Observation: The ambient air temperature measuring device (degrees F) located in the reach-in freezer is broken and therefore is not accurate.
Correction: Provide a temperature measuring device that is accurate within +/- 3 degrees Fahrenheit in the intended range of use.
- Temperature Measuring Devices (repeated violation)
Observation: There was no temperature measuring device located in the display reach-in cooler on the breakfast bar.
Correction: Provide a temperature measuring device in all hot or cold holding equipment used to store potentially hazardous foods so that employees can routinely monitor the ambient air temperature.
- Critical: Equipment and Utensils - Before Use After Cleaning* (corrected on site)
Observation: The food-contact surfaces of the equipment were not observed sanitized. Observed the breakfast attendant washing equipment in the two basin sink but did not sanitize the equipment. There was no sanitizer on site available to prepare the sanitizing solution. The person in charge (PIC) immediately purchased some bleach and the sanitizing solution was set up in the two basin sink at a concentration of 50 ppm.
Correction: After cleaning and rinsing of the food-contact surface, the surface shall be effectively sanitized before coming in contact with food and before use.
- Critical: Toxics - Common Name/working Containers of Toxics* (corrected on site)
Observation: Working container (spray bottle) of all purpose cleaner is not properly labeled. The PIC immediately labeled the bottle.
Correction: Working containers of toxic items are to be properly labeled with contents. Identification of these containers with the common name of the material helps prevent the dangerous misuse of the contents.

Observation: Two rodent bait stations are not properly stored to prevent the contamination of food, equipment, utensils, linens or single service items. Observed the rodent bait stations stored in two cabinets with coffee filters, bags of coffee and bags of oatmeal. The PIC discarded both rodent bait stations.
Correction: Containers of poisonous or toxic materials must be located in an area that is not above food, equipment, utensils, linens or single service items.

FDA Form 1-B provided to post in the employee area. 2. Ensure that all spray bottles of cleaners are labeled. Observed one spray bottle unlabeled. Observed the overall breakfast facility very clean and well maintained.

Observation: Employees or applicants are not aware of the reporting procedures concerning information about their health as they relate to the symptoms caused by illness, infection, or other source that is associated with an acute gastrointestinal illness or a lesion.
Correction: Have the license holder require employees or applicants report to the person in charge if they are experiencing the following symptoms or conditions: (1) diarrhea, fever, vomiting, jaundice, or sore throat with fever. (2) A lesion containing pus such as a boil or infected wound that is open or draining and is on the hands or wrists, unless an impermeable cover such as a finger cot or stall protects the lesion and a single-use glove is worn over by an impermeable cover or on other parts of the body, unless the lesions covered by a dry durable, tight-fitting bandage.

Correction: Provide a temperature measuring device in all hot or cold holding equipment used to store potentially hazardous foods so that employees can routinely monitor the ambient air temperature.

Observation: The person in charge could not provide a food temperature measuring device.
Correction: Obtain at least one food temperature measuring device so employees can properly monitor the temperature of food items in the establishment.
