Discussed the following with the PIC: Transfer or acquire CFM for new owner to the establishment. Remove equipment from customer restroom and maintain cleanliness of the restroom. Ensure that Kenmore refrigerator is capable of maintaining a low enough temperature before food is placed within. Continue to receive pest treatment at the facility from a licensed PCO since there is evidence of pests outside of the establishment
  • Critical: Food - Consumption of Raw or Undercooked Animal Foods (repeated violation)
    Observation: Inaccurate consumer advisory on menu.
    Correction: Place an asterisk next to all items that will be cooked to order (possibly undercooked).
  • Critical: Warewashing - Manual & Mechanical Warewashing Equipment, Chemical Sanitization - Temperature, pH, Concentration & Hardness*
    Observation: Chlorine sanitizing solution used was not at an acceptable concentration.
    Correction: Adjust the chlorine sanitizing solution to a level between 50 and 100 parts per million when the pH is at 10 or less and the water temperature is 100°F.
  • Non-Food Contact Surfaces
    Observation: The nonfood contact surface of the kenmore refrigerator interior and the bottom surface of the kenmore freezer have accumulations of grime and debris.
    Correction: Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents.

    Observation: Following noted in need of cleaning: customer rest room, wall behind stove and ceiling above soup hot holding unit.
    Correction: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean.

    Observation: Facility is storing unnecessary items to the operation or maintenance of the establishment.
    Correction: Remove all unnecessary items from the facility. The presence of unnecessary articles, including equipment which is no longer used, makes regular and effective cleaning more difficult and less likely. It can also provide harborage for insects and rodents.

    Correction: The permit holder shall provide a Chesapeake registered, certified manager to monitor and manage all food establishment operations and who is authorized to take actions to ensure that the Regulations' objectives are fulfilled.
  • Hair Restraints - Effectiveness
    Observation: Employees observed working in the food service area without proper hair restraints.
    Correction: Ensure all employees wear hair restraints such as hats, hair coverings or nets, beard restraints, and clothing that covers body hair, that are designed and worn to effectively keep their hair from contacting exposed food, clean equipment, utensils and linens, and unwrapped single-service and single-use articles. This section does not apply to food employees such as counter staff who only serve beverages and wrapped or packaged foods, hostesses, and wait staff if they present a minimal risk of contaminating exposed food

    Observation: Wiping cloths improperly stored between uses(>400ppm quats today).
    Correction: Ensure wet wiping cloths are stored in a chemical sanitizer at the proper concentration(150-400ppm) between uses.

    Observation: Foods in the Kenmore refrigerator cold holding at improper temperatures
    Correction: Relocate food to a refrigeration unit that is capable of maintaining food storage at 41°F or below. Repair or replace the refrigerator.

    Observation: Inaccurate consumer advisory on menu.
    Correction: Place an asterisk next to the name of each animal based food item that is cooked to order(rare or under cooked) and increase the size of the disclaimer at the bottom of the page.
  • Sanitizing Solutions, Testing Devices
    Observation: There is no properly working quats test kit provided in the facility for monitoring the concentration of the chemical sanitizing solutions.
    Correction: Obtain a quaternary ammonium test kit so that employees can accurately monitor the concentration of the sanitizing solutions and maintain them at a safe and effective level.
  • Physical Facilities in Good Repair
    Observation: Physical structure is not maintained in good repair: ceiling over desk, water damaged, moldy walls/ceiling in customer rest room.
    Correction: Make all repairs withim two weeks. Rest room must be in good repair, clean, and stocked for use. Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.
