The purpose of today's visit was to conduct a follow-up assessment to verify presence of Certified Food Manager (CFM) in food establishment. No CFM card was received, as requested by Environmental Health Specialist. Called food establishment on 3/11/15 requesting copy of CFM card. CFM was not on site on 3/11/15, as verified by employee. Left a voicemail requesting to speak to CFM. Since no correspondence was received, an in-person visit conducted 3/12/15 in which employee had valid Prometric certificate but no CFM card.
  • Clean Ice Bins, Beverage Dispenser Nozzles, Etc. per Manufacturer Specs. (repeated violation)
    Observation: Surfaces of ice bins, beverage dispensing nozzles, and enclosed components of equipment are not being cleaned as required. INTERIOR OF ICE BIN COVERED IN PINK MOLD AND DEBRIS ACCUMULATION.
    Correction: PLEASE WASH, RINSE, AND SANITIZE ICE BIN AT A MIMIMUM OF BIWEEKLY TO AVOID ACCUMULATION. Surfaces of utensils and equipment contacting food that is NOT potentially hazardous (time/temperature control for safety food) such as ice bins, beverage dispensing nozzles, and enclosed components of equipment such as ice makers, cooking oil storage tanks and distribution lines, beverage and syrup dispensing lines or tubes, coffee bean grinders, and water vending equipment at a frequency specified by the manufacturer or at a frequency necessary to preclude accumulation of soil or mold.

The purpose of today's visit was to conduct a routine assessment. Please obtain copy of Certified Food Manager Card (CFM) within 24 hours. Operation has been closed due to lack of CFM on-site. When you have obtained your CFM card, please contact the Health Department so that approval can be granted for reopening. Suggested having multiple employees with CFM card in the event that manager cannot be present on-site. Also suggested making photo copies of CFM card in the event that card is forgotten at home. As previously stated before, please do not use hand sink to store items. Hand sinks are designated for hand washing only and storing items in them is a deterrent to proper hand washing techniques. A follow up inspection will be conducted in 10 days to verify CFM.
  • Wiping Cloths/Use Limitation/Wet Towel Storage (corrected on site)
    Observation: Wet wiping cloths used for wiping counters and other equipment surfaces were observed stored on IN HAND WASHING SINK, COUNTER TOP.
    Correction: Wet wiping cloths are to be used for wiping counters and other equipment surfaces and are to be held in a chemical sanitizer solution at the appropriate concentration between uses. WET WIPING CLOTHS PLACED IN BUCKET.

    Observation: There is no Certified Food Manager present in the establishment. ESTABLISHMENT HAS BEEN CLOSED DUE TO LACK OF CFM. CFM AT OTHER BUSINESS ESTABLISHMENT IN MARYLAND DURING THE TIME OF THE INSPECTION. NO OTHER EMPLOYEE POSSESS CFM CARD OR FOOD MANAGER CERTIFICATE.
    Correction: PLEASE OBTAIN CFM CARD WITHIN 24 HOURS. ESTABLISHMENT WILL REMAINED CLOSED UNTIL CFM IS AVAILABLE AND APPROVAL GRANTED BY HEALTH DEPARTMENT. It shall be unlawful to operate a food establishment unless it is under the immediate control of a certified food manager. It is highly suggested that you have more than one employee with the food manager's license. ORS Interactive, Inc. (see handout provided) issues the required photo identification card with proof of successful completion of a certified food manager's exam. Failure to have a certified food manager on site during ALL hours of operation including food preparation, food service, and cleaning/sanitizing of equipment/utensils will result in closure of the establishment.
  • Handwashing Sink / Used for Hand Wash Only (corrected on site) (repeated violation)
    Observation: Observed the handsink at the COFFEE STATION AND SANDWICH STATION being used WET WIPING BUCKETS, WIPING CLOTHS, SPATULAS.
    Correction: Handsinks are to be used only for washing hands to encourage proper hand washing and to prevent contamination of cleaned hands. ITEMS REMOVED FROM HAND SINK.

    Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device:--OBSERVED BREADED CHICKEN BREAST AT 48F, EGG PATTIES 47F, SAUSAGE PATTIES 46F, TUNA SALAD 46F INSIDE THE DELFIELD 4 DRAWER PREP COOLER.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth.--CERTIFIED FOOD MANAGER (CFM) STATED THAT THE COOLER WAS RESTOCKED WITH THESE ITEMS AT 2:00 PM. FOOD WAS MOVED TO THE WALK-IN COOLER. DO NOT PLACE POTENTIALLY HAZARDOUS FOODS (PHF'S) INSIDE THIS UNIT UNTIL IT CAN HOLD AT 41F OR BELOW. CFM WILL FAX OR EMAIL THE REPAIR INVOICE TO THE HEALTH DEPARTMENT FOR THIS UNIT.
  • Equipment / Good Repair / Operation
    Observation: The following refrigeration unit is not operating properly and is unable to maintain cold food at or below 41°F: --OBSERVED DELIFILED 4 DRAWER PREP COOLER AT 47F.
    Correction: Repair/adjust the unit so it is able to maintain foods at or below 41°F. Do not use the unit to store potentially hazardous foods while it is not operating properly.--CERTIFIED FOOD MANAGER(CFM) STATED THAT THIS COOLER WAS RESTOCKED AT 2:00 PM. POTENTIALLY HAZARDOUS FOODS(PHF'S) WERE MOVED TO THE WALK-IN COOLER. DO NOT PLACE PHF'S INSIDE THIS UNIT UNTIL IT CAN HOLD AT 41F OR BELOW. CFM WILL FAX OR EMAIL A COPY OF THE REPAIR INVOICE FOR THIS UNIT TO THE HEALTH DEPARTMENT.

    Observation: Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy.
    Correction: A complete employee health policy must have the following elements:
    1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout)
  • Critical: Eating, Drinking, or Tobacco Use / Proper Food Employee Drinking Containers (corrected on site)
    Observation: Open drinking containers stored in a manner that may contaminate food, food contact surfaces or utensils.--OBSERVED OPEN DRINKING CONTAINER ON COFFEE MACHINE
    Correction: Food employees may have beverages in the kitchen areas as long as they are covered and consumed through a straw and stored in a manner that will prevent contamination to food, equipment, utensils, linens, and single service items. --CFM REMOVED DRINKING CUP.
  • Handwashing Sink / Accessible at All Times
    Observation: The handwashing facility located at the BACK OF THE KITCHEN is blocked, preventing access by employees for easy handwashing.--OBSERVED BLOCKED HAND SINK BY LADDER, COOLING RACK AND KELVINATOR FREEZER.
    Correction: A handwashing sink shall be maintained so that it is accessible at all times for employee use.
