  • Critical: Handwashing / When to wash hands (corrected on site)
    Observation: A food employee failed to wash her hands after handling raw eggs with gloves and before handling ready-to-eat foods.
    Correction: ALL food employees shall wash their hands OFTEN especially after handling trash, using the restroom, handling raw meats, handling soiled equipment and utensils, BEFORE putting on clean gloves, etc. Proper handwashing frequencies will aid in the reduction of foodborne illness. Discussed and training provided with food staff concerning when to wash hands.
  • Wiping Cloths/Use Limitation/Wet Towel Storage (repeated violation)
    Observation: Wet wiping cloths used for wiping counters and other equipment surfaces were observed stored on the counter.
    Correction: Wet wiping cloths are to be used for wiping counters and other equipment surfaces and are to be held in a chemical sanitizer solution at the appropriate concentration between uses.
  • Food Storage / Preventing Contamination (corrected on site)
    Observation: Observed container of beef stored in walk-in cooler floor.
    Correction: Food shall be protected from contamination by storing the food at least 6 inches off the floor on approved shelving units or dunnage racks. Milk crates, soda crates, or bread racks are not suitable for food storage. Beef was properly stored during today's inspection.
  • Clean Ice Bins, Beverage Dispenser Nozzles, Etc. per Manufacturer Specs. (repeated violation)
    Observation: Observed interior of ice machine in need of cleaning.
    Correction: Surfaces of utensils and equipment contacting food that is NOT potentially hazardous (time/temperature control for safety food) such as ice bins, beverage dispensing nozzles, and enclosed components of equipment such as ice makers, cooking oil storage tanks and distribution lines, beverage and syrup dispensing lines or tubes, coffee bean grinders, and water vending equipment at a frequency specified by the manufacturer or at a frequency necessary to preclude accumulation of soil or mold.
  • Handwashing Sink / Accessible at All Times (corrected on site)
    Observation: The handwashing facility located by the 3-vat sink is blocked with mopping equipment, preventing access by employees for easy handwashing.
    Correction: A hand washing sink shall be maintained so that it is accessible at all times for employee use. ITEMS BLOCKING HAND SINK WERE REMOVED.

Pest Control: Twice a year. (building property management takes care of it.)
Grease Trap: Monthly
Hood: Every 6 months, hood filters every week.
REMINDER:
1) Do not leave raw shell eggs outside with ice.
2) Monitor cold holding and date marking, these violations have been cited 3 times.
if these violations are observed again, you will be subject to further enforcement action.
3) Adjust all the refrigerator temperatures to 36F during summer to maintain the food temperatures at 41F or below.

  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site) (repeated violation)
    Observation: The following food items were observed cold holding at improper temperatures using a calibrated food temperature measuring device: sliced cheese 48F--Beverage Air 3DR prep refrigerator. Note: Per the employee they left it out during breakfast rush.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth. Note: Discussed with the manager not to leave potentially hazardous foods out even during rush hour.
  • Critical: Cold Holding of Raw Shell Eggs at Ambient Temperature of 45°F or less (corrected on site) (repeated violation)
    Observation: Observed raw shell eggs stored at room temperature in a container during breakfast rush. Note: Put ice on it and put the container in ice.
    Correction: Shell eggs that have not been treated to destroy Salmonella shall be stored in refrigerated equipment that maintains an ambient temperature of 45°F or less. Discussed with the manager either to leave the eggs in prep refrigerator or put ice with raw shell eggs when left out to maintain the temperature.
  • Critical: Date Marking of Refrigerated, Ready-to-Eat PHF (TCS Food) (corrected on site) (repeated violation)
    Observation: The following refrigerated, ready-to-eat, potentially hazardous food that is commercially processed is not used or discarded within 24 hours of when the original container was opened and is not observed to be date marked: American cheese, feta cheese, sliced turkey
    Correction: Refrigerated, ready-to-eat, potentially hazardous commercially processed foods when opened and kept for more than 24 hours cannot have a shelf-life of more than 7 days, including the day of the original container was opened. These food items must be marked with either the "consume-by" date and/or "preparation" date, color-coded labels, or another marking system/method that effectively indicates shelf-life that is clearly understood and properly used by the employees. This date cannot exceed any manufacturer determined "use-by" date based on food safety.

Note: The facility does not serve salmon.
REMINDERS:
**Do NOT allow food items to sit out at room temperature during busy times because it is convenient. Potentially hazardous food items must be held HOT at or above 135F or COLD at or below 41F.
**Test the sanitizer using the test strips.

  • Critical: No Bare Hand Contact with RTE Foods (corrected on site)
    Observation: A food employee was observed handling the following ready-to-eat food using their bare hands: sliced cheese. Note:Discarded.
    Correction: Except when washing fruits and vegetables, food employees may not contact ready-to-eat foods using their bare hands. Employees shall use suitable utensils such as deli tissue, spatulas, tongs, or clean disposable gloves to handle all ready-to-eat foods.
  • Critical: Food Protection from Contamination / Separate Raw from RTE (corrected on site)
    Observation: Raw animal food holding in a manner that may cause cross contamination of ready-to-eat food as follows: raw shell eggs stored above produce--walk-in refrigerator.
    Correction: Food shall be protected from cross contamination by separating raw animal foods like poultry, beef, pork, seafood, and eggs during holding from ALL other foods such as cooked foods, ready-to-eat foods, washed produce, etc.

    Observation: The following food items were observed cold holding at improper temperatures using a calibrated food temperature measuring device: Sliced cheese 51F--prep top. Note: Some discarded and rest put back in refrigerator.

    Observation: The concentration of the chlorine solution was measured at more than 200 ppm in wet wiping bucket. This concentration exceeds the approved limits for use with food-contact surfaces of equipment and utensils in a foodservice establishment.
    Correction: Maintain the concentration of chlorine solution at 50- 200 ppm. Verify concentration using the appropriate test kit.

    Observation: Employees observed wearing jewelry on their arms and hands while preparing food.
    Correction: Food employees shall limit their jewelry to a plain wedding ring while preparing food. Food employees may not wear bracelets, watches, etc. because they can inhibit proper hand washing.
  • Hair Restraint Effectiveness (corrected on site)
    Observation: Employees observed working in the food service area without proper hair restraints. One employee hada bangs, not restrained by hair net.
    Correction: Food employees shall wear hair restraints such as hats, visors, hair nets or beard nets to effectively keep their hair from contacting exposed food, clean equipment, utensils, linens, and unwrapped single-service items. This section does not apply to food employees such as counter staff who ONLY serve beverages and wrapped or packaged foods, hostesses, and wait staff.

    Correction: Contact your fish supplier and ask for a written statement stating that the fish were raised in net pens if in open water OR land-based operations AND fed formulated feed that contains no live parasites. FAX a copy of this letter to your inspector and keep this statement on site at the food establishment for 90 days beyond the time of service of the fish.

    Observation: The operator is not testing the chemical sanitizing solution to ensure proper concentration of the solution. Also, the test strip used by employee during inspection was not accurate (did not change color).
    Correction: The operator shall be monitoring the concentration of the sanitizing solution using a test kit or other device.
  • Single-Service and Single-Use Articles / No Reuse (corrected on site)
    Observation: Manufacturer container of mozarrella cheese were observed reused for the storage of spianch.
    Correction: Manufacturer containers may not be reused for secondary storage of other foods because the containers are not durable enough for multiple reuse and become uncleanable.

    Observation: The following equipment/utensils were observed soiled to sight and touch: can opener

    Observation: The drain under the wash and rinse sink going to grease trap draining very slowly and Plumbing connections under the 3-vat sink and handksink piping are leaking.
    Correction: A plumbing system shall be maintained in good repair.

    Observation: Observed that the Ceiling in entire facility including ceiling vents, filters and area around filters of hood system, in between equipment and freezer and refrigerator doors is in need of cleaning.
    Correction: Clean the physical facilities at a frequency necessary to ensure the protection and safe preparation of food.
  • Critical: Sanitizers, 21 CFR Criteria for Food Contact (corrected on site)
    Observation: The concentration of the bleach solution was measured at more than 200 ppm in wet wiping bucket. This concentration exceeds the approved limits for use with food-contact surfaces of equipment and utensils in a foodservice establishment.
