    Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: CHICKEN 56F, PORK EGGROLL 60F - ON COUNTERTOP AT ROOM TEMPERATURE
    Correction: GARLIC IN OIL 65F, RAW EGG 51F - COOKLINE AT ROOM TEMPERATURE

    Observation: When tested, the low temperature, chemical sanitizing warewashing machine was observed with a sanitizing rinse concentration of less than 50 ppm chlorine solution. OBSERVED DISH MACHINE SANITIZER AT 10PPM.
    Correction: Immediately discontinue use of the dishmachine. Until the machine is adequately repaired, set up the three-compartment sink to manually wash, rinse and sanitize food-contact surfaces using a sanitizing solution at the proper concentration of at least 50 to 100 ppm total chlorine. CFM CALLED FOR DISH MACHINE REPAIR. 3VAT SINK SET UP WITH PROPER LEVELS OF CHLORINE SANITIZER.

    Observation: A food employee failed to wash his or her hands before engaging in food preparation, after touching bare human body parts, after coughing, sneezing, eating, after handling soiled utensils, or after engaging in any activity that could contaminate the hands. OBSERVED FOOD EMPLOYEE RETURN TO KITCHEN AND PROCEED TO HANDLE FOOD WITHOUT WASHING HANDS UPON ENTERING KITCHEN.
    Correction: ALL food employees shall wash their hands OFTEN especially after handling trash, using the restroom, handling raw meats, handling soiled equipment and utensils, BEFORE putting on clean gloves, etc. Proper handwashing frequencies will aid in the reduction of foodborne illness.     Correction: Foods shall be protected from contamination by being stored in packages, covered containers or wrappings unless being cooled. CFM DISCARDED WONTONS AND BUCKET. CFM WILL PLACE NEW CRUNCHY NOODLES IN NEW CONTAINER WITH LID.

    Observation: The following food items that are not easily identified by appearance were observed without a label: FLOUR, SUGAR, CORN STARTCH IN DRY STORAGE CONTAINERS.
    Correction: Unless a food item can be easily recognized, all foods and ingredients removed from their original containers and stored in working containers shall be labeled using the common name of the food. For example: cooking oils, salt, sugar, flour, spices, herbs, etc.

    Observation: Wet wiping cloths used for wiping counters and other equipment surfaces were observed stored on the counter.

The purpose of today's visit was to perform a Risk Factor Assessment.
Please continue to monitor the temperature of the Beverage Air two door reach in cooler. Have the unit serviced immediately if the unit is unable to maintain foods at 41 F or colder.

  • Critical: Food Protection from Contamination / Separate Raw from RTE (corrected on site)
    Observation: Raw animal food stored over ready-to-eat food in the refrigeration unit. Raw pork was stored above uncovered fried rice in the walk in cooler.
    Correction: Raw foods like poultry, beef, pork, seafood, and eggs shall be stored properly inside refrigerators and freezers to prevent cross contamination. Raw foods shall be stored on shelving units BELOW or SEPARATED from all cooked foods, commercially processed foods, washed produce, leftover foods, etc.
  • Critical: Food Protection from Contamination / Separate Types of Raw Animal Foods (corrected on site) (repeated violation)
    Observation: Different types of raw animal foods stored in such a manner that may cause cross contamination as follows: Raw chicken was stored above raw beef in the walk in cooler and walk in freezer.
    Correction: Raw animal foods shall be sorted by their "type" and cooking temperatures while in storage inside a refrigerator or freezer. For example, if using ONE shelving unit for storage of ONLY raw foods, the foods shall be sorted in this manner: 1) raw poultry (chicken, turkey, or duck) shall be stored on the BOTTOM shelf (165°F cook temperature), 2) raw ground beef and ground pork shall be stored on the shelf above poultry (155°F cook temperature), and 3) raw pork (chops, ribs), seafood, and shell eggs shall be stored on the top shelf above beef (145°F cook temperature).

    Correction: Potentially hazardous foods shall be cooled from 135°F to 70°F within 2 hours and to 41°F within a total of 6 hours to prevent the growth of harmful bacteria. NOTE: Employees agreed to discard the rice. EHS offered guidance about proper cooling methods.

    Correction: Potentially hazardous foods (time/temperature conrol for safety food) shall be held cold at a temperature of 41°F or below. NOTE: A thermometer was provided for the Beverage Air two door reach in cooler, and the unit was adjusted. Potentially hazardous foods were relocated to other units capable of maintaining foods at 41 F or below.

  • Ice Used as Exterior Coolant, Prohibited as Ingredient (corrected on site)
    Observation: Ice used as a medium for cooling the exterior surfaces of food is also for beverage ice or used as food. Observed food being stored in the ice machine.
    Correction: After use as a medium for cooling the exterior surfaces of food, packaged foods, beverages, or cooling coils/tubes, the ice may not be used as food.

    Observation: The following cooked potentially hazardous foods are not being adequately cooled from 135°F to 70°F within 2 hours Beef cooling for 3 hours in the walkin cooler observed at 77°F.
    Correction: Potentially hazardous foods shall be cooled from 135°F to 70°F within 2 hours to prevent the growth of harmful bacteria. PRODUCT WAS DISCARDED.

    Observation: The following methods used for cooling were not adequate to facilitate proper cooling: Observed rice and noodles cooling in 6-8 inch deep plastic tubs in the walkin cooler.

    Observation: The following ready-to-eat food that is made using a combination of ready-to-eat, potentially hazardous ingredients is not properly dated for discard using the date of the first-prepared ingredient: 1) Foods that have been cooked and cooled down and held for longer than 24 hours (egg rolls, General Tso's chicken, etc.)
    Correction: Refrigerated, ready-to-eat, potentially hazardous food (time/temperature control for safety foods) that combined with additional ingredients, or portions of the same food, must retain the date marking of the earliest or first prepared ingredient.
  • Nonfood-Contact Surface/Corrosion Resistant/Nonabsorbent (repeated violation)
    Observation: The nonfood-contact surface of the following equipment is not corrosion resistant, nonabsorbent, and/or smooth:
    1) Cardboard lining shelving (remove)

    Correction: Non-food contact surfaces of equipment that are exposed to splash, spillage, or other foods that require frequent cleaning shall be constructed of a corrosion-resistant, nonabsorbent, and smooth material.

    Observation: The nonfood-contact surface of the following equipment is not designed or constructed to be easily cleanable:
    1) Observed peeling plastic on the exterior of the microwave (remove)

    Correction: Non-food contact surfaces shall be free of unnecessary ledges, projections, and crevices, and designed and constructed to allow easy cleaning and to facilitate maintenance.

    Correction: The above mentioned items shall be maintained in good repair.

    Observation: The door gaskets of the following unit(s) are (missing, damaged):
    1) Beverage Air prep cooler

    Correction: Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications.
  • Critical: Equipment / Food-Contact / Visibly Clean (corrected on site) (repeated violation)
    Observation: The following equipment/utensils were observed soiled to sight and touch: 1) Meat grinder, 2) Can opener, 3) Slicer
    Correction: Equipment food-contact surfaces and utensils shall be clean to the sight and touch. A surface that is soiled cannot be effectively sanitized.
  • Nonfood-Contact Surfaces Clean at Frequency to Preclude Accumulation
    Observation: The nonfood-contact surfaces of the following equipment had accumulations of grime and debris:
    1) Shelving,
    2) Exterior of equipment,
    3) Ventilation hood/filters,
    4) Gaskets on the reachin/walkin units,
    5) Interior of the reachin/walkin units

    Correction: Nonfood-contact surfaces of equipment and utensils shall be cleaned at a frequency necessary to preclude accumulation of soil residues.
  • Critical: Certified Food Manager/Presence Required (corrected on site)
    Observation: There is no Certified Food Manager present at the beginning of the inspection.
    Correction: It shall be unlawful to operate a food establishment unless it is under the immediate control of a certified food manager. It is highly suggested that you have more than one employee with the food manager's license. ORS Interactive, Inc. (see handout provided) issues the required photo identification card with proof of successful completion of a certified food manager's exam. Failure to have a certified food manager on site during ALL hours of operation including food preparation, food service, and cleaning/sanitizing of equipment/utensils will result in closure of the establishment.
  • Cleaning Frequency for Physical Facilities
    Observation: Observed that the following is in need of cleaning:
    1) Walls,
    2) Ceiling tiles,
    3) Floor/wall junctures underneath and behind equipment and shelving.

    Correction: Clean the physical facilities at a frequency necessary to ensure the protection and safe preparation of food.
  • Ventilation Systems, Shall Be Clean
    Observation: Intake and exhaust air ducts are not being cleaned at a frequency necessary to prevent contamination by dust, dirt, and other materials.
    Correction: Clean intake and exhaust air ducts at regularly intervals to prevent them from becoming sources of contamination.
