    Observation: There is no Certified Food Manager present at the beginning of the inspection.
    Correction: It shall be unlawful to operate a food establishment unless it is under the immediate control of a certified food manager. It is highly suggested that you have more than one employee with the food manager's license. ORS Interactive, Inc. (see handout provided) issues the required photo identification card with proof of successful completion of a certified food manager's exam. Failure to have a certified food manager on site during ALL hours of operation including food preparation, food service, and cleaning/sanitizing of equipment/utensils will result in closure of the establishment.     Observation: The person in charge (PIC) has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc.
    Correction: Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. (see handout provided). A copy of the photo identification card can be faxed to the food safety section at (703) 653-9448. Failure to provide a copy of the photo identification card may result in further enforcement action.

    Observation: Observed that sign or poster that notifies food employees to wash their hands is not provided at all handwashing sinks used by food employees.
    Correction: Provide a sign or poster at all handwashing sinks used by food employees that reminds food employees to wash their hands.

    Observation: Observed that the wall by the walkin cooler is not maintained in good repair.
    Correction: Repair the physical facilities. Poor repair and maintenance compromises the functionality of the physical facilities.

The purpose of this visit was to conduct a follow-up inspection on the 2dr glass coke cooler. Cooler was observed at 41F and was able to keep potentially hazardous foods at required temperatures. EHS also referred to walkin cooler to confirm that the 2dr glass coke cooler was in working order.
  • Equipment / Fixed / Space / Seal (repeated violation)
    Observation: The following equipment is not installed in a manner that allows accessibility for cleaning, nor is it installed with closely abutting surfaces to minimize the need for cleaning: mopsink needs recaulking
    Correction: Equipment that is fixed because it is not easily movable shall be installed so that it is: 1) spaced to allow access for cleaning along the sides, behind, and above the equipment, 2) spaced from adjoining equipment, walls, and ceilings a distance of not more than 1 millimeter or one thirty-second inch, and 3) sealed to adjoining equipment or walls if the equipment is exposed to spillage or seepage.

    Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: feta cheese 45F, cooked chicken 48F both in 2dr glass cooler coke
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth. CFM removed all potentially hazardous food items and placed in walkin cooler

The purpose of today's visit was to conduct a routine inspection. EHS provided manager until end of August 2014 to obtain or fax/email the CFM photo card. Please continue to monitor and check 3DR prep cooler to verify that it is able to maintain 41F or below.
  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site) (repeated violation)
    Observation: The following food item(s) in the bottom portion of the Delfield 3DR prep cooler were observed cold holding at improper temperatures using a calibrated food temperature measuring device:
    1) Coocked chicken at 44F,
    2) Cooked beef at 44F,
    3) Cooked pasta at 45F.

    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth. MOVED SOME ITEMS IN BOTTOM SECTION OF PREP COOLER TO WALKIN COOLER TO CHILL.
  • Temp Meas Devices for Ambient Air Location
    Observation: The temperature measuring device in the following mechanically refrigerated unit was not properly located in the warmest part of the unit (front of unit):-------Delfield 3DR prep cooler.
    Correction: In a mechanically refrigerated unit the sensor of a temperature measuring device shall be located to measure the air temperature of a simulated product temperature in the warmest part of the refrigerator. Store the thermometer near the door of the unit.

    Observation: Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy.
    Correction: A complete employee health policy must have the following elements:
    1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout)

    Observation: The following food items were found subject to contamination after receipt and during storage due to the condition as described: OBSERVED A FLY CATCHER CONTAINING DICHOLOROVAS PLACED ABOVE SHELF OVER BOXES OF FOOD.
    Correction: Food shall be protected from contamination that may result from a factor or source not specified under Subparts 3-301 through 3-306. CORRECTED: CFM REMOVED THE ITEM FROM THE AREA.

    Observation: The following food items were observed cold holding at improper temperatures using a calibrated food temperature measuring device:OBSERVED CHICKEN NUGGETS AT 49F, CUT SAUSAGES AT 47F, SLICED PROVOLONE AT 45F AND DICED TOMATOES AT 47F IN THE 2 DR GLASS FRONT REFRIGERATOR ( BACK )
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth. CORRECTED: INSTRCUTED CFM TO MOVE ALL ITEMS TO THE WALK IN REFRIGERATOR.
  • Food Temperature Measuring Devices, F Scale, Accurate within 2°F (corrected on site)
    Observation: The food temperature measuring devices located on the premises are not accurate in °F:
    Correction: Food temperature measuring devices that are scaled only in Fahrenheit shall be accurate to +/- 2°F in the intended range of use. CORRECTED: ALL THERMOMETERS WERE CALIBRATED DURING THE INSPECTION.
  • Permit to Operate Required, Must be Posted, Must be Valid (corrected on site)
    Observation: The establishment has not posted the permit to operate in a conspicuous location.
    Correction: No person shall operate a food establishment unless that person possesses a valid permit issued by the Director. Only a person who complies with the requirements of this Chapter shall be entitled to receive or retain such a permit. Permits are not transferable from one owner to another. A valid permit shall be posted in every food establishment in a conspicuous place in the public view. Food establishment permits are issued for a calendar year and expire on December 31 regardless of the month of issue. CORRECTED: CFM OBTAINED THE PERMIT AND WILL DISPLAY IT IN FRONT AREA.

    Observation: Plumbing connections under the 3 vat sink piping are leaking.Also repair the leak in the hose.
    Correction: A plumbing system shall be maintained in good repair.

    Observation: Observed that the 3 VAT sink/wall junction area is not maintained in good repair.OBSERVED CAULKING NEEDS TO BE REDONE AND CLEAN THE BLACK MOLD/MILDEW FROM THE AREA.
