  • Hair Restraint Effectiveness
    Observation: Employees observed working in the food service area without proper hair restraints.
    Correction: Food employees shall wear hair restraints such as hats, visors, hair nets or beard nets to effectively keep their hair from contacting exposed food, clean equipment, utensils, linens, and unwrapped single-service items. This section does not apply to food employees such as counter staff who ONLY serve beverages and wrapped or packaged foods, hostesses, and wait staff. Note: Observed and discussed that the employee should restrain the bangs or wear hat.
  • Equipment / Good Repair / Components / Gaskets
    Observation: The door gaskets of the following unit is damaged: Beverage Air 1DR upright refrigerator.
    Correction: Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications.
  • Handwashing Cleanser, Available at Handwashing Facilities (corrected on site)
    Observation: Observed that hand soap was not provided at each handwashing sink.
    Correction: Provide hand soap at each hand sink to allow employees to properly wash their hands.
  • Hand Drying Provision / Individual, Disposable Towels (corrected on site)
    Observation: Observed that paper towels were not provided at each handwashing sink.
    Correction: Provide paper towels at each hand sink to allow employees to properly dry their hands after handwashing.
  • Physical Facilities Good Repair (repeated violation)
    Observation: Observed the paint for ceiling above oven is chipping and doors for some cabinets has peeling vineer and is not maintained in good repair.
    Correction: Repair the physical facilities. Poor repair and maintenance compromises the functionality of the physical facilities. The surfaces should be smooth and easily cleanable.
  • Cleaning Frequency for Physical Facilities (repeated violation)
    Observation: Observed that the shelves and vent above the oven in the kitchen is in need of cleaning.
    Correction: Clean the physical facilities at a frequency necessary to ensure the protection and safe preparation of food.

The purpose of today's visit is to conduct a routine inspection.
Please fax a copy invoice for fixing the upright refrigerator within 48 hours. It is recommended that all the teachers also get training for employee health policy because they distribute food to the kids.
Facility Data:
Pest Control: Every month.
  • Critical: Discard RTE, PHF(TCS) if Not Consumed Within 7 Days at 41°F (Unless Frozen) (corrected on site)
    Observation: Observed the following ready-to-eat, potentially hazardous food being held for more than 7 days at 41°F: Sliced American cheese dated 8/12/2013. Note: Discarded.
    Correction: Foods shall be discarded if it: 1) exceeds 7 days or at 41°F (except the time it is frozen)
  • Temp Meas Devices for Ambient Air Location (corrected on site)
    Observation: The temperature measuring device in the following mechanically refrigerated unit was not properly located in the warmest part of the unit: Beverage 1DR upright refrigerator.
    Correction: In a mechanically refrigerated unit the sensor of a temperature measuring device shall be located to measure the air temperature of a simulated product temperature in the warmest part of the refrigerator. Store the thermometer near the door of the unit.
  • Equipment / Good Repair / Operation
    Observation: The following refrigeration unit is not operating properly and is unable to maintain cold food at or below 41°F: Beverage Air 1DR upright refrigerator @ 48F. Note: All the potentially hazardous foods transferred to freezer or discarded. Per the manager the temperature is good in morning, but it goes up during lunch rush.
    Correction: Repair/adjust the unit so it is able to maintain foods at or below 41°F. Do not use the unit to store potentially hazardous foods while it is not operating properly.
  • Physical Facilities Good Repair (repeated violation)
    Observation: Observed that the cabinets and drawers is not maintained in good repair.
    Correction: Repair the physical facilities. Poor repair and maintenance compromises the functionality of the physical facilities.
  • Cleaning Frequency for Physical Facilities
    Observation: Observed that the hinges and area around cabinets in the kitchen is in need of cleaning.
    Correction: Clean the physical facilities at a frequency necessary to ensure the protection and safe preparation of food.
