CPO present at the time of inspection but has not received her certificate.. Establishment in compliance with required corrections. Walls in the walk-in refrigerator were replaced and resealed. Baseboards replaced or resealed throughout the establishment. Ceiling tiles above the equipment storage shelf were not replaced. Spoke with district manager, tiles will be replaced immediately. Facility much improved and is operating well. No pest activity observed.

Will be the general manager until other manager's obtain their serve-safe certification. 4 Employees enrolled in 5/05/14 food handlers class and remaining employees will be completed by Friday of the next week. Remaining violations will be corrected for the final follow-up inspection. Final follow-up scheduled for on or about June 02, 2014. Contacted supervisor with inspection results. Verbal authorization to re-open.

No CFM present at anytime of inspection. No temperature logs used. Heavy infestation of mice observed. Office contacted with inspection results. Establishment agreed to voluntarily close until facility is cleaned and has been treated by a licensed exterminator. Must provide invoice from exterminator. Informed owner to remain closed and to call EHS once proper sanitary conditions are sustained and the entire facility is ready for reopening inspection. Must have a hired CFM to reopen. May reopen when re-inspection is complete and satisfactory to the department.
