The purpose of today's visit was to conduct a routine assessment in conjunction to a complaint investigation dated 1/11/16. The following suggestions have been made as a means to incorporate Active Managerial Control:
1. Ensure employees are reheating prepared items like tomato sauce and macaroni and cheese in the microwave oven rather than Alto Sham to ensure items are reheated to 165F or above in the proper time. Train employees to put items in the hot holding cabinet only after they have reached proper reheating temperatures.
2. Create a designated space for employee storage (i.e., bottom shelves in kitchen prep area) to avoid contamination of food and food contact surfaces.
Please send the following to the Health Department by 2/11/16:
1. Copy of CFM card

  • Critical: Hot Holding of Potentially Hazardous Food at 135°F or above (corrected on site)
    Observation: The following food item(s) were found hot holding at improper temperatures using a calibrated food temperature measuring device: IN ALTO SHAM 2 DR HOT HOLDING CABINET #1- MACARONI, TOMATO SAUCE, CHICKEN BROTH (113F, 107F, 128F).
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be hot held at a temperature of 135°F or above unless the permit holder is using "time as public health control" as specified under 3-501.19. ALL ITEMS REHEATED TO 165F OR ABOVE.
  • Certified Food Manager/Certificate Process
    Observation: The person in charge (PIC) has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc.
    Correction: PLEASE OBTAIN A CFM CARD WITHIN 30 DAYS. A COPY OF YOUR CFM CARD CAN BE SENT TO THE HEALTH DEPARTMENT VIA FAX OR E-MAIL. Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. (see handout provided). A copy of the photo identification card can be faxed to the food safety section at (703) 653-9448.
  • Lockers or Employee Storage in Designated Room/Area
    Observation: Observed that lockers and personal storage facilities are not located to protect food, equipment, linens, and/or single-service items from contamination. OBSERVED EMPLOYEE JACKETS, SCARVES, AND PURSES BEING STORED ON DRY STORAGE SHELVING NEAR COOKING OILS AND BAKING MIXES.
    Correction: PLEASE DESIGNATE EMPLOYEE STORAGE AREA WITHIN 30 DAYS TO PREVENT CONTAMINATION OF FOOD AND FOOD CONTACT SURFACES. Relocate lockers and personal storage facilities to prevent the potential contamination of food, food equipment and utensils, linens, or single-service items.

The purpose of today's visit was to conduct a risk factor assessment. The following practices are being used as a means to demonstrate Active Managerial Control:
1. Sanitizer concentration measured three times a day. Test strips kept on file for reference and accountability.
2. Food temperatures taken every 2 hours.
3. Food and equipment temperature logs maintained on file.
4. Segregation of raw protein and produce prep areas.
Please continue to use these methods as a way to promote public health.

Please be advised to continually provide employee health training and education for employees.   • Critical: Responsibility of PIC to Require Food Employees to Report GI Symptoms or Cuts, Disease, Medical Condition or Exposure to PIC (repeated violation)
    Observation: Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy.
    Correction: A complete employee health policy must have the following elements:
    1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout)
  • Sanitizer Test Kit Required (repeated violation)
    Observation: A chemical vegetable wash solution is used in the establishment. No test kit is provided to measure the concentration of the chemical in the solution.
    Correction: A chemical vegetable wash is considered a food additive and must be maintained at the concentration recommended by the manufacturer in order to be safe. Provide the approved test kit.
