The purpose of this visit was to conduct a risk factor assessment. The following good retail practice was discussed during the inspection: 1) Provide hot water at a minimum temperature of 110 degrees F. to the 3 vat sink. NOTE: PLEASE REMEMBER TO PROVIDE SERVICE INVOICE FOR HANDSINK HOT WATER TEMPERATURE (MINIMUM 100 DEGREES F) TO THE HEALTH DEPARTMENT WITHIN 10 DAYS.
Handwashing Sink / Mix Valve or Combination Faucet , Water at 100°F (repeated violation) Observation: Water from the handwashing sink at @LOCATION@ was measured at a temperature less than 100°F: 86 degrees F.
Correction: A handwashing sink shall be equipped to provide water at a temperature of at least 100°F through a mixing valve or combination faucet. NOTE: Provide service invoice to Health Department within 10 days confirming handsink is supplied with water of 100 degrees F minimum.

Critical: Responsibility of PIC to Require Food Employees to Report GI Symptoms or Cuts, Disease, Medical Condition or Exposure to PIC Observation: Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy.
Correction: A complete employee health policy must have the following elements:
1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout)
Food Temperature Measuring Device Required & Readily Accessible Observation: The certified food manager could not provide a food temperature measuring device.
Correction: A metal-stemmed probe food thermometer shall be provided and readily accessible for use in ensuring attainment and maintenance of food temperatures.

PREOPERATIONAL FINAL INSPECTION *Milk if stored on sited shall be stored in an NSF certified or equivalent refrigerator. *This is a Catering-in only Food Service. Food shall be prepared elsewhere, transported ready-eat (hot or cold) in individually portioned containers to the serving location here at the school. The pre-portioned individual bags are given to students. *Approval for a Health Department Permit is hereby granted. This inspection report shall serve as your permit until official permit is delivered to you. The following items to be corrected: 1. Install soap and paper towel dispensers at the hand washing sink. 2. Provide a thermometer for the walk-in refrigerator. 3. Install a mop rack at the mop sink. No violation noted during this evaluation.
