Recommended for permit renewal. Permit issued. Left food manager schedule to get another staff member certified after day manager retires. Also left new food handler information.
  • Warewashing Equipment, Determining Chemical Sanitizer Concentration (corrected on site)
    Observation: The chemical sanitizer was too strong. Employee dissolved two tablets in a small container and then poured sanitizer concentrate into bucket and bucket was observed at 400ppm. Employee diluted solution and stated he would use one tablet next time.
    Correction: Use the quaternary ammonium chemical test kit to ensure the sanitizing solution is maintained at the proper solution.
  • Critical: Toxics - Storage of Toxic Containers* (corrected on site)
    Observation: Containers of sanitizer solution are not properly stored to prevent the contamination of food, equipment, utensils, linens or single service items. Sanitizer bucket observed on prep table, employee moved to top of crate next to table.
    Correction: Containers of sanitizer and other chemicals must be located in an area that is not above or near food, equipment, utensils, linens or single service items.

Person in Charge (PIC) present. Food handler to have card while at the establishment. Make table is brand new with functioning lower compartments for cold holding. Delivery was received prior to inspection so elevated unit temperatures were observed. Spoke with PIC regarding backflow prevention device and hose attachment. Food storage is neat and organized. PIC is quite knowledgeable and diligent with employees. Current permit to be displayed..
  • Critical: Demonstration of Knowledge* (repeated violation)
    Observation: A food service employee was working in the food service establishment without having a valid food service card.
    Correction: A food service employee shall have a valid food service card prior to working in the food service establishment. The person in charge shall not employ anyone to work in the food service establishment until they posses a valid food service card. A food service employee shall have a valid food service card while working in a food establishment.
  • Equipment - Good Repair and Proper Adjustment
    Observation: The fan covers above the compressor, were observed in a state of disrepair, dusty and dirty in the walk-in refrigerator.
    Correction: Repair the fan covers to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the @EQUIPMENT@, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.
  • Critical: Backflow Prevention Device, When Required* (corrected on site) (repeated violation)
    Observation: Observed a hose attached to a faucet fixture. The hose extended below the flood rim level of the sink basin.
    Correction: Install an approved backflow prevention device or alter the length of the hose to provide the necessary separation between the water supply and the flood rim level of the sink basin. The minimum allowable separation distance must be at least 2x the diameter of the water supply inlet and at least 1 inch. Hose removed at the time of the inspection.
  • Floor and Wall Junctures, Coved, and Enclosed or Sealed
    Observation: Floor wall juncture under the food prep table is not coved and closed to no larger than 1/32 inch space.
    Floor and wall juncture outside the employee unisex toilet facility is not sealed

    Correction: Cove floor wall juncture to no larger than a 1/32 inch space.
    Seal floor and wall juncture in rooms where water flush cleaning methods are used.

Three CFM's on staff, one for each shift. Facility well organized. New Form 1-B's issued. Recommended for permit renewal. Permit issued.

    Observation: A few food service employee are working in the food service establishment without having a valid food service card, all employees are newly hired drivers. Left schedule with PIC.
    Correction: A food service employee shall have a valid food service card prior to working in the food service establishment. The person in charge shall not employ anyone to work in the food service establishment until they posses a valid food service card. A food service employee shall have a valid food service card while working in a food establishment.
  • Equipment - Cutting Surfaces
    Observation: The red cutting board are heavily scratched and scored. The food contact surface is no longer easily cleaned and sanitized due to condition.
    Correction: Resurface or discard and replace the cutting boards to provide a food contact surface that is smooth, easily cleaned and sanitized.
  • Critical: Backflow Prevention Device, When Required* (corrected on site)
    Observation: Observed a hose attached to a faucet fixture without a backflow preventer. The hose extended below the flood rim level of the sink basin. PIC removed hose and states that she will instruct night staff to remove hose after use.
    Correction: Install an approved backflow prevention device or alter the length of the hose to provide the necessary separation between the water supply and the flood rim level of the sink basin. The minimum allowable separation distance must be at least 2x the diameter of the water supply inlet and at least 1 inch.
  • Physical Facilities in Good Repair
    Observation: Top countertop of island counter and back wall near triple sink are not maintained in good repair. PIC states they are waiting on the wall to be fixed and will get them to also seal countertop.
    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.
