A CFM must be onsite at ALL times of preparation and hours of operation. It is unlawful to operate an establishment unless under the direct supervision of a CFM. The CFM has many responsibilities, one of which is to make sure that food is being prepared and served safely to prevent foodborne illness. The lack of a CFM is grounds for closure.
** It is recommended that more than one person on staff be a Certified Food Manager. A Certified Food Manager must be on the premises during ALL food preparation and service.

    Observation: The following food items were observed cold holding at improper temperatures using a calibrated food temperature measuring device: shredded mozzarella cheese 46F--prep top. Pizza sauce on prep refg 60F.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth. Note: Remove some cheese from top and put inside the refrigerator. Pizza sauce inside the refrigerator. Recommend using the prep top of refrigerator for the sauce.

    Observation: The following refrigerated, ready-to-eat, potentially hazardous food that is commercially processed is not used or discarded within 24 hours of when the original container was opened and is not observed to be date marked: chickpeas and feta cheese opened 2 days ago.
    Correction: Refrigerated, ready-to-eat, potentially hazardous commercially processed foods when opened and kept for more than 24 hours cannot have a shelf-life of more than 7 days, including the day of the original container was opened. These food items must be marked with either the "consume-by" date and/or "preparation" date, color-coded labels, or another marking system/method that effectively indicates shelf-life that is clearly understood and properly used by the employees. This date cannot exceed any manufacturer determined "use-by" date based on food safety.

    Observation: There is no Certified Food Manager present at the beginning of the inspection.
    Correction: It shall be unlawful to operate a food establishment unless it is under the immediate control of a certified food manager. It is highly suggested that you have more than one employee with the food manager's license. ORS Interactive, Inc. (see handout provided) issues the required photo identification card with proof of successful completion of a certified food manager's exam. Failure to have a certified food manager on site during ALL hours of operation including food preparation, food service, and cleaning/sanitizing of equipment/utensils will result in closure of the establishment.
  • Plumbing / Maintained in Good Repair
    Observation: The faucet at the 3-vat sink is not functional. Only the spray hose works.
    Correction: A plumbing system shall be maintained in good repair.
  • Physical Facilities Good Repair
    Observation: Observed that the wall in the dry storage area has several holes in the wall and is not maintained in good repair.
    Correction: Repair the physical facilities. Poor repair and maintenance compromises the functionality of the physical facilities.
  • Critical: Sanitizers, 21 CFR Criteria for Food Contact (corrected on site) (repeated violation)
    Observation: The concentration of the quaternary ammonia solution was measured at 500 ppm. This concentration exceeds the approved limits for use with food-contact surfaces of equipment and utensils in a foodservice establishment.
    Correction: Maintain the concentration of quaternary ammonia solution at 200 ppm or 150 to 400 ppm based on manufacturer instructions. Verify concentration using the appropriate test kit. Note: Add more water.

    Observation: The following food items that are not easily identified by appearance were observed without a label: salt
    Correction: Unless a food item can be easily recognized, all foods and ingredients removed from their original containers and stored in working containers shall be labeled using the common name of the food. For example: cooking oils, salt, sugar, flour, spices, herbs, etc.
  • Wiping Cloths/Use Limitation/Wet Towel Storage
    Observation: Wet wiping cloths used for wiping counters and other equipment surfaces were observed stored on the counter.
    Correction: Wet wiping cloths are to be used for wiping counters and other equipment surfaces and are to be held in a chemical sanitizer solution at the appropriate concentration between uses.
  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site) (repeated violation)
    Observation: The following food item were observed cold holding at improper temperatures using a calibrated food temperature measuring device: pizza sauce 55F--container on prep refg. The manager said it was busy and the sauce has been out very often.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth. Note: Put inside the refrigertor. Discussed about putting sauce in a container on prep top with all the other food items to keep it 41F or below.

  • Food Temperature Measuring Device Required & Readily Accessible (repeated violation)
    Observation: The certified food manager could not provide a food temperature measuring device.
    Correction: A metal-stemmed probe food thermometer shall be provided and readily accessible for use in ensuring attainment and maintenance of food temperatures.
  • Critical: Backflow Prevention, Air Gap / When Required (corrected on site)
    Observation: Observed a hose attached to a faucet fixture. The hose extended below the flood rim level of the sink basin.
    Correction: A plumbing system shall be installed to preclude backflow of a solid, liquid, or gas contaminant into the water supply system at each point of use at the food establishment, including on a hose bib if a hose is attached or on a hose bib if a hose is not attached and backflow prevention is required by Law by providing an air gap as specified under 5-202.13. Note: The manager twined the hose around the faucet.
  • Outer Openings, Protected / Screen Requirements
    Observation: Openings to the exterior of the building are present along the back door. Observed weather stripping damaged on the right side of the door.
    Correction: Fill or close holes and other gaps along floors, walls, and ceilings to protect against the entry of insects and rodents.
  • Lighting Intensity/ Food Prep Area / 50 foot candles
    Observation: Observed that inadequate lighting was provided in the food preparation areas. Around pizza prep area observed at less than 50 foot candles.
    Correction: Increase the lighting to provide a minimum of 50 foot candles in all food preparation areas. Replace the missing bulbs and also using higher voltage bulbs.
  • Physical Facilities Good Repair
    Observation: Observed that the ceiling tile in women restroom is not maintained in good repair, one tile missing.
    Correction: Repair the physical facilities. Poor repair and maintenance compromises the functionality of the physical facilities.

    Observation: The concentration of the quaternary ammonia solution was measured at more than 500 ppm. This concentration exceeds the approved limits for use with food-contact surfaces of equipment and utensils in a foodservice establishment.

The purpose of this visit was to conduct a routine risk evaluation. Quaternary ammonium sanitizer in 3 vat sink: initial/final:150/400 ppm. PIC was aware of grease trap and need for routine maintenance. Reviewed grease trap handout with PIC who will start a log of grease trap service. Discussed public health significance of accurate date marking and time for public health control records with PIC. Provided Northern Virginia Certified Food Manager License information sheet to PIC. NOTE: PLEASE REMEMBER TO PROVIDE COPY OF NORTHERN VIRGINIA CERTIFIED FOOD MANAGER LICENSE TO HEALTH DEPARTMENT WITHIN 10 DAYS AS REQUIRED ABOVE.
  • Critical: No Bare Hand Contact with RTE Foods (corrected on site)
    Observation: A food employee was observed handling the following ready-to-eat food using their bare hands: slicing raw green peppers with potential for use in salads.
    Correction: Except when washing fruits and vegetables, food employees may not contact ready-to-eat foods using their bare hands. Employees shall use suitable utensils such as deli tissue, spatulas, tongs, or clean disposable gloves to handle all ready-to-eat foods. NOTE: PIC discussed with employee who washed hands and donned gloves.

    Observation: Cutting board(s) observed to beheavily scratched and scored. The food contact surface is no longer easily cleaned and sanitized due to condition.
    Correction: Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced if they can no longer be effectively cleaned and sanitized, or discarded if they are not capable of being resurfaced.
  • Certified Food Manager/Certificate Process
    Observation: The person in charge (PIC) has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc.
    Correction: Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. (see handout provided) within 10 business days. A copy of the photo identification card can be faxed or emailed to the food safety section at (703) 385-9568. Failure to provide a copy of the photo identification card will result in further enforcement action.

Observed the sanitizing solution in the 3-vat sink with a concentration of 150ppm total quaternary ammonium compound.
    Correction: The quaternary ammonium sanitizer used in this facility shall have a minimum concentration of at least 200 ppm OR a concentration between 150-400ppm based on the manufacturer's use directions and using the appropriate test kit.

There is inconsistent hot water in the facility. The tankless water heater displayed a temperature of 140F, yet handsink and 3 vat sink temperatures were less than 100F. Within 72 hours, fax a copy of the receipt to the health department for adjusting the hot water.
Fax a copy of the receipt for the purchase of the sanitizer test kit to the health department within 72 hours.
**Approval is hereby granted for the issuance of a Health Department Permit. This inspection report shall serve as your permit until the official permit is received. All outstanding violations are to be corrected within ten (10) days but no later than the time of your first routine inspection (approximately 30 days). Equipment additions/replacement/changes are not allowed without Health Department approval.**
Note the following recommendations:
It is recommended that more than one person on staff be a Certified Food Manager. A Certified Food Manager must be on the premises during ALL food preparation and service.
Hood system is a Type II system with no grease collection filters. Clean regularly as manufacturer requires.
It is recommended that the grease trap be cleaned at least every two months. Maintain records of grease trap cleaning for Health Department review.
It is recommended that a monthly pest control program be implemented. Maintain records of pest control treatments for Health Department review.

  • Sanitizer Test Kit Required
    Observation: There is no test kit located in the facility for monitoring the concentration of the chemical sanitizing solutions.
    Correction: Obtain a quaternary ammonia test kit to be used to monitor the sanitizer concentration at the 3 vat sink.
  • Handwashing Sink / Mix Valve or Combination Faucet , Water at 100°F
    Observation: Hot water at the 3 vat sink and kitchen handsink was at an inadequate temperature, while the restroom handsinks had exceptionally hot water.
    Correction: Hot water at the 3 vat sink must be at least 110F. Hot water at all handsinks must be at least 100F. Service the hot water supply.
  • Critical: Plumbing / Maintained in Good Repair
    Observation: There is no faucet handle for the hot water at the front handsink. There is no hot water for handwashing.
    Correction: Repair the handsink. There must be hot water at all handsinks of at least 100F.
  • Light Bulbs / Locations where Shielding is Required
    Observation: The light bulb in the walkin refrigerator is not shielded.
    Correction: Provide a shield(cover) on the light bulb in the walkin refrigerator. This may necessitate changing the bulb entirely, as well.
