A complaint was received on 6/29/15 stating that the complainant at southwest chicken salad on 6/27/15 and was ill driving home.
During the inspection the manger on duty was aware of the complaint. All potentially hazardous food were registered at adequate temperature. No contamination or temperature abuse observed. Good employee hygiene practice observed. Reviewed employee health policies and best hygiene practices with the operator and must be reviewed with the employees. Manager provided data that between 6/22/15 to 6/28/15 954 transaction occurred (food sold to 954 people). No other complaint was received. Complaint not found and addressed.

    Correction: Provide a sign or poster that notifies food employees to wash their hands at all handwashing lavatories used by food employees.

    Observation: Toilet tank observed broken in women's restroom.
    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.

    Observation: COOLERS was observed in a state of disrepair and damaged.
    Correction: Repair /service coolers to proper function.

    Observation: The following utensils were observed soiled to sight and touch: knife
    Correction: Clean and sanitize these surfaces for food contact. And store in clean manner and clean location.

    Observation: The handwashing facility located at the kitchen is blocked, preventing access by employees for easy handwashing.
    Correction: Access to the handwashing facility identified above is to be available during all hours of operation. Remove the items preventing its use.

    Observation: Medicines and employee items are store with food.
    Correction: Locate medicines and employee items to prevent contamination of food, equipment, utensils, linens, and single-service and single-use articles
