The purpose of this visit was to conduct a routine inspection. Please submit service report or invoice for repair of 3DR prep cooler within five days. Thank you for your time today. If you have any questions, please call (703) 246-2444.
  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (repeated violation)
    Observation: The following food items were observed cold holding at improper temperatures using a calibrated food temperature measuring device: tuna at 58F and sliced tomatoes 50F.
    Correction: Potentially hazardous foods (time/temperature conrol for safety food) shall be held cold at a temperature of 41°F or below. Placed on ice to cool 41F.
  • Equipment / Good Repair / Operation (repeated violation)
    Observation: The following refrigeration unit is not operating properly and is unable to maintain cold food at or below 41°F: 3DR prep cooler at 50F.
    Correction: Repair/adjust the unit so it is able to maintain foods at or below 41°F. Do not use the unit to store potentially hazardous foods while it is not operating properly.
  • Plumbing / Maintained in Good Repair
    Observation: Plumbing connections for the 3-vat sink piping are leaking.
    Correction: A plumbing system shall be maintained in good repair.

The purpose of todays' visit is to conduct a routine inspection.
Please fax following items to my attention within 2 weeks:
1) CFM card for Sun Obrien. CFM card expired 1/11/14.
2) Copy of updated menu with CA for eggs. Burger always fully cooked.
REMINDERS:
**Focus on CRITICAL and REPEAT violations to avoid future enforcement actions. Some violations may require TRAINING and MONITORING of employees to ensure that they remain corrected.
REHEATING METHODS:
**Potentially hazardous foods (time/temperature control for safety food) that is cooked, cooled, and reheated for hot holding shall be reheated to a minimum temperature of 165°F for 15 seconds.

  • Critical: Handwashing / When to wash hands (corrected on site)
    Observation: A food employee failed to remove gloves and wash hands before engaging in food preparation, after touching bare breaking raw shell eggs.
    Correction: ALL food employees shall wash their hands OFTEN especially after handling trash, using the restroom, handling raw meats, handling soiled equipment and utensils, BEFORE putting on clean gloves, etc. Proper handwashing frequencies will aid in the reduction of foodborne illness.
  • Hair Restraint Effectiveness (corrected on site) (repeated violation)
    Observation: Employees observed working in the food service area without proper hair restraints.
    Correction: Food employees shall wear hair restraints such as hats, visors, hair nets or beard nets to effectively keep their hair from contacting exposed food, clean equipment, utensils, linens, and unwrapped single-service items. This section does not apply to food employees such as counter staff who ONLY serve beverages and wrapped or packaged foods, hostesses, and wait staff.
  • Critical: No Bare Hand Contact with RTE Foods (corrected on site)
    Observation: A food employee was observed handling the following ready-to-eat food using their bare hands: cabbage for cole slaw.
    Correction: Except when washing fruits and vegetables, food employees may not contact ready-to-eat foods using their bare hands. Employees shall use suitable utensils such as deli tissue, spatulas, tongs, or clean disposable gloves to handle all ready-to-eat foods. Note: Washed the cabbage again.
  • Food Storage Containers, Identified with Common Name of Food (corrected on site) (repeated violation)
    Observation: The following food items that are not easily identified by appearance were observed without a label: sugar, salt , rice
    Correction: Unless a food item can be easily recognized, all foods and ingredients removed from their original containers and stored in working containers shall be labeled using the common name of the food. For example: cooking oils, salt, sugar, flour, spices, herbs, etc.
  • Cooling Methods (corrected on site) (repeated violation)
    Observation: The following methods used for cooling were not adequate to facilitate proper cooling: observed cooked chicken and home fries cooling with lid on.
    Correction: Cooling hot foods shall be accomplished as follows: 1) placing the food in shallow pans
  • Critical: Hot Holding of Potentially Hazardous Food at 135°F or above (corrected on site) (repeated violation)
    Observation: The following food item were found hot holding at improper temperatures using a calibrated food temperature measuring device: chili 130F reheated to 167F.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be hot held at a temperature of 135°F or above unless the permit holder is using "time as public health control" as specified under 3-501.19.
  • Critical: Date Marking of Refrigerated, Ready-to-Eat PHF (TCS Food) (corrected on site) (repeated violation)
    Observation: The following refrigerated, ready-to-eat, potentially hazardous food that is commercially processed is not used or discarded within 24 hours of when the original container was opened and is not observed to be date marked: deli meat, sliced american cheese
    Correction: Refrigerated, ready-to-eat, potentially hazardous commercially processed foods when opened and kept for more than 24 hours cannot have a shelf-life of more than 7 days, including the day of the original container was opened. These food items must be marked with either the "consume-by" date and/or "preparation" date, color-coded labels, or another marking system/method that effectively indicates shelf-life that is clearly understood and properly used by the employees. This date cannot exceed any manufacturer determined "use-by" date based on food safety.
  • Critical: Consumer Advisory Required for Raw or Undercooked Animal Food
    Observation: A review of the menu with the foodservice operator indicates that there is no consumer advisory for the following food item that may be served raw and/or undercooked: Eggs on menu board and printed menu.
    Correction: If an animal food such as beef, eggs, fish, lamb, milk, pork, poultry, or shellfish is served or sold raw or undercooked the permit holder shall inform consumers of the significantly increased risk of consuming such foods by way of a disclosure and reminder using brochures, deli case or menu advisories, label statements, table tents, placards, or other effective written means. Note: Menu board corrected during inspection. They will fax a copy of printed menu with corrected CA.
  • Single-Service and Single-Use Articles / No Reuse (repeated violation)
    Observation: Manufacturer containers of cottage cheese were observed reused for the storage of chicken broth.
    Correction: Manufacturer containers may not be reused for secondary storage of other foods because the containers are not durable enough for multiple reuse and become uncleanable.
  • Cleaning Frequency for Physical Facilities
    Observation: Observed that the ceiling tile and vent above the hood system in the kitchen is in need of cleaning.
    Correction: Clean the physical facilities at a frequency necessary to ensure the protection and safe preparation of food.

Please fax invoice for fixing the 3DR prep refrigerator and Superior 1DR display refrigerator (use to hold packaged milk) within 10 days.

Note:
1) The facility serves eggs and burgers fully cooked. No consumer advisory required.
2) The facility agrees to use Time as a Public Health Control 4 hours for fries and partially fried chicken.--not using TPHC anymore. They will either hot hold it or cool it.
REMINDERS:
**Focus on CRITICAL and REPEAT violations to avoid future enforcement actions. Some violations may require TRAINING and MONITORING of employees to ensure that they remain corrected.
**Do NOT allow food items to sit out at room temperature during busy times because it is convenient. Potentially hazardous food items must be held HOT at or above 135F or COLD at or below 41F.

Note: All packaged milk discarded. chicken salad 51F, tuna salad 51F, shredded cheese 54F, sliced turkey 49F and sliced ham 49F--3DR prep top. Sliced ham 48F, sliced cheese 48F--Victory 3DR prep refg. Note: All items were refilled after lunch per the manager. Put all the items in upright refrigerator.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth.

    Correction: Non-food contact surfaces of equipment that are exposed to splash, spillage, or other foods that require frequent cleaning shall be constructed of a corrosion-resistant, nonabsorbent, and smooth material.

    Observation: The following refrigeration unit is not operating properly and is unable to maintain cold food at or below 41°F: Superior display refrigerator 55F and Victory 3DR prep refrigerator 50f. Note: Adjust thermostat temperature came down to 41F for the prep refrigerator.
    Correction: Repair/adjust the unit so it is able to maintain foods at or below 41°F. Do not use the unit to store potentially hazardous foods while it is not operating properly.
  • Single-Service and Single-Use Articles / No Reuse (repeated violation)
    Observation: Manufacturer container of paprika was observed reused for the storage of ketchup.
    Correction: Manufacturer containers may not be reused for secondary storage of other foods because the containers are not durable enough for multiple reuse and become uncleanable.
  • Handwashing Sink / Accessible at All Times
    Observation: The handwashing facility located at the 3-vat sink is blocked, preventing access by employees for easy handwashing.
    Correction: A handwashing sink shall be maintained so that it is accessible at all times for employee use.
