  • Critical: Food Protection from Contamination / Separate Types of Raw Animal Foods (corrected on site)
    Observation: Different types of raw animal foods stored in such a manner that may cause cross contamination as follows: chicken stored over beef in walk-in cooler (meats). Discussed proper storage with person in charge. Beef and chicken were moved.
    Correction: Raw animal foods shall be sorted by their "type" and cooking temperatures while in storage inside a refrigerator or freezer. For example, if using ONE shelving unit for storage of ONLY raw foods, the foods shall be sorted in this manner: 1) raw poultry (chicken, turkey, or duck) shall be stored on the BOTTOM shelf (165°F cook temperature), 2) raw ground beef and ground pork shall be stored on the shelf above poultry (155°F cook temperature), and 3) raw pork (chops, ribs), seafood, and shell eggs shall be stored on the top shelf above beef (145°F cook temperature).

    Observation: The following food items were observed cold holding at improper temperatures using a calibrated food temperature measuring device: tres leches in dessert cooler observed at 45°F.
    Correction: Potentially hazardous foods (time/temperature conrol for safety food) shall be held cold at a temperature of 41°F or below. Ice used to cool tres leches to 41°F or below.

    Observation: Ice machine was observed in need of cleaning.

The purpose of this visit was to conduct a routine inspection. Certified food manager indicated that Carne asada has been removed from the menu and is no longer being offered. Recommendations were made to monitor dessert refrigerator items. Dessert refrigerator was adjusted to 37°F to help maintain tres leches at 41°F or below.     Observation: Open cup of water stored on shelf above empanada prep table.
    Correction: Food employees may have beverages in the kitchen areas as long as they are covered and consumed through a straw and stored in a manner that will prevent contamination to food, equipment, utensils, linens, and single service items.
  • Utensils That are In-Use / Between-Use Storage (corrected on site) (repeated violation)
    Observation: Observed knives in between equipment and bowls used to scoop salsa and rice.
    Correction: During pauses in food preparation or dispensing, food utensils shall be stored in one of the following manners: 1) in the food with their handles above the top of the food and the container, 2) in food that is not potentially hazardous (time/temperature control for safe food) with their handles above the top of the food within containers or equipment that can be closed, 3) on a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food contact surface of the food preparation table or cooking equipment are cleaned and sanitized, 4) in running water of sufficient velocity to flush particulates to the drain, 5) in a clean, protected location if the utensils are used only with a food that is NOT potentially hazardous (time/temperature control for safe food), or 6) in a container of water if the water is maintained at a temperature of at least 135°F and the container is cleaned frequently.

    Observation: The following methods used for cooling were not adequate to facilitate proper cooling: Observed boiled yuca cooling in deep 4-5 inch deep plastic container at 80F in the walk-in refrigerator.

  • Critical: Consumer Advisory Required for Raw or Undercooked Animal Food
    Observation: A review of the menu with the foodservice operator indicates that there is no consumer advisory for the following food items that may be served raw and/or undercooked: Carne asada on menu board.
    Correction: If an animal food such as beef, eggs, fish, lamb, milk, pork, poultry, or shellfish is served or sold raw or undercooked the permit holder shall inform consumers of the significantly increased risk of consuming such foods by way of a disclosure and reminder using brochures, deli case or menu advisories, label statements, table tents, placards, or other effective written means.

    Correction: Cold or hot holding equipment used for the storage of potentially hazardous foods (time/temperature control for safety food) shall be designed to include and shall be equipped with at least one integral or permanently affixed temperature measuring device that is located to allow easy viewing of the device's temperature display.
  • Equipment / Good Repair / Operation
    Observation: (REPEAT) The following refrigeration unit is not operating properly and is unable to maintain cold food at or below 41°F: short display refrigerator front 48F.
    Correction: Repair/adjust the unit so it is able to maintain foods at or below 41°F. Do not use the unit to store potentially hazardous foods while it is not operating properly.
  • Equipment / Good Repair / Components / Gaskets (repeated violation)
    Observation: The door gaskets of the following unit is damaged: Turbo Air 2DR upright freezer. Also, observed that the door of the True 2DR prep refrigerator in the cookline is damaged and does not close properly.
    Correction: Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications. Fix the door for the 2DR prep refrigerator and replace the gasket for the 1DR upright freezer.
  • Cutting Boards / Resurface / Discard
    Observation: The cutting board along the cookline are heavily scratched and scored. The food contact surface is no longer easily cleaned and sanitized due to condition.
    Correction: Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced if they can no longer be effectively cleaned and sanitized, or discarded if they are not capable of being resurfaced.
  • Clean Ice Bins, Beverage Dispenser Nozzles, Etc. per Manufacturer Specs. (repeated violation)
    Observation: Inner surfaces of ice bins have moldy accumulation on it.

Observation:-(REPEAT) (CORRECTED DURING INSPECTION) The following methods used for cooling were not adequate to facilitate proper cooling: Observed yuca and cooked chicken cooling in deep plastic containers.
Correction: Cooling hot foods shall be accomplished as follows: 1) placing the food in shallow pans

  • Critical: Responsibility of PIC to Require Food Employees to Report GI Symptoms or Cuts, Disease, Medical Condition or Exposure to PIC
    Observation: Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy." Note: New employee was not trained and does not have a signed agreement.
    Correction: A complete employee health policy must have the following elements:
    1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout)
  • Critical: Food Protection from Contamination / Separate Raw from RTE (corrected on site)
    Observation: Raw animal food preparation conducted in a manner that may cause cross contamination of ready-to-eat food as follows: raw shell eggs stored above cooked chicken and sauce --2DR prep refg kitchen.
    Correction: Food shall be protected from cross contamination by separating raw animal foods like poultry, beef, pork, seafood, and eggs during preparation from cooked foods, ready-to-eat foods, washed produce, etc. Do NOT prepare raw foods on the same work table as ready to eat foods.
  • Critical: Raw Animal Foods Cooked to 165°F (corrected on site)
    Observation: The following poultry item was not cooked to a sufficient temperature and/or time to eliminate pathogenic bacteria: Observed rotissaire turkey was at 150, 157, 155F. Note: Put back in oven to finish cooking. Discussed importance of monitoring cooked temperature and mearing temperature at the thickest part of the meat. This turkey was for tasting for owner and employees.
    Correction: Raw animal foods such as poultry, wild game animals, stuffed fish, stuffed meat, stuffed pasta, stuffed poultry, stuffed ratites, or stuffing containing any of these raw animal foods shall be cooked to a minimum internal temperature of 165°F for at least 15 seconds.
  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site) (repeated violation)
    Observation: The following food item were observed cold holding at improper temperatures using a calibrated food temperature measuring device: rice pudding 55F, 58F, flan 57F, 60F--Open display refrigerator. Note: Move all items to 3DR prep refrigerator front. Only few packages were left there for display only. They will not serve the displayed items.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth.

The purpose of today's visit is to conduct a routine inspection.
**Employee Health policy training was conducted last year, however the manager could not find the signed agreements. Provided red folder with handouts in Spanish and English. The employees are aware of the policy and it was confirmed by questioning and talking to employees.
Note:
1) Please fax a copy of updated menu and picture of updated menu board with correct consumer advisory (* sign next to Carne Asada), within 10 days to my attention. Fax# 703-385-9568
2) Please monitor cooling methods and maintain cooling logs. (Training and information on cooling methods, handouts for cooling methods, cooling signs and sample cooling logs provided.)
3) In future kindly get the Health Department approval before purchasing/replacing new dishmachine or water heater.
4) Discussed with the manager that the big pots/utensils that do not fit in the dish machine and 3-vat sink should be sanitized with sanitizing solution in bucket after washing with soap and water.

    Observation: Employees observed wearing jewelry on their arms and hands while preparing food. Observed employee wearing bracelet while preparing food.
    Correction: Food employees shall limit their jewelry to a plain wedding ring while preparing food. Food employees may not wear bracelets, watches, etc. because they can inhibit proper hand washing.

    Observation: Dispensing and/or in-use utensils improperly stored between use as follows: Observed tongs hanging on the grill/hot plate handle. (Tongs touching the employee clothing.)

(Note: Put back in refrigerator). pico de gayo 45F --True 2DR prep refg kitchen. Raw beef 46, raw ribs x2 @44F and 46F, raw steak 42F--walk-in refg #2. (Note: The walk-in was busy and the items were in use.) Discussed about adjusting temperature to 36to 37F for all refrigerators to ensure proper cold holding for foods.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth.

    Observation: The following raw and/or undercooked foods are provided on the menu, menu board without proper disclosure: Carne Asada taco. The statment is provided but the (* ) is not placed adjacent to carne asada on menu and menu board. Note: The manager will manually add the * on menu and menu board send me a picture by email or fax.
    Correction: The Consumer Advisory shall include a disclosure statement that includes a description of the animal-derived foods as raw/undercooked or identification of the animal-derived foods by asteriking (*) them to a footnote that states that the items are served raw or undercooked, or contain raw or undercooked ingredients.
