The purpose of today's visit was to conduct a risk factor assessment. The following violation has been observed for the second time:
1. Improper cold holding
The following suggestions have been made as a means to Incorporate Active Managerial Control:
1. Train employees on corrective actions to take when cold holding/hot holding equipment is unable to maintain correct temperatures
2. Clearly label improperly functioning equipment to discourage use by employees
3. Review and monitor temperature logs daily
Please send the following document to the Health Department within 7 days:
1. Copy of service invoice for Bev Air Milk Cooler #2

  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site) (repeated violation)
    Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: BEV AIR MILK COOLER #2- SKIM MILK (63F).
    Correction: Potentially hazardous foods (time/temperature conrol for safety food) shall be held cold at a temperature of 41°F or below. SKIM MILK DISCARDED.

The purpose of today's visit was to perform a routine assessment. Please refrain all employees from using McCall 2 door cooler to store potentially hazardous foods (PHFs) until service has been received. Please send copy of service via fax or e-mail within 5 days (4/15/15). Please continue to use food and equipment temperature logs to ensure items are being cold/hot held at the correct temperatures to limit bacterial growth. Please continue to routinely educate employees on employee health policy and food safety in order to promote public health.

    Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: MCCALL 2 DOOR COOLER- CHEESE STICK (45F), HUMMUS (46F)
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth. ITEMS MOVED TO BEV AIR 2 DOOR COOLER.
  • Equipment / Good Repair / Operation
    Observation: The following refrigeration unit is not operating properly and is unable to maintain cold food at or below 41°F: MCCALL 2 DOOR COOLER (45F).
    Correction: PLEASE DO NOT USE COOLER TO STORE POTENTIALLY HAZARDOUS FOODS. PLEASE HAVE UNIT SERVICED WITHIN 5 DAYS. Repair/adjust the unit so it is able to maintain foods at or below 41°F. Do not use the unit to store potentially hazardous foods while it is not operating properly.

    Observation: Surfaces of ice bins, beverage dispensing nozzles, and enclosed components of equipment are not being cleaned as required.--OBSERVED PINK GROWTH ON INTERIOR OF HOSHIZAKI ICE MACHINE.
    Correction: Surfaces of utensils and equipment contacting food that is NOT potentially hazardous (time/temperature control for safety food) such as ice bins, beverage dispensing nozzles, and enclosed components of equipment such as ice makers, cooking oil storage tanks and distribution lines, beverage and syrup dispensing lines or tubes, coffee bean grinders, and water vending equipment at a frequency specified by the manufacturer or at a frequency necessary to preclude accumulation of soil or mold.--EHS PROVIDED TRAINING TO CFM ON HOW AND WHEN TO CLEAN THE ICE MACHINE.
