  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site) (repeated violation)
    Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: Several items observed in the prep top portion of the two door prep cooler were observed overfilled over the top and were being held in the danger zone: cooked pork rib 56f, cooked chicken wing 58f, sweet and sour chicken 52f. In addition, cooked noodles 62f and bean sprouts 68f were being stored on the countertop along the cookline.
    Correction: The cooked pork rib, sweet and sour chicken, cooked chicken wing were re porportioned so that the prep top containers were half full with food and the top portion was relocated in the bottom portion of this prep cooler. The cooked noodles and the bean sprouts were placed in the 2nd prep cooler. Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth.
  • Critical: Equipment / Food-Contact / Visibly Clean (corrected on site)
    Observation: The following equipment/utensils were observed soiled to sight and touch: Observed knives, meat choppers and other utensils hanging in clean storage on the magnetic strip and the food contact surfaces of the above utensils had dried food debris on the surface.
    Correction: A food worker removed the above utensils and began washing, rinsing and sanitizing at the three vat sink. Equipment food-contact surfaces and utensils shall be clean to the sight and touch. A surface that is soiled cannot be effectively sanitized.

  • Handwashing / Where to Wash (corrected on site) (repeated violation)
    Observation: A food employee was observed cleaning their hands in 3-vat sink
    Correction: ALL food employees shall wash their hands in ONLY a designated handsink. Employee was moved to wash hands in the hand sink.
  • Critical: Eating, Drinking, or Tobacco Use / Proper Food Employee Drinking Containers (corrected on site)
    Observation: A food employee was observed eating and drinking in areas where they may contaminate food, clean equipment, utensils or other items needing protection.
    Correction: Food employees may only eat, drink, or use tobacco in a designated area like a dining room or office area. Employees shall not eat or drink in the kitchen areas, warewashing areas, or storage areas where items may become contaminated. Manager told employees that they can not eat and drink in the kitchen.
  • Critical: No Bare Hand Contact with RTE Foods (corrected on site)
    Observation: A food employee was observed handling the following ready-to-eat food using their bare hands: cooked noodles to mix in oil.
    Correction: Except when washing fruits and vegetables, food employees may not contact ready-to-eat foods using their bare hands. Employees shall use suitable utensils such as deli tissue, spatulas, tongs, or clean disposable gloves to handle all ready-to-eat foods. Noodles were discarded.
  • Wiping Cloths/Use Limitation/Wet Towel Storage (corrected on site)
    Observation: Wet wiping cloths used for wiping counters and other equipment surfaces were observed stored on the counter. There was no sanitizing wiping cloth bucket.
    Correction: Wet wiping cloths are to be used for wiping counters and other equipment surfaces and are to be held in a chemical sanitizer solution at the appropriate concentration between uses. Made sanitizer and made buckets for wetwiping clothes.
  • Food Storage / Preventing Contamination
    Observation: Food stored on the floor and/or food stored less than 6 inches off the floor: vegetables and food on floor in walk-in freezer and walk-in cooler.
    Correction: Food shall be protected from contamination by storing the food at least 6 inches off the floor on approved shelving units or dunnage racks. Milk crates, soda crates, or bread racks are not suitable for food storage.
  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site) (repeated violation)
    Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: bean sprout (49F) on counter. cooked shrimp (49F), cooked chicken (49F) on prep top in 2dr prep cooler - left.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth. Made room in prep top for bean sprout so it can be properly cold holding. 2dr prep cooler was emptied out and maintenance requested for 2dr prep cooler - left.
  • Critical: Date Marking of Refrigerated, Ready-to-Eat PHF (TCS Food) (corrected on site)
    Observation: The following refrigerated, ready-to-eat, potentially hazardous food that is prepared on site is not used or discarded within 24 hours of preparation and was not observed to be date marked: cooked duck, cooked chicken, cooked pork in walk-in cooler.
    Correction: Refrigerated, ready-to-eat, potentially hazardous foods that are prepared on site and kept for more than 24 hours cannot have a shelf-life of more than 7 days, including the day of preparation. These food items must be marked with either the "consume-by" date and/or "preparation" date, color-coded labels, or another marking system/method that effectively indicates shelf-life that is clearly understood and properly used by the employees. Dated.
  • Food Temperature Measuring Device Required & Readily Accessible (corrected on site)
    Observation: The certified food manager could not provide a food temperature measuring device.
    Correction: A metal-stemmed probe food thermometer shall be provided and readily accessible for use in ensuring attainment and maintenance of food temperatures.
  • Equipment / Good Repair / Operation
    Observation: The following refrigeration unit is not operating properly and is cold holding properly: 2dr prep cooler (54F).
    Correction: Repair/adjust the unit so it is able to maintain foods at or below 41°F. Do not use the unit to store potentially hazardous foods while it is not operating properly. maintenance is called.

    Observation: Window or door is kept open for ventilation and opening is not protected by a screen or other effective means.
    Correction: Provide screens (16 mesh to 1 inch), air curtains, or other effective means to protect against the entry of insects and rodents.

    Observation: Observed that the following are in need of cleaning: shelves, outside of containers, floor and outside of equipments.
    Correction: Clean the physical facilities at a frequency necessary to ensure the protection and safe preparation of food.

  • Critical: Food Protection from Contamination / Separate Types of Raw Animal Foods (corrected on site) (repeated violation)
    Observation: Different types of raw animal foods stored in such a manner that may cause cross contamination as follows: raw beef over raw shrimp, raw chicken over raw shrimp, raw beef over raw egg in walk-in cooler.
    Correction: Raw animal foods shall be sorted by their "type" and cooking temperatures while in storage inside a refrigerator or freezer. For example, if using ONE shelving unit for storage of ONLY raw foods, the foods shall be sorted in this manner: 1) raw poultry (chicken, turkey, or duck) shall be stored on the BOTTOM shelf (165°F cook temperature), 2) raw ground beef and ground pork shall be stored on the shelf above poultry (155°F cook temperature), and 3) raw pork (chops, ribs), seafood, and shell eggs shall be stored on the top shelf above beef (145°F cook temperature).

    Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: garlic and oil mix on chef's table (68F).
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth. Discarded.
  • Critical: Sanitize/Chemical/Manual & Mechanical/Concentration & Exposure Time (corrected on site)
    Observation: Chemical sanitizer immersion time was not adequate for chlorine sanitizing solution. Employee did not perform sanitizing step while washing dishes.
    Correction: Chemical sanitization (manual or mechanical operations) including the application of sanitizing chemicals by immersion, manual swabbing, brushing, or pressure spraying methods using a solution by providing: 1) an exposure time of at least 10 seconds for a chlorine solution, 2) an exposure time of at least 7 seconds for a chlorine solution of 50mg/L that has a pH of 10 or less and a water temperature of 100°F or a pH of 8 or less and a water temperature of 75°F, 3) an exposure time of at least 30 seconds for other chemical sanitizing solutions, or 4) an exposure time used in relationship with a combination of temperature, concentration, and pH that when evaluated for efficacy yields sanitization.

    Observation: A food employee failed to wash his or her hands before engaging in food preparation: after handling raw pork, after coming in from outside.
    Correction: ALL food employees shall wash their hands OFTEN especially after handling trash, using the restroom, handling raw meats, handling soiled equipment and utensils, BEFORE putting on clean gloves, etc. Proper handwashing frequencies will aid in the reduction of foodborne illness.

    Observation: The nonfood-contact surface of the following equipment is not corrosion resistant, nonabsorbent, and/or smooth: rusted metal shelves.
    Correction: Non-food contact surfaces of equipment that are exposed to splash, spillage, or other foods that require frequent cleaning shall be constructed of a corrosion-resistant, nonabsorbent, and smooth material.
  • Equipment / Non Food / Design / Easy Clean
    Observation: Soda crate(s) found used for the following purpose(s): hold up foods in kitchen, dry storage and in walk-in cooler.
    Correction: Milk crates, soda crates, and/or bread racks are not designed to store or elevate food, equipment, utensils, clean linens, and single-service items. Provide only ANSI-certified (such as NSF-listed) shelving units, storage units, or dunnage racks.
  • Cutting Boards / Resurface / Discard (repeated violation)
    Observation: The cutting board(s) along prep coolers are heavily scratched and scored. The food contact surface is no longer easily cleaned and sanitized due to condition.
    Correction: Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced if they can no longer be effectively cleaned and sanitized, or discarded if they are not capable of being resurfaced.

    Observation: The food-contact surfaces of the following equipment were not observed sanitized: cutting boards.
    Correction: All utensils and food-contact surfaces of equipment shall be sanitized before use and after cleaning.

    Observation: The establishment has not posted the permit to operate in a conspicuous location.
    Correction: No person shall operate a food establishment unless that person possesses a valid permit issued by the Director. Only a person who complies with the requirements of this Chapter shall be entitled to receive or retain such a permit. Permits are not transferable from one owner to another. A valid permit shall be posted in every food establishment in a conspicuous place in the public view. Food establishment permits are issued for a calendar year and expire on December 31 regardless of the month of issue.
