Per the Certified Food Manager (CFM), the revisions to the menu's consumer advisory will be completed by end of April, 2016. Consumer advisory templates have been provided to the CFM. Discussed the following with the CFM: 1) Written employee health and sickness policy (FDA Form 1B provided) 2) Re-train employees on the following: Drinking beverages in the food prep area, when to wash hands, not using soiled cloths to wipe gloves during food prep, using the hand washing sink for hand washing only 3) Not overfilling the condiment bottles while storing them cold in the tumblers.

    Observation: Multiple tongs and a spatula observed with cracks and chips, and are not designed and constructed to be durable.

    Observation: The FLOOR of the WALK IN REFRIGERATOR was observed damaged, and peeling off. It is in a condition that prevents necessary maintenance and easy cleaning..
    Correction: Repair the FLOOR of the WALK IN REFRIGERATOR to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the floor of the walk in refrigerator, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.

    Observation: Multiple cutting boards along the cook line, along the meat slicer, and at the crepe station are heavily scratched and scored. The food contact surface is no longer easily cleaned and sanitized due to condition.
    Correction: Resurface or discard and replace the cutting boards to provide a food contact surface that is smooth, easily cleaned and sanitized.
  • Warewashing - Mechanical Warewashing Equipment, Hot Water Sanitization Temperatures (corrected on site)
    Observation: The water temperature in the manifold servicing the high temperature ware wash machine was below the minimum allowable level of 180ºF. The high temperature sensor strip registered a temperature of 160 - 170 F. Person in charge (PIC) called a Ecolab professional and the professional adjusted the rinse temperature to 180 F as per the high temperature sensor strip.
    Correction: Immediately discontinue the use of the machine until it is operating in accordance with minimum requirements. Initiate manual wash, rinse, and sanitize method to properly clean and sanitize equipment and utensils.
  • Warewashing - Mechanical Warewashing Equipment, Sanitization Pressure (corrected on site)
    Observation: The pressure gauge for the high temperature warewash machine was below 15 PSI. The actual pressure during the rinse cycle was observed at 10 psi. Person in charge (PIC) called a Ecolab professional and the professional adjusted the pressure to between 15 - 25 psi.
    Correction: Immediately discontinue the use of the machine until it is operating in accordance with minimum requirements. The range for proper operation is between 15 and 25 PSI. Initiate manual wash, rinse, and sanitize method to properly clean and sanitize equipment and utensils. To minimize workload you can move to all single service utensils.
