Reveiwed menu. Most items received precooked frozen. Pizza Hut delivers every Friday. Pizza is immediately put in the Hot Box for hot holding. Temperature logs for food and equipment were reveiwed. Recommend turning Cooler #2 down so cooler will cold hold at all times at < 41 degrees (35 -37) Reminder to date mark on any items not used in 24 hours. Employee Health training documentation observed.

    Observation: There was no temperature measuring device located in the cold plates
    Correction: Provide a temperature measuring device in all hot or cold holding equipment used to store potentially hazardous foods so that employees can routinely monitor the ambient air temperature.
  • Equipment - Good Repair and Proper Adjustment
    Observation: The door gasket to the milk cooler is in poor repair.
    Correction: Repair or replace the door gasket in accordance with the manufacturer's specifications.

    Observation: Handsink close to ice machine was observed in a state of disrepair and damaged.- very slow to drain
    Correction: Repair the handsink to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the @EQUIPMENT@, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.

    Observation: The water pressure gauge on the dishmachine is not in good repair and/ or not accurate in the range of use.
    Correction: Repair, replace, and/or maintain the calibration of gauges for accuracy and function in the intended range of use.

    Observation: Ice scoop observed unprotected from contamination.
    Correction: Store ice scoop in an approved dispenser.

    Observation: Wall or wall covering behind prep table near ice machine is not smooth and easily cleanable.
    Correction: Repair or replace wall or wall covering to make it smooth and easily cleanable.
10/23/2015Routine
Reviewed CDC Handouts with PIC. Handouts given. Discussed cooling methods and temperatures. Handout given. Victory RIC observed not cold holding appropriately 46-60 degrees. Temp log from a.m. observed at 39 degrees. All food had only been in for @ 30 minutes at time of observation so food was moved to another RIC or RIF and will remain empty until unit can be fixed. Reminder if food can not be held at proper temperature will need to use time and then discard. Hot water sanitizing gauge on dishmachine observed not working properly @ 80 degrees but dishwasher tape read at 160 and 180 respectively. Recommend gauge be fixed. Signed Employee Health 1B forms observed. Recommend renewing every year when school starts. Recommend using temperature logs for cooking temperatures as well.

    Observation: Eggo pancakes cold holding at improper temperatures
    Correction: Relocate food to a refrigeration unit that is capable of maintaining food storage at 41°F or below.

Recommend checking backflow device on dish machine and replacing if needed. Observed good handwashing techniques by Employees during inspection.

    Observation: Person in Charge was unable to provide evidence that employees are trained to report health issues of concern.
    Correction: Recommend that PIC have a means to verify that employees are trained to report health issues of concern. EHS provided Form 1-B to Person in Charge.

    Observation: Employees or applicants are not aware of the reporting procedures concerning information about their health as they relate to the symptoms caused by illness, infection, or other source that is associated with an acute gastrointestinal illness or a lesion.
    Correction: Have the license holder require employees or applicants report to the person in charge if they are experiencing the following symptoms or conditions: (1) diarrhea, fever, vomiting, jaundice, or sore throat with fever. (2) A lesion containing pus such as a boil or infected wound that is open or draining and is on the hands or wrists, unless an impermeable cover such as a finger cot or stall protects the lesion and a single-use glove is worn over by an impermeable cover or on other parts of the body, unless the lesions covered by a dry durable, tight-fitting bandage.

    Observation: Observed Employee drying a tray with a cloth rather than air-drying after cleaning and chemical sanitization.
    Correction: Items must be allowed to drain and air-dry before being stacked or stored. Cloth-drying may transfer microorganisms to equipment or utensils.

Discussed reserving unopened cartons of milk, menu and employee health. Form 1 B signed by employees and OEHS guide available. Observed leak around backflow device on hot water into dishmachine. Recommend repairing line and checking backflow device. EHSS requested that PIC call when this done. Discussed reusing BBQ containers. PIC stated containers cleaned for and used by art department.
