The purpose of this visit is to investigate a complaint of cockroaches allegedly being observed inside businesses within this strip mall. The complaint was received by our office on September 15, 2014. EHS inspected areas under boxes, near pipes below the three vat sink, near the mop sink, along drain pipes, under shelving units and along baseboards and ceiling/wall junctures. No live or dead roaches observed during today’s visit. Pest control is conducted every three weeks to one month. Last inspections took place on 8/11/14 and then again on 9/15/14. Manager was not aware of the complaint. It is suggested that the businesses within this strip mall coordinate the pest control inspections to take place near the same day/time to reduce the chance that the cockroaches may relocate to a space within a business while pest control take place at a neighboring business. I have provided you with a guidance document to educate you in how to reduce the chance of a cockroach infestation. The complaint cannot be confirmed and is closed. The purpose of this visit is for a complaint of cockroaches being observed within the shopping center where this facility is located and complainant suspects that cockroaches may be coming from this facility. During inspection, no live or dead cockroaches observed during today's visit. Facility has monthly pest control inspections. Management was aware of the complaint and states that he did observe on live cockroach in early June and that he responded by calling pest control and obtaining inspection and treatment. Manager had pest control report available for EHS to observed. Because the EHS did not observed live or dead cockroaches during this inspection, the complaint is closed and is not confirmed.
