Adequate thermometer and chlorine test kit for sanitizer used in the kitchen. Spray sanitizer is quaternary ammonia and is dispensed in the laundry room.
Notes:
1. Cream cheese and whipped butter packets must be kept at 41 F or less at all times if staff plan to keep it. Keep it nested in ice/water like the yogurt cups. This was not mentioned in previous inspections over the past two years, so this serves as the facility's notice to maintain these items cold, or they must be discarded at the end of breakfast daily (which the facility does not want to do).
2. Employee health forms are kept on file in the office.
3. At least one employee needs to become ServSafe certified in early 2016 to be compliance with a new regulation.

    Observation: Quaternary ammonium sanitizer being applied to food contact surfaces does not meet the requirements of 40 CFR 180.940. Sanitizer in the spray bottle tested at >400 ppm.
    Correction: Utilize only 200 ppm that meet the requirements of 40 CFR 180.940 when applying to food contact surfaces

02/19/2015Routine
Thermometer present, but off by 24 degrees F. Recalibrated during the inspection. Staff did not know where the thermometer and chlorine test kit were located, and were not using them.
Manager set up the 3-vat sink, used hot water with bleach, and did not use the chlorine test kit to determine the concentration of the solution. The inspector emptied the sanitizer and showed staff the correct way to do it.
The normal breakfast employees were not present today. A new employee (only second day) had some food safety knowledge from her previous employer.
Any employee that has ANY food handling duties must be educated on basic food safety practices, and the importance of employee health. Neither of the employees working breakfast this morning recognized FDA form 1-B and the signs/symptoms of when food handling cannot be done.
Hand sinks are for hand washing and no other purpose at ALL times.
Gloves can NEVER be removed and put back on. A pair of used gloves observed on the counter, but staff were NOT observed trying to re-using them.

    Observation: Employees or applicants are not aware of the reporting procedures concerning information about their health as they relate to the symptoms caused by illness, infection, or other source that is associated with an acute gastrointestinal illness or a lesion.
    Correction: Have the license holder require employees or applicants report to the person in charge if they are experiencing the following symptoms or conditions: (1) diarrhea, fever, vomiting, jaundice, or sore throat with fever. (2) A lesion containing pus such as a boil or infected wound that is open or draining and is on the hands or wrists, unless an impermeable cover such as a finger cot or stall protects the lesion and a single-use glove is worn over by an impermeable cover or on other parts of the body, unless the lesions covered by a dry durable, tight-fitting bandage.

    Observation: Observed food thermometer is not calibrated to ensure accuracy. Off by 24 degrees.
    Correction: Calibrate food thermometer according to manufacturer's specification as necessary to ensure accuracy
  • Handwashing - Using a Handwashing Lavatory - No Other Purpose
    Observation: The handwash station in the kitchen area is being used for purposes other than washing hands. Wiping cloths being rinsed out in the hand sink.
    Correction: The handwash facility identified above is to be used for washing hands only
  • Critical: Sanitizer - Criteria/Chemicals for food contact*
    Observation: Bleach water in the sanitizer compartment being applied to food contact surfaces does not meet the requirements of 40 CFR 180.940. Tested at 200 ppm+.
    Correction: Utilize only bleach water that meet the requirements of 40 CFR 180.940 when applying to food contact surfaces, measuring at 50-100 ppm. Inspector showed employee how to properly mix and test sanitizer water.

    Observation: Yogurt cups on ice cold holding at improper temperatures
    Correction: Relocate food to a refrigeration unit that is capable of maintaining food storage at 41°F or below. Add enough ice to cover all yogurt cups while sitting out.
