The purpose of today's visit was to conduct a routinet inspection. Employee Health policy in place. Observed proper handwashing during inspection today. Employee Health training documents and all customers orders are cooked to 165F or above. Thank you.   • Critical: Eating, Drinking, or Tobacco Use / Proper Food Employee Drinking Containers (corrected on site) (repeated violation)
    Observation: Open drinking containers stored in a manner that may contaminate food, food contact surfaces or utensils.
    Correction: Manager discarded the open drinks at the cookline. Food employees may have beverages in the kitchen areas as long as they are covered and consumed through a straw and stored in a manner that will prevent contamination to food, equipment, utensils, linens, and single service items.
  • Food on Display Protected From Contamination
    Observation: The food on display is not protected from contamination at the seasonings bar as the seasonings/toppings are in open bowls not protected with a sneeze guard. .
    Correction: Provide a sneeze guard over the toppings/seasonings bar. Food on display shall be protected from contamination by the use of packaging, counter, service line, or salad bar food guards, display cases, or other effective means.

The purpose of today's visit was to conduct a risk factor assessment inspection. Observed facility clean and organized. Observed the following good active managerial control practices in place: knowledgeable CFM, cold holding logs, grease trap cleaning/maintenance log, designated employee belongings section, easily accessible Employee Health training documents and all customers orders are cooked to 165F or above.
No violation noted during this evaluation.

The purpose of this visit is to conduct a routine inspection. In addition to today's visit, a complaint inspection was also completed. The results for the complaint inspection are detailed in the complaint report. Today's visit revealed that the employee health policy was in place. Sanitizer observed at acceptable level. Observed two employees wash hands following proper technique during inspection. supplier invoices reviewed and found to be approved supplier(s). Food temperatures were observed within an acceptable limit.
**Remember to keeps a lid and a straw on employee drinks to reduce the chance of cross contamination in food prep areas/dishwashing areas.

    Correction: Manager discarded the drinks and spoke to the employees about open drinking containers. Food employees may have beverages in the kitchen areas as long as they are covered and consumed through a straw and stored in a manner that will prevent contamination to food, equipment, utensils, linens, and single service items.

    Observation: Dispensing and/or in-use utensils improperly stored between use as follows: rice scoops stored in 76f-78f water along the cookline
    Correction: Manager removed the rice scoops and containers and replaced clean rice scoops and dry containers and will change the rice scoops and containers out every 4 hours. During pauses in food preparation or dispensing, food utensils shall be stored in one of the following manners: 1) in the food with their handles above the top of the food and the container, 2) in food that is not potentially hazardous (time/temperature control for safe food) with their handles above the top of the food within containers or equipment that can be closed, 3) on a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food contact surface of the food preparation table or cooking equipment are cleaned and sanitized, 4) in running water of sufficient velocity to flush particulates to the drain, 5) in a clean, protected location if the utensils are used only with a food that is NOT potentially hazardous (time/temperature control for safe food), or 6) in a container of water if the water is maintained at a temperature of at least 135°F and the container is cleaned frequently.

    Observation: The handwashing facility located at the buffet is blocked with utensils and sanitizing solution bucket, preventing access by employees for easy handwashing.
    Correction: A hand washing sink shall be maintained so that it is accessible at all times for employee use. Do not place anything in any hand washing sink to make them easily available for use by employees.

    Observation: Observed the handsink at the buffet is being used to store small utensils.
    Correction: Handsinks are to be used only for washing hands to encourage proper hand washing and to prevent contamination of cleaned hands.

This is was a follow-up to routine inspection performed on March 14, 2014 to observe Employee Health Policy.
**Please fax and provide a copy of additional CFM certification by May 16, 2014 since there is repeated violations of no CFM on-site during hours of operation.

    Observation: There is no Certified Food Manager present at the beginning of the inspection.
    Correction: It shall be unlawful to operate a food establishment unless it is under the immediate control of a certified food manager. It is highly suggested that you have more than one employee with the food manager's license. ORS Interactive, Inc. (see handout provided) issues the required photo identification card with proof of successful completion of a certified food manager's exam. Failure to have a certified food manager on site during ALL hours of operation including food preparation, food service, and cleaning/sanitizing of equipment/utensils will result in closure of the establishment.

  • Critical: Responsibility of PIC to Require Food Employees to Report GI Symptoms or Cuts, Disease, Medical Condition or Exposure to PIC
    Observation: Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy.
    Correction: A complete employee health policy must have the following elements:
    1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout)

    Observation: Raw animal food holding in a manner that may cause cross contamination of ready-to-eat food as follows: raw chicken, raw beef over shredded cheese in 2dr prep cooler at grill.
    Correction: Food shall be protected from cross contamination by separating raw animal foods like poultry, beef, pork, seafood, and eggs during holding from ALL other foods such as cooked foods, ready-to-eat foods, washed produce, etc. Manager rearranged.

    Observation: Different types of raw animal foods stored in such a manner that may cause cross contamination as follows: raw beef over raw scallop, raw turkey over raw beef in walk-in cooler.
    Correction: Raw animal foods shall be sorted by their "type" and cooking temperatures while in storage inside a refrigerator or freezer. For example, if using ONE shelving unit for storage of ONLY raw foods, the foods shall be sorted in this manner: 1) raw poultry (chicken, turkey, or duck) shall be stored on the BOTTOM shelf (165°F cook temperature), 2) raw ground beef and ground pork shall be stored on the shelf above poultry (155°F cook temperature), and 3) raw pork (chops, ribs), seafood, and shell eggs shall be stored on the top shelf above beef (145°F cook temperature).   • Thawing / Approved Methods
    Observation: Observed the following food thawing using an improper method: raw chicken, raw beef on counter.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be thawed using one of the following methods: 1) under refrigeration that maintains the food temperature at 41°F or less, 2) completely submerged under running water at a water temperature of 70°F or below with a sufficient velocity to agitate and float off loose particles of food as long as held for period of time at a temperature of 41°F or less, or 3) as part of a cooking process if the food is cooked as specified by Law or thawed in a microwave oven and immediately transferred to a conventional cooking process.
  • Temp Meas Devices for Cold & Hot Hold Equipment, Present
    Observation: There was no temperature measuring device located in the following cold or hot holding equipment: in both 2dr prep cooler at self serve bar.
    Correction: Cold or hot holding equipment used for the storage of potentially hazardous foods (time/temperature control for safety food) shall be designed to include and shall be equipped with at least one integral or permanently affixed temperature measuring device that is located to allow easy viewing of the device's temperature display.
  • Equip/Utensils/Linens/Single-Service/Invert, Cover
    Observation: Clean sample spoons at self serve bar were observed stored with the food-contact surface facing upward.
    Correction: Clean equipment and utensils shall be stored in a self-draining position that allows air drying and covered/inverted.

    Correction: Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. (see handout provided) within 10 business days. A copy of the photo identification card can be faxed to the food safety section at (703) 385-9568. Failure to provide a copy of the photo identification card will result in further enforcement action.
  • Refuse / Cover Receptacles / Outdoors / Tight-Fitting Lids
    Observation: Outside refuse container was uncovered.
    Correction: Receptacles and waste handling units for refuse, recyclables, and returnables shall be kept covered with tight fitting lids or doors if kept outside the food establishment.
  • Hand Drying Provision / Individual, Disposable Towels (repeated violation)
    Observation: Observed that paper towels were not provided at each handwashing sink: at bar and at self serve area.
    Correction: Provide paper towels at each hand sink to allow employees to properly dry their hands after handwashing. Manager obtained paper towels.
  • Critical: Common Name on Working Containers of Toxics (Cleaners, Sanitizers) (corrected on site)
    Observation: Working containers of poisonous and toxic materials are not properly labeled with a common name: cleaning spray bottles.
    Correction: Label all working containers of poisonous and toxic materials, including cleaners and sanitizers, with a common name to prevent the dangerous misuse of these materials.

    Observation: Different types of raw animal foods stored in such a manner that may cause cross contamination as follows: raw chicken over raw shrimp, raw pork over raw shrimp, raw chicken over raw beef in walk-in cooler.
    Correction: Raw animal foods shall be sorted by their "type" and cooking temperatures while in storage inside a refrigerator or freezer.   • Critical: Quaternary Ammonium Compound Sanitizer Solution Concentration per Use Directions (corrected on site) (repeated violation)
    Observation: When tested, the quaternary ammonium compound-based sanitizing solution was observed to be at a concentration less than what is required for effective sanitization of food-contact surfaces of equipment and utensils. Observed the sanitizing solution in the wet wiping bucket with a concentration of 0 ppm total quaternary ammonium compound.
    Correction: The quaternary ammonium sanitizer used in this facility shall have a minimum concentration of at least 200 ppm OR a concentration between 150-400ppm based on the manufacturer's use directions and using the appropriate test kit.

    Correction: Wet wiping cloths are to be used for wiping counters and other equipment surfaces and are to be held in a chemical sanitizer solution at the appropriate concentration between uses.

    Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: marinara sauce (49F) in sauce cooler at buffet table - left - section closed off and maintenance called.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth.
  • Equipment / Good Repair / Operation
    Observation: The following refrigeration unit is not operating properly and is unable to maintain cold food at or below 41°F: sauce cooler at buffet line - left
    Correction: Repair/adjust the unit so it is able to maintain foods at or below 41°F. Do not use the unit to store potentially hazardous foods while it is not operating properly.

    Observation: When tested, the chlorine-based sanitizing solution was observed to be at a concentration less than what is required for effective sanitization of food-contact surfaces of equipment and utensils. Observed the sanitizing solution in the chlorine with a concentration of less than 50 ppm total chlorine.
    Correction: A chlorine-based sanitizing solution shall have a minimum concentration of at least 50 to100 ppm total chlorine when tested using the appropriate test kit.
