REMINDERS:
1) Recommend checking the sanitizer concentration for dish machinie and quateranary ammonia dispenser at least once a day.
2) Do not turn off the dipper well.
3) Please maintain the temperature log for cold holding, hot holding, cooking and receiving temperatures for TCS (Temperature Control for Safety) foods.

  • Utensils That are In-Use / Between-Use Storage (corrected on site) (repeated violation)
    Observation: Dispensing and/or in-use utensils improperly stored between use as follows: Observed frozen custard scoops in standing water. (dipper well was not turned on).
    Correction: During pauses in food preparation or dispensing, food utensils shall be stored in one of the following manners: 1) in the food with their handles above the top of the food and the container, 2) in food that is not potentially hazardous (time/temperature control for safe food) with their handles above the top of the food within containers or equipment that can be closed, 3) on a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food contact surface of the food preparation table or cooking equipment are cleaned and sanitized, 4) in running water of sufficient velocity to flush particulates to the drain, 5) in a clean, protected location if the utensils are used only with a food that is NOT potentially hazardous (time/temperature control for safe food), or 6) in a container of water if the water is maintained at a temperature of at least 135°F and the container is cleaned frequently.

    Observation: The following food items were observed cold holding at improper temperatures using a calibrated food temperature measuring device: Sliced cheese 48F and grilled onions 46F on ice near grill.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth. Note: Put back in refrigerator.
  • Critical: Chlorine Sanitizer Solution Concentration based on pH and Water Temp (corrected on site) (repeated violation)
    Observation: When tested, the low temperature, chemical sanitizing warewashing machine was observed with a sanitizing rinse concentration of less than 0 ppm chlorine solution.
    Correction: Immediately discontinue use of the dishmachine. Until the machine is adequately repaired, set up the three-compartment sink to manually wash, rinse and sanitize food-contact surfaces using a sanitizing solution at the proper concentration of at least 50 to 100 ppm total chlorine. New sanitizer container attached and primed.
  • Plumbing / Maintained in Good Repair
    Observation: Plumbing connections under the dipper well (frozen custard machine) was not in good repair. Also, observed a leak near the back flow prevention device for the dish washing machine.
    Correction: A plumbing system shall be maintained in good repair.

The purpose of today's visit is to conduct a follow-up inspection to assess the floor condition in grill/warewashing area. Observed no water pooling in grill area. The leak has been fixed. Please grout the area in the grill/ware wash area and send a picture within 10 days to prevent further damage/water accumulation and pest infestation.
Also, the facility management will re-grout the entire floor in the year 2016.
Please get CFM card for all the managers with CFM certification ASAP.

    Observation: The person in charge (PIC) has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc.
    Correction: Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. (see handout provided). A copy of the photo identification card can be faxed to the food safety section at (703) 653-9448. Failure to provide a copy of the photo identification card may result in further enforcement action.

Note: Observed fruit flies. Please deep clean all the floor drains, area under 3-vat sink and call pest control service within 10 days to treat for fruit flies. Please fax or email the invoice/report for pest control invoice and fixing the dish machine within 10 days.
Repair/fix floor tiles within 30 days and call the EHS. A follow-up inspection will be conducted within 30 days to verify compliance on damaged floor tile. Note: Water is accumulating under the floor tiles near dish washing area. Keep the area dry and clean.

    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth. Note: Put all items inside the refrigerator.
  • Equipment / Fixed / Space / Seal
    Observation: The following equipment is not installed in a manner that allows accessibility for for cleaning, nor is it installed with closely abutting surfaces to minimize the need for cleaning: caulking following equipment is missing or damaged: 3-vat sink, dish machine and hand sink in restrooms.
    Correction: Equipment that is fixed because it is not easily movable shall be installed so that it is: 1) spaced to allow access for cleaning along the sides, behind, and above the equipment, 2) spaced from adjoining equipment, walls, and ceilings a distance of not more than 1 millimeter or one thirty-second inch, and 3) sealed to adjoining equipment or walls if the equipment is exposed to spillage or seepage.

Note: The manager primed the machine and it was sanitizing at 100 ppm.
  • Sanitizing Solution Test Kit Accurate & Used
    Observation: The operator is not testing the chemical sanitizing solution to ensure proper concentration of the solution. Observed that both chlorine and quaternary ammonia test strips are damaged and not usable.
    Correction: The operator shall be monitoring the concentration of the sanitizing solution using a test kit or other device.
  • Cutting Boards / Resurface / Discard
    Observation: The cutting boards along the preparation line are heavily scratched and scored. The food contact surface is no longer easily cleaned and sanitized due to condition.
    Correction: Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced if they can no longer be effectively cleaned and sanitized, or discarded if they are not capable of being resurfaced.

    Observation: The following equipment/utensils were observed soiled to sight and touch: can opener, milk frother (smoothie blender) milk dripping from both.
    Correction: Equipment food-contact surfaces and utensils shall be clean to the sight and touch. A surface that is soiled cannot be effectively sanitized.
  • Equipment & Utensils / Air-Drying or Adequate Draining Required
    Observation: Plasic pans and utensils were found stacked while wet after cleaning and chemical sanitization.
    Correction: After cleaning and sanitizing, equipment and utensils shall be air-dried or used after adequate draining before contact with food.

    Observation: Observed that the floor tiles is not maintained in good repair. Floor near the grill and dish machine area holds water under it and splashes water when walking on it. The facility puts carboard on the floor to soak all the water. Also, the floor tiles in the entire kitchen area has missing or damaged grout.
    Correction: Repair the physical facilities. Poor repair and maintenance compromises the functionality of the physical facilities. Note: Fix the floor near dish machine and re-grout the floor tiles in kitchen.
  • Controlling Pests by Eliminating Harborage Conditions
    Observation: Harborage conditions for fruit flies exist. Standing water in between tiles. Area under 3-vat sink not clean.
    Correction: Insects and other pests are capable of transmitting disease to people by contaminating food and food-contact surfaces. Eliminate harborage conditions to control and prevent the presence of pests on the premises. Note: Keep the area under 3-vat sink, smoothie and custard machine area clean. Re-grout the floors.
  • Removing Dead or Trapped Birds, Insects, Rodents, etc.
    Observation: An accumulation of dead fruit flies were found on floor of 1DR upright refrigerator.
    Correction: Remove dead rodents, birds, and insects from the control devices at a frequency necessary to ensure that the establishment is clean and sanitary and to prevent carcasses from attracting other pests.

  • Critical: Handwashing / When to wash hands (corrected on site)
    Observation: A food employee failed to wash his or her hands before engaging in food preparation, after handling soiled utensils, or after engaging in any activity that could contaminate the hands. OBSERVED FOOD SERVICE WORKERS PUTTING ON NEW GLOVES WITHOUT WASHING HANDS.
    Correction: ALL food employees shall wash their hands OFTEN especially after handling trash, using the restroom, handling raw meats, handling soiled equipment and utensils, BEFORE putting on clean gloves, etc. Proper handwashing frequencies will aid in the reduction of foodborne illness. ORAL & WRIITEN INFORMATION PROVIDED. RECOMMEND, MORE TRAINING.
  • Critical: Gloves/Use Limitation/No Re-Use/Use For Only One Task (corrected on site)
    Observation: Single-use gloves worn during multi-tasked food preparation. Worker (grill operator) handled raw hamburger refrigerator tub, touching inside, and dropped tongs inside tub, and then proceeded to handle utensils, cheese and buns of burgers and hot dogs same with gloved hands.
    Correction: If used, single-use gloves shall be used for only ONE task and used for no other purposes. The single-use gloves shall be discarded when damaged or soiled or when interruptions occur in the operation. Supervisor changed tongs and educated worker. ORAL & WRIITEN INFORMATION PROVIDED. RECOMMEND, MORE TRAINING.

Observed the sanitizing solution in the WET WASH CLOTH BUCKETS with a concentration of ~100 ppm total quaternary ammonium compound.
    Correction: The quaternary ammonium sanitizer used in this facility shall have a minimum concentration of at least 200 ppm OR a concentration between 150-400ppm based on the manufacturer's use directions and using the appropriate test kit. It is highly suggested that you have more than one employee with the food manager's license. ORS Interactive, Inc. (see handout provided) issues the required photo identification card with proof of successful completion of a certified food manager's exam.     Observation: Openings to the exterior of the building are present along the rear exterior door
    Correction: Fill or close holes and other gaps along floors, walls, and ceilings to protect against the entry of insects and rodents. Provide a door sweep.
