result in the contamination of food. These pesticides shall not be used.
