The purpose of this visit was to conduct a routine inspection. Quaternary ammonium solution 3 vat sink: 300 ppm. NOTE: PLEASE REMEMBER TO PROVIDE COPY OF SERVICE INVOICE FROM LICENSED PEST CONTROL OPERATOR TO HEALTH DEPT. WITHIN 10 DAYS REGARDING RODENT INFESTATION AT DUMPSTER.
  • Critical: Handwashing / When to wash hands
    Observation: A food employee failed to wash his or her hands before engaging in food preparation, after touching bare human body parts, after coughing, sneezing, eating, after handling soiled utensils, or after engaging in any activity that could contaminate the hands.
    Correction: ALL food employees shall wash their hands OFTEN especially after handling trash, using the restroom, handling raw meats, handling soiled equipment and utensils, BEFORE putting on clean gloves, etc. Proper handwashing frequencies will aid in the reduction of foodborne illness. NOTE: Discussed with employee and with PIC.
  • Handwashing / Where to Wash (corrected on site)
    Observation: A food employee was observed cleaning their hands in a three vat sink.
    Correction: ALL food employees shall wash their hands in ONLY a designated handsink.
  • Wiping Cloths/Use Limitation/Wet Towel Storage (corrected on site)
    Observation: Wet wiping cloths used for wiping counters and other equipment surfaces were observed stored on the counter.
    Correction: Wet wiping cloths are to be used for wiping counters and other equipment surfaces and are to be held in a chemical sanitizer solution at the appropriate concentration between uses. NOTE: Discussed with PIC.
  • Equipment & Utensils / Air-Drying or Adequate Draining Required (repeated violation)
    Observation: Metal pans were found stacked while wet after cleaning and chemical sanitization.
    Correction: After cleaning and sanitizing, equipment and utensils shall be air-dried or used after adequate draining before contact with food.
  • Hand Drying Provision / Individual, Disposable Towels (corrected on site) (repeated violation)
    Observation: Observed that paper towels were not provided at each handwashing sink.
    Correction: Provide paper towels at each hand sink to allow employees to properly dry their hands after handwashing. NOTE: PIC provided paper towels.
  • Critical: Controlling Pests / Methods to Use When Pests Are Found (corrected on site)
    Observation: Methods are not being used to control pests. NOTE: Active rat infestation observed abound dumpster area.
    Correction: Insects and other pests are capable of transmitting disease to people by contaminating food and food-contact surfaces. If pests are found on the premises methods shall be used to control their presence.

The purpose of this visit was to conduct a routine inspection. Quaternary ammonium sanitizer 3 vat sink/wiping cloth bucket: 350/300 ppm.
  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site)
    Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: sour cream at 1 dr prep cooler FAX area.
    Correction: Potentially hazardous foods (time/temperature conrol for safety food) shall be held cold at a temperature of 41°F or below. NOTE: Food determined to be out of temperature <2 hours and was relocated to WI cooler.

  • Critical: PHF(TCS) Prepared from Ingredients at Room Temperature Cooled within 4 Hours
    Observation: The following food item prepared from ambient air temperature ingredients is not being adequately cooled to 41°F or less within 4 hours: PICO DE GALLO IN WALK-IN-COOLER AT 46F AFTER COOLING FOR 4 HOURS.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be cooled within 4 hours from 41°F or less if prepared from ingredients at ambient air temperature (i.e. reconstituted foods and canned tuna). PLACED PICO DE GALLO ON ICE AND BROUGHT THE TEMPERATURE DOWN TO 41F.
  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less
    Observation: The following food items were observed cold holding at improper temperatures using a calibrated food temperature measuring device: COLDWELL (1) SHREDDED MONTEREY JACK CHEESE AT 66F
    Correction: 1DR CONTINENTAL COOLER (2) SOUR CREAM AT 46F.

The purpose of today's visit was to conduct a routine inspection.
Grease trap is serviced twice a month.

  • Equipment & Utensils / Air-Drying or Adequate Draining Required (corrected on site)
    Observation: METAL CONTAINERS were found stacked while wet after cleaning and chemical sanitization.
    Correction: After cleaning and sanitizing, equipment and utensils shall be air-dried or used after adequate draining before contact with food. SPOKE WITH THE DISHPERSON ABOUT PROPER DRYING.
  • Permit to Operate Required, Must be Posted, Must be Valid
    Observation: The establishment has not posted the permit to operate in a conspicuous location.
    Correction: No person shall operate a food establishment unless that person possesses a valid permit issued by the Director. Only a person who complies with the requirements of this Chapter shall be entitled to receive or retain such a permit. Permits are not transferable from one owner to another. A valid permit shall be posted in every food establishment in a conspicuous place in the public view. Food establishment permits are issued for a calendar year and expire on December 31 regardless of the month of issue.
  • Certified Food Manager/Certificate Process
    Observation: The person in charge (PIC) has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc.
    Correction: Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. (see handout provided) within 10 business days. A copy of the photo identification card can be faxed to the food safety section at (703) 385-9568. Failure to provide a copy of the photo identification card will result in further enforcement action.
  • Refuse / Outdoor Containers /Capacity
    Observation: The quantity of refuse exceeded the storage capacity of the exterior storage enclosure.
    Correction: An outside storage area and enclosure and receptacles shall be of sufficient capacity to hold refuse, recyclables, and returnables that accumulate.
  • Hand Drying Provision / Individual, Disposable Towels (corrected on site) (repeated violation)
    Observation: Observed that paper towels were not provided at each handwashing sink. OBSERVED NO PAPER TOWELS AT THE HANDSINK BY THE 3 VAT SINK.
    Correction: Provide paper towels at each hand sink to allow employees to properly dry their hands after handwashing. PROVIDED PAPER TOWELS.
  • Mops in Air-dry Position (corrected on site)
    Observation: Observed that mops are improperly stored between use.
    Correction: Store mops in a position that allows them to air dry without soiling walls, equipment, or supplies. PUT THE MOPS IN AIR-DRY POSITION.
