Today’s visit was to conduct a Risk Factor Assessment inspection.
Both hot and cold buffet items get discarded when facility closes at 3PM.
Do not leave out potentially hazardous foods out at room temperature! They must be kept cold (41F and below) and/or hot (135F and above)!
It is highly recommended to have more than one Certified Food Manager on duty.
EHS provided additional consultation, training, handouts, and/or written instructions on: reheating, cross contamination, date-marking

  • Handwashing / Where to Wash (corrected on site)
    Observation: A food employee was observed cleaning their hands in a food preparation sink or in a mop sink.
    Correction: ALL food employees shall wash their hands in ONLY a designated handsink.
  • Critical: No Bare Hand Contact with RTE Foods (corrected on site)
    Observation: A food employee was observed handling the following ready-to-eat food using their bare hands: bread
    Correction: Except when washing fruits and vegetables, food employees may not contact ready-to-eat foods using their bare hands. Employees shall use suitable utensils such as deli tissue, spatulas, tongs, or clean disposable gloves to handle all ready-to-eat foods.     Observation: Raw animal food holding in a manner that may cause cross contamination of ready-to-eat food as follows: 1) raw fish over cooked rice, raw chicken over sauces in walkin cooler, 2) raw shell eggs over cooked chicken in Turbo Air m3 2dr preptop cooler
    Correction: Food shall be protected from cross contamination by separating raw animal foods like poultry, beef, pork, seafood, and eggs during holding from ALL other foods such as cooked foods, ready-to-eat foods, washed produce, etc. Food employee rearranged raw items to the last shelf

    Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: butter 115F, cream 61F, cooked chicken 57F, cooked potato 52F all at room temperature
    Correction: Potentially hazardous foods (time/temperature conrol for safety food) shall be held cold at a temperature of 41°F or below. All items were placed back in the walkin cooler
  • Critical: Date Marking of Refrigerated, Ready-to-Eat PHF (TCS Food) (corrected on site)
    Observation: The following ready-to-eat food that is made using a combination of ready-to-eat, potentially hazardous ingredients is not properly dated for discard using the date of the first-prepared ingredient: cooked chicken, curry, cooked potato all prepped on December 12th.
    Correction: Refrigerated, ready-to-eat, potentially hazardous food (time/temperature control for safety foods) that combined with additional ingredients, or portions of the same food, must retain the date marking of the earliest or first prepared ingredient.     Observation: There is no Certified Food Manager present at the beginning of the inspection.
    Correction: It shall be unlawful to operate a food establishment unless it is under the immediate control of a certified food manager. It is highly suggested that you have more than one employee with the food manager's license. ORS Interactive, Inc. (see handout provided) issues the required photo identification card with proof of successful completion of a certified food manager's exam. Failure to have a certified food manager on site during ALL hours of operation including food preparation, food service, and cleaning/sanitizing of equipment/utensils will result in closure of the establishment.     Observation: Dispensing and/or in-use utensils improperly stored between use as follows: spoons in standing water and ice scoop lying on top of ice
    Correction: During pauses in food preparation or dispensing, food utensils shall be stored in one of the following manners: 1) in the food with their handles above the top of the food and the container, 2) in food that is not potentially hazardous (time/temperature control for safe food) with their handles above the top of the food within containers or equipment that can be closed, 3) on a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food contact surface of the food preparation table or cooking equipment are cleaned and sanitized, 4) in running water of sufficient velocity to flush particulates to the drain, 5) in a clean, protected location if the utensils are used only with a food that is NOT potentially hazardous (time/temperature control for safe food), or 6) in a container of water if the water is maintained at a temperature of at least 135°F and the container is cleaned frequently.

    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth. All potentially hazardous food items were removed from the cooler

    Observation: The nonfood-contact surfaces of the following equipment had accumulations of grime and debris: towel holder at kitchen handsink and handles on equipment
    Correction: Nonfood-contact surfaces of equipment and utensils shall be cleaned at a frequency necessary to preclude accumulation of soil residues.

    Observation: Observed that clean utensils were stored in an manner that exposed the item(s) to contamination.
    Correction: Store clean equipment, utensils, and linens in a clean, dry location where they are not exposed to splash, dust, or other contamination and at least 6 inches above the floor.

The purpose of this visit is to conduct a risk factor assessment.
It is recommended to increase cleaning frequency to ensure food debris build up does not occur.

  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (repeated violation)
    Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device:
    Buffet: Rice Curd 50F
    2dr refrigeration unit: Paneer 54F

    Correction: Potentially hazardous foods (time/temperature conrol for safety food) shall be held cold at a temperature of 41°F or below. MANAGER DISCARDED PRODUCTS.
  • Handwashing Sink/Accessible at All Times (corrected on site)
    Observation: Observed a burner directly in front of the handsink preventing access to the handsink.
    Correction: A hand washing sink shall be maintained so that it is accessible at all times for employee use. Manager removed burner from the front of the handsink.
11/17/2014Risk Factor
The purpose of today's visit was to conduct the first routine inspection at the above facility after a change of ownership. EHS provided owner with and discussed the following handouts: cooling log, sign and methods, Food for Thought Newsletter Titled "How Cold Is It", Equipment Monitoring Log, Food temperature log and food time and temperature reminders, CFM guidelines and various other topics as future resource and reference. Please fax or email a third party service report for the Delfield 2DR prep within 5 calendar days, no later than Monday, June 30, 2014 at 4:00 pm. Please continue to train and monitor employees regarding the critical violations observed today. Please also make sure that all non-critical violations observed during the change of ownership inspection are corrected and remain corrected.

    1) Ice scoop observed laying and partially under ice in the ice bin,
    2) Spoons and cooking utensils being stored in stil/stagnant room temperature water.

    Correction: During pauses in food preparation or dispensing, food utensils shall be stored in one of the following manners: 1) in the food with their handles above the top of the food and the container, 2) in food that is not potentially hazardous (time/temperature control for safe food) with their handles above the top of the food within containers or equipment that can be closed, 3) on a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food contact surface of the food preparation table or cooking equipment are cleaned and sanitized, 4) in running water of sufficient velocity to flush particulates to the drain, 5) in a clean, protected location if the utensils are used only with a food that is NOT potentially hazardous (time/temperature control for safe food), or 6) in a container of water if the water is maintained at a temperature of at least 135°F and the container is cleaned frequently.
  • Critical: PHF(TCS) Cooled from 135°F to 41°F within 6 Hours Total (corrected on site)
    Observation: The following cooked potentially hazardous food(s) are not being adequately cooled from 135°F to 70°F within two hours and to 41°F within a total of six hours:
    1) Tomato sauce cooling in large deep plastic container covered with plastic lid cooling over 12 hours at 64F,
    2) Cooked onion sauce cooling in large deep plastic container covered with plastic lid cooling over 12 hours at 49F.

    Correction: Potentially hazardous foods shall be cooled from 135°F to 70°F within two hours and to 41°F within a total of six hours to prevent the growth of harmful bacteria. Foods that have been temperature abused are to be discarded. DISCARDED
  • Critical: Hot Holding of Potentially Hazardous Food at 135°F or above (corrected on site)
    Observation: The following food item(s) were found hot holding at improper temperatures using a calibrated food temperature measuring device:--------sheer karma (dessert with milk & noodle) on buffet at 102F.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be hot held at a temperature of 135°F or above unless the permit holder is using "time as public health control" as specified under 3-501.19. DISCARDED.
  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site)
    Observation: The following food item(s) in the Delfield 2DR prep cooler were observed cold holding at improper temperatures using a calibrated food temperature measuring device:
    1) Cooked chicken at 57F,
    2) Cooked vegetables at 69F,
    3) Heavy cream # 1 at 65F,
    4) Heavy cream # 2 at 52F.

    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth. ALL POTENTIALLY HAZARDOUS FOOD IN DELFIELD 2DR PREP COOLER DISCARDED.

    Observation: Water from the handwashing sink in kitchen was measured at a temperature less than 100°F (observed at 70F). The hot water was not working.
    Correction: A handwashing sink shall be equipped to provide water at a temperature of at least 100°F through a mixing valve or combination faucet.
  • Handwashing Sink / Accessible at All Times (corrected on site)
    Observation: The handwashing facility located in the kitchen and bar area are blocked, preventing access by employees for easy handwashing. -----Observed large stove burner in front of kitchen handsink. Observed boxes of single service items being stored on top of the foot pump of the bar handsink.
    Correction: A handwashing sink shall be maintained so that it is accessible at all times for employee use.
  • Plumbing / Maintained in Good Repair (repeated violation)
    Observation: Plumbing connections under kitchen handsink are leaking.
    Correction: A plumbing system shall be maintained in good repair.
  • Outer Openings, Protected / Screen Requirements
    Observation: Back door observed open for ventilation and opening is not protected by a screen or other effective means.
    Correction: Provide screens (16 mesh to 1 inch), air curtains, or other effective means to protect against the entry of insects and rodents.
  • Critical: Separation of Toxics by Spacing or Partitioning (corrected on site)
    Observation: Observed that poisonous and toxic materials are not properly stored to prevent the contamination of food, equipment, utensils, linens or single service items.--------Observed residential pest control spray, bathroom cleaner spray bottle and window cleaner container being stored on the same shelf as dry storage food items and clean utensils in two different locations.
    Correction: All poisonous and toxic materials shall be stored to prevent contamination of food, equipment, utensils, linens, and single service items by separating the poisonous and toxic materials by spacing or partitioning. RE-ARRANGED AND RESIDENTIAL PEST SPRAY DISCARDED.

**Approval is hereby granted for the issuance of a Health Department Permit. This inspection report shall serve as your permit until the official permit is received. All outstanding violations are to be corrected within ten (10) days but no later than the time of your first routine inspection (approximately 30 days). Equipment additions/replacement/changes are not allowed without Health Department approval.**
Note the following recommendations:
It is recommended that more than one person on staff be a Certified Food Manager. A Certified Food Manager must be on the premises during ALL food preparation and service.
Recommendation is made to have exhaust hood cleaned at least every six months. Maintain records of hood cleaning for Health Department review.
Recommendation is made to clean hood filters at least every two weeks.
It is recommended that the grease trap be cleaned at least every two months. Maintain records of grease trap cleaning for Health Department review.
It is recommended that a monthly pest control program be implemented. Maintain records of pest control treatments for Health Department review.

    Observation: Observed the following to be in disrepair:
    1) Shelving underneath the prep tables (rusted)

    Correction: The above mentioned items shall be maintained in good repair.
  • Equipment / Good Repair / Components / Gaskets
    Observation: The door gaskets of the following unit(s) are (missing, damaged):
    1) US Cooler walkin cooler

    Correction: Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications.
  • Nonfood-Contact Surfaces Clean at Frequency to Preclude Accumulation
    Observation: The nonfood-contact surfaces of the following equipment had accumulations of grime and debris:
    1) Gaskets on the walkin/reachin units,
    2) Exterior of equipment along the grill line,
    3) Ventilaiton hood (professional cleaning required)

    Correction: Nonfood-contact surfaces of equipment and utensils shall be cleaned at a frequency necessary to preclude accumulation of soil residues.

    Observation: Observed that the following is not maintained in good repair:
    1) Cove molding along the floor/wall juncture behind the handsink (loose),
    2) Soap and paper towel dispensers by the prep sink (remove),
    3) Ceiling tiles,
    4) Walls in the back food prep area.

    Correction: Repair the physical facilities. Poor repair and maintenance compromises the functionality of the physical facilities.
  • Maintaining Premises Free of Unnecessary Items and Unused Equipment
    Observation: Observed that items that are unnecessary to the operation or maintenance of the establishment are being stored on the premises.
    Correction: Remove unnecessary items, including equipment which is no longer in use, from the premises to allow for regular and effective cleaning and to prevent harborage conditions.
  • Cleaning Frequency for Physical Facilities
    Observation: Observed that the following is in need of cleaning:
    1) Walls,
    2) Floor/wall junctures underneath and behind equipment and shelving in the food prep area,
    3) Ceiling tiles,
    4) Ceiling vents
