    Observation: Wiping cloths improperly used for the following activity: ued to dry vegetables and herbs
    Correction: Cloths shall not be used for any other purposes. For example, cloths shall not be placed under cutting boards to prevent the boards from sliding while in use.
  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site) (repeated violation)
    Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: 1) turkey 44F, cream cheese 46F, cooked chicken 52F all in Beverage AIr 2dr preptop cooler, 2) half&half 75F at room temp
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth. CFM placed potentially hazardous food items in the leader cooler, half&half was discarded
  • Critical: Date Marking of Refrigerated, Ready-to-Eat PHF (TCS Food) (corrected on site) (repeated violation)
    Observation: The following ready-to-eat food that is made using a combination of ready-to-eat, potentially hazardous ingredients is not properly dated for discard using the date of the first-prepared ingredient: cut watermelon
    Correction: Refrigerated, ready-to-eat, potentially hazardous food (time/temperature control for safety foods) that combined with additional ingredients, or portions of the same food, must retain the date marking of the earliest or first prepared ingredient. CFM labeled watermelon with prep date of 8/27

    Observation: There is no test kit located in the facility for monitoring the concentration of the chemical sanitizing solutions.
    Correction: Obtain a Chlorine test kit.

    Observation: The following refrigeration unit is not operating properly and is unable to maintain cold food at or below 41°F: Beverage Air 2dr preptop cooler 42F
    Correction: Repair/adjust the unit so it is able to maintain foods at or below 41°F. Do not use the unit to store potentially hazardous foods while it is not operating properly.

  • Critical: Responsibility of PIC to Require Food Employees to Report GI Symptoms or Cuts, Disease, Medical Condition or Exposure to PIC
    Observation: Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy.
    Correction: A complete employee health policy must have the following elements:
    1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout)
  • Critical: Approved Food Source / No Home Prepared Food
    Observation: The following food item(s) is prepared somewhere else and source could be verified: Butter chicken, chick peas.
    Correction: Food prepared in a private home may NOT be used or offered for human consumption in a food establishment. Cooked cooked at an other food establishment may be served provided the source or the restaurant where the food was cooked/prepared is approved and can be verified.
  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site)
    Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device:butter chicken, chick peas, pastrami, taziki sauce.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below. (relocated food to an other refrigerated unit)
  • Critical: Date Marking of Refrigerated, Ready-to-Eat PHF (TCS Food)
    Observation: The following refrigerated, ready-to-eat, potentially hazardous food that is prepared on site is not used or discarded within 24 hours of preparation and was not observed to be date marked: chick peas, butter chicken, rice.
    Correction: Refrigerated, ready-to-eat, potentially hazardous foods that are prepared on site and kept for more than 24 hours cannot have a shelf-life of more than 7 days, including the day of preparation. These food items must be marked with either the "consume-by" date and/or "preparation" date, color-coded labels, or another marking system/method that effectively indicates shelf-life that is clearly understood and properly used by the employees.
  • Critical: Date Marking of Refrigerated, Ready-to-Eat PHF (TCS Food)
    Observation: The following refrigerated, ready-to-eat, potentially hazardous food that is commercially processed is not used or discarded within 24 hours of when the original container was opened and is not observed to be date marked:sliced cooked chicken, ham.
    Correction: Refrigerated, ready-to-eat, potentially hazardous commercially processed foods when opened and kept for more than 24 hours cannot have a shelf-life of more than 7 days, including the day of the original container was opened. These food items must be marked with either the "consume-by" date and/or "preparation" date, color-coded labels, or another marking system/method that effectively indicates shelf-life that is clearly understood and properly used by the employees. This date cannot exceed any manufacturer determined "use-by" date based on food safety.
  • Equipment / Good Repair / Operation
    Observation: The following refrigeration unit is not operating properly and is unable to maintain cold food at or below 41°F: 2-door prep table .
    Correction: Repair/adjust the unit so it is able to maintain foods at or below 41°F. Do not use the unit to store potentially hazardous foods while it is not operating properly.
  • Equip/Utensils/Linens/Single-Service/Original Package (corrected on site)
    Observation: Coffee filters were observed stored unprotected at the coffee machine unit.
    Correction: Single-service items shall be stored in a clean, dry location that is not exposed to splash, dust, or other contamination and at least 6 inches off the floor on approved shelving units or dunnage racks. In addition, the single-service items shall be kept in the original protective package or stored by using other means that afford protection from contamination until used.
  • Equip/Utensils/Linens/Single-Service/Elevation
    Observation: Boxes of styro foam cups was observed directly on the floor in the dinner area.
    Correction: Store single services utensils off the floor to protect them from being contaminated.
  • Single-Service & Single-Use, Displayed & Dispensed / Prevent Contamination (corrected on site)
    Observation: Plastic spoons and forks were found displayed or dispensed with the food or lip-contact surface facing upward or toward customers.
    Correction: Single-service items and cleaned/sanitized utensils shall be handled, displayed, and dispensed so that contamination of food and lip contact surfaces are prevented.
  • Permit to Operate Required, Must be Posted, Must be Valid
    Observation: The establishment has not posted the permit to operate in a conspicuous location.
    Correction: No person shall operate a food establishment unless that person possesses a valid permit issued by the Director. Only a person who complies with the requirements of this Chapter shall be entitled to receive or retain such a permit. Permits are not transferable from one owner to another. A valid permit shall be posted in every food establishment in a conspicuous place in the public view. Food establishment permits are issued for a calendar year and expire on December 31 regardless of the month of issue.
  • Mops in Air-dry Position (corrected on site)
    Observation: Observed that mops are improperly stored between use. A wet mop was noted sitting in the mop bucket and not hung to properly air dry.
    Correction: Store mops in a position that allows them to air dry without soiling walls, equipment, or supplies.
12/09/2014Routine
The Health Department has been notified that settlement on a change of ownership will occur on August 25, 2014. This inspection was conducted to facilitate the issuance of a Health Department Permit to Operate during the change of ownership at this establishment. At the time of this inspection, the establishment is in compliance with Chapter 43.1, Fairfax County Food Code. The current permit is not transferable. In order for a Permit to Operate to be issued after settlement, please contact Ron Campbell, Environmental Health Specialist III, at 703-246-8427.
A first routine inspection will be conducted approximately 30 days after the Permit to Operate is issued. All outstanding violations from this pre-opening inspection must be corrected by the time of the first routine inspection.
Note the following recommendations:
It is recommended that more than one person on staff be a Certified Food Manager. A Certified Food Manager must be on the premises during ALL food preparation and service.
There is not a hood exhaust system. There is no grease-producing cooking being done.
It is recommended that the grease trap be cleaned at least every two months. Maintain records of grease trap cleaning for Health Department review.
It is recommended that a monthly pest control program be implemented. Maintain records of pest control treatments for Health Department review.
Water heater: State SB6-52-15

  • Nonfood-Contact Surface/Corrosion Resistant/Nonabsorbent
    Observation: The Leader 2DR refrigerator has protective plastic sheeting on the doors from when it was new. The surface is not smooth and easily cleanable.
    Correction: Remove the plastic sheeting. All equipment surfaces must be easily cleanable.
  • Plumbing / Maintained in Good Repair
    Observation: The following plumbing issues were observed:
    1) cold water faucet handle at the three vat sink leaks when the water is turned on.
    2) the handsink drink drains slowly.

    Correction: Repair both plumbing problems before the first routine inspection in approximately 30 days.
