This visit was made to conduct a follow up inspection. At the time of the inspection there was no one present with a certified food protection manager card (NOVA).
Management is in the process of updating the consumer advisory, and updated copy will be emailed to me for approval.
A civil penalty in the amount of 100.00$ has been issued (F-16-04300) for violation of 11-2-24 No CFM present (NOVA). Repeat violation.

  • Certified Food Manager: Certificate Process (repeated violation)
    Observation: The Person in Charge (PIC) has a valid certificate of successful completion of an accredited Certified Food Manager (CFM) exam, but does not possess the Northern Virginia CFM card.
    Correction: Obtain a Northern Virginia CFM card from ORS Interactive, Inc. within 10 business days. A copy of the Northern Virginia CFM card can be emailed to the Environmental Health Division at Kendra.Washington@vdh.virginia.gov. Failure to provide a copy of the photo identification card may result in further enforcement action.

This visit was made to conduct a routine food inspection. The following items require long term correction:
1. While there were several staff members with a food safety manager certification, no one on site had a Northern Virginia Certified Food Protection Manager card at the time of the visit.
Establishment shall be under the control of someone holding a valid Northern Virginia Certified Food Protection Manager card at all operating times. Card is to be obtained from ORS Interactive at 6066 Leesburg Pike, Suite. 200B Falls Church Va within 10 business days. A follow up visit will be conducted to verify that the cards have been obtained.
2. Food employees shall clean their hands during food preparation as often as necessary to remove contamination and to prevent cross contamination when changing tasks. Examples include when switching between working with raw food and working with ready-to-eat food, before putting on gloves for working with food, after handling soiled equipment or utensils, after using the toilet room, after touching bare human body parts, after coughing, sneezing, eating, and drinking, and after engaging in other activities that contaminate hands.
3. Employees must not touch ready to eat foods (salads, pita bread, lemons and drink garnishes) with their bare hands. A new pair of gloves, or tongs, or another utensil must be used to handle these items.
4.Facility must work towards achieving active managerial control. Active managerial control requires training and monitoring of employees to ensure staff are following food safety handling procedures and practices. Please work with staff to ensure procedures are being followed.

    Correction: ALL food employees shall be washing their hands OFTEN especially after handling trash, using the restroom, handling raw meats, handling soiled equipment and utensils, BEFORE putting on clean gloves, etc. Proper handwashing frequencies will aid in the reduction foodborne illness.     Observation: Employees wearing jewelry on their arms and hands while preparing food. Observed bartender, and several chefs wearing bracelets, watches and jewelry.
    Correction: Ensure employees are not wearing jewelry on their arms and hands while preparing food. This section does not apply to medical alert jewelry or a plain ring such as a wedding band.

    Observation: Observed several bottles of water above kitchen line, and in areas around kitchen.
    Correction: Employee may drink from a cup with a lid and straw. Employees’ drinks may be kept on the bottom shelf of a worktable while employees are actively working with food. (discussed proper beverage containers with PIC)

    Observation: Tags missing from the molluscan shellfish containers. Observed bag of mussels in walk in with no tags attached.
    Correction: Shellstock tags must remain attached to the container in which the shellstock are received until the container is empty. (instructed manager to properly identify mussels, or discard if tags could not be found. Discussed shell tag requirements with PIC)

    Correction: Cloths in use for wiping counters and other equipment surfaces shall be held between uses in a chemical sanitizer solution at a specified concentration. A liner that can be washed rinsed and sanitized may be used to stabilize cutting boards.

    Observation: The following food item(s) were measured at improper temperatures: tzatzki, lettuce, chicken salad, lettuce, sliced tomatoes, feta (make table), sausage, mozzarella cheese (pizza prep cooler). The tzatki was discarded and all foods will be discarded after lunch. During the lunch service all temperature controlled foods were placed on ice.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below.
  • Food Contact Surface/Smooth, Easily Cleanable
    Observation: The food-contact surfaces of the following equipment and/or utensils are not finished to have a smooth, cleanable surface: the lid on the flour bin is cracked and tapped together. Replace the lid.
    Correction: Materials that are used in the construction of utensils and food-contact surfaces of equipment may not allow the migration of deleterious substances or impart colors, odors, or tastes to food and under normal use conditions shall be finished to have a smooth, easily cleanable surface.
  • Equipment / Good Repair / Operation
    Observation: The following mechanical refrigeration unit is not currently operating as required to cold hold food at a temperature of 41°F or less: the make table refrigerator.
    Correction: Refrigerators shall be capable of holding foods at a temperature of 41°F or below. Repair the unit(s) immediately. Foods exposed to temperature abuse for over 2 hours shall be discarded, otherwise, the foods shall be stored in properly operating refrigerators.
  • Equipment / Good Repair / Components / Gaskets
    Observation: The door gaskets of the following unit(s) are damaged: make table refrigerator.
    Correction: Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications.
  • Equip/Utensils/Linens/Single-Service/Storage, Location (corrected on site)
    Observation: Clean containers were found stored on the floor. Containers were washed and relocated during the inspection.
    Correction: Cleaned equipment and utensils, laundered linens, and single-service items shall be stored in a clean, dry location where they are not exposed to splash, dust, or other contamination, and at least 6 inches off the floor on NSF approved shelving units or dunnage racks.

    Observation: Clean food containers were found handled, displayed or dispensed with the food or lip-contact surface facing upward.
    Correction: Single-service items and cleaned/sanitized utensils shall be handled, displayed, and dispensed so that contamination of food and lip contact surfaces are prevented.

    Observation: Harborage conditions exist. Soiled cleaning equipment observed in the mop sink area. Increase pest control service to eliminate existing fruit flies.

    Observation: The following farm-raised fish served or sold in a ready-to-eat form without freezing for parasite destruction is not properly identified as such: salmon.
    Correction: If raw or partially cooked fish are served or sold in ready-to-eat form and the fish is raised and fed as specified by Law, a written agreement or statement from the supplier or aquaculturist stipulating that the fish were raised and fed shall be obtained by the person in charge and retained in the records for 90 calendar days beyond the time of service or sale to a consumer.

    Observation: The following methods used for cooling were not adequate to facilitate cooling of the product from 135°F to 41°F within 6 hours: gyro meat was improperly cooling on top of the make table refrigerator. The gyro meat was thinly sliced and refrigerated inside the cooler.
    Correction: Cooling hot foods shall be accomplished as follows: 1) placing the food in shallow pans, 2) separating the food into smaller or thinner portions, 3) using rapid cooling equipment, 4) stirring the food in a container placed in an ice water bath, 5) using containers that facilitate heat transfer, 6) adding ice as an ingredient, or 7) other effective methods.

    Observation: Due to improper operation of the mechanical warewashing machine, equipment food-contact surfaces and utensils are not sanitized after cleaning. (See 4-501.112) Dishmachine not at proper wash or rinse temperature.
    Correction: Hot water warewashing machines shall achieve a utensil/equipment surface temperature of at least 160°F as measured by an irreversible registering temperature indicator. The thermolabel provided on the equipment/utensils shall turn black if the warewashing machine is properly heat sanitizing as required. If the thermolabel does not turn black, the warewashing machine shall not be used. Employees will have to set up the 3-vat sink to manually wash, rinse, and chemically sanitize all equipment, utensils, dish ware, glass ware, etc. A follow-up inspection will be conducted within 24 hours to ensure the machine has been repaired.

    Correction: Refrigerators shall be capable of holding foods at a temperature of 41°F or below. Repair the unit(s) immediately. Foods exposed to temperature abuse for over 2 hours shall be discarded, otherwise, the foods shall be stored in properly operating refrigerators.
  • Handwashing Cleanser, Available at Handwashing Facilities (corrected on site)
    Observation: Observed that hand soap was not provided at each handwashing sink.
    Correction: Provide hand soap at each hand sink to allow employees to properly wash their hands.

This visit was conducted to complete a routine food safety evaluation:
1. Ensure that Shell stock tags are maintained correctly.
2. Ensure that prepared foods in the walkin cooler are correctly dated.
3. The manager was quick to resolve issued raised.

  • Molluscan Shellstock/Maintaining ID/Tags Attached until Container Emptied (corrected on site) (repeated violation)
    Observation: Tags missing from the molluscan shellfish containers.
    Correction: Shellstock tags shall remain attached to the container in which the shellstock are received until the container is empty.
  • Molluscan Shellstock/Maintaining ID/Recording the Date Sold (repeated violation)
    Observation: The date on which the last shellstock from the container is sold or served was not recorded on the tag or label.
    Correction: The date when the last shellstock from the container is sold or served shall be recorded on the tag or label.
  • Utensils That are In-Use / Between-Use Storage (corrected on site) (repeated violation)
    Observation: Knives were observed stored between preparation counters. This is not a clean surface.
    Correction: During pauses in food preparation or dispensing, food utensils shall be stored in one of the following manners: 1) in the food with their handles above the top of the food and the container, 2) in food that is not potentially hazardous (time/temperature control for safe food) with their handles above the top of the food within containers or equipment that can be closed, 3) on a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food contact surface of the food preparation table or cooking equipment are cleaned and sanitized, 4) in running water of sufficient velocity to flush particulates to the drain, 5) in a clean, protected location if the utensils are used only with a food that is NOT potentially hazardous (time/temperature control for safe food), or 6) in a container of water if the water is maintained at a temperature of at least 135°F and the container is cleaned frequently.
  • Food Storage / Preventing Contamination (corrected on site)
    Observation: Food stored on the floor and/or food stored less than 6 inches off the floor.
    Correction: Food shall be protected from contamination by storing the food at least 6 inches off the floor on approved shelving units or dunnage racks. Milk crates, soda crates, or bread racks are not suitable for food storage.

    Observation: The following food item(s) were measured at improper temperatures: Sliced tomatoes were observed at 48°F, Diced tomatoes at 49°F, Crab cakes 43°F, Chicken wings 47°F and mozzarella at 46°F.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below. (Moved / Discarded)
  • Date Marking of RTE, PHF(TCS) Prepared On-Site, Stored at 41°F or less (corrected on site)
    Observation: Dating of food in the walk-in refrigerator was not consistent. Ensure that food is correctly dated, and old labels removed.
    Correction: Refrigerated, ready-to-eat, potentially hazardous foods (time/temperature control for safety food) that are prepared on site and held at 41°For below shall be clearly labeled and date marked with a "USE BY" not exceeding 7 TOTAL DAYS. If the food is held for only 24 hours, no date marking is required. For example, if foods like leftover soups, chili, tuna salad, potato salad, pasta salad, etc. are prepared on @DATE@ they shall be date marked with a "use by" date not exceeding @DATE@.

This visit was conducted to complete a risk factor based food safety evaluation.
1. You must ensure that there is someone on site who holds the Northern Virginia Food Manager's Card. Please obtain this card within the next 10 business days, and fax a copy to our offices. If I do not receive a copy of this, a follow up visit will be have to be carried out to check compliance.
2. Ensure that the shell stock tags remain with the shell stock until the last position is sold. At this point, the date should be recorded on the tag before it is filed. This was raised at your last inspection, and must be resolved. If a further violation is observed, a civil penalty ticket will be issued.
3. The manager was knowledgeable with regard to employee health.

  • Certified Food Manager: Certificate Process
    Observation: The Person in Charge (PIC) has a valid certificate of successful completion of an accredited Certified Food Manager (CFM) exam, but does not possess the Northern Virginia CFM card.
    Correction: Obtain a Northern Virginia CFM card from ORS Interactive, Inc. within 10 business days. A copy of the Northern Virginia CFM card can be faxed to the Environmental Health Division at 703.746.4910. Failure to provide a copy of the photo identification card may result in further enforcement action.
  • Eating, Drinking, or Tobacco Use / Proper Food Employee Drinking Containers (corrected on site)
    Observation: A food handler was observed drinking from an uncovered beverage container while in the kitchen.
    Correction: Food employees may drink only from a covered container using a straw.
