    Observation: Wet wiping cloth buckets improperly stored IN BUCKET ON FLOOR
    Correction: Wet wiping cloth buckets containing chemical sanitizing solutions shall not be stored on the floor. The wet towel buckets shall also be used in a manner that prevents contamination of food, equipment, utensils, linens, and single service items.
  • Critical: Hot Holding of Potentially Hazardous Food at 135°F or above (corrected on site)
    Observation: The following food items were found hot holding at improper temperatures using a calibrated food temperature measuring device: IN RACK BY FRYERS: CHICKEN TENDERS AT 126F AND HASH BROWNS AT 128F.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be hot held at a temperature of 135°F or above unless the permit holder is using "time as public health control" as specified under 3-501.19. ITEMS WERE DEISCARDED.
  • Food Temperature Measuring Device Required & Readily Accessible
    Observation: The certified food manager could not provide a food temperature measuring device.
    Correction: A metal-stemmed probe food thermometer shall be provided and readily accessible for use in ensuring attainment and maintenance of food temperatures.
  • Sanitizer Test Kit Required
    Observation: There is no test kit located in the facility for monitoring the concentration of the chemical sanitizing solutions.
    Correction: Obtain a QUATERNARY AMMONIA test kit FOR SANITIZER USED IN 3-VAT SINK.
  • Critical: Chlorine Sanitizer Solution Concentration based on pH and Water Temp (corrected on site)
    Observation: Chlorine santizing solution was not observed with an acceptable concentration. Observed the sanitizing solution in the WIPING CLOTH BUCKET with a concentration of LESS THAN 50 ppm.
    Correction: A chlorine (bleach) sanitizing solution shall have a concentration between 50-100ppm. MANAGER MIXED NEW SOLUTION.
  • Clean Ice Bins, Beverage Dispenser Nozzles, Etc. per Manufacturer Specs. (corrected on site) (repeated violation)
    Observation: Surfaces of ice bins, beverage dispensing nozzles, and enclosed components of equipment are not being cleaned as required. WHITE PLASTIC INTERIOR SURFACE OF ICE BIN AT BACK OF KITCHEN HAD SMALL AMOUNT OF PINK SOILAGE.
    Correction: Surfaces of utensils and equipment contacting food that is NOT potentially hazardous (time/temperature control for safety food) such as ice bins, beverage dispensing nozzles, and enclosed components of equipment such as ice makers, cooking oil storage tanks and distribution lines, beverage and syrup dispensing lines or tubes, coffee bean grinders, and water vending equipment at a frequency specified by the manufacturer or at a frequency necessary to preclude accumulation of soil or mold. CLEANED BY MANAGER.

    Observation: The door gaskets of the following unit are damaged: Kolpak Walk-In Cooler.
    Correction: Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications. CORRECTIVE ACTION: REPLACE DOOR GASKETS.
  • Handwashing Sink / Accessible at All Times (corrected on site)
    Observation: The handwashing facility located in the kitchen near the 3-compartment sink was blocked, preventing access by employees for easy handwashing.
    Correction: A handwashing sink shall be maintained so that it is accessible at all times for employee use. CORRECTIVE ACTION: TRASH RECEPTACLE WAS MOVED TO RENDER SINK ACCESSABLE.
  • Refuse Container with Cover/ Women's & Unisex Toilet Room (repeated violation)
    Observation: There is no covered refuse container for the disposal of feminine napkins in the ladies room stall. ONE OF TWO STALLS HAS NO CONTAINER.
    Correction: A toilet room used by females shall be provided with a covered receptacle for sanitary napkins. CORRECTIVE ACTION: PROVIDE CONTAINER
  • Floor , Wall & Ceiling / Design (repeated violation)
    Observation: The ceiling tile located NEAR THE GARLAND OVEN IS DAMAGED, AND CEILING TILES IN THE BATHROOM HALLWAY HAVE WATER STAINS.
    Correction: Wall, floor, and ceiling surfaces shall be smooth, durable, easily cleanable, and non-absorbent for areas subject to moisture such as food preparation areas, walk-in refrigerators, warewashing areas, and toilet rooms. CORRECTIVE ACTION: REPLACE DAMAGED TILES.

Hot Water Heater: AO Smith BT 100-230

  • Critical: Responsibility of PIC to Require Food Employees to Report GI Symptoms or Cuts, Disease, Medical Condition or Exposure to PIC (repeated violation)
    Observation: Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy." Observation: Manager was aware of program and FCHD folder and documentation was located, but no action has been taken
    Correction: discussed importanceof policy and implementation.
  • Equipment / Good Repair / Components / Gaskets
    Observation: The door gaskets of the following unit(s) are (missing, damaged): Kolpak Walk-In Cooler.
    Correction: Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications. Replace gaskets: manager stated replacements were on order.

    Observation: Surfaces of ice binsare not being cleaned as required. Observation: discoloration on inner panel of ice machine.
    Correction: Surfaces of utensils and equipment contacting food that is NOT potentially hazardous (time/temperature control for safety food) such as ice bins, beverage dispensing nozzles, and enclosed components of equipment such as ice makers, cooking oil storage tanks and distribution lines, beverage and syrup dispensing lines or tubes, coffee bean grinders, and water vending equipment at a frequency specified by the manufacturer or at a frequency necessary to preclude accumulation of soil or mold.
  • Fairfax County improperly designed water heater
    Observation: On November 29, 2005 the Fairfax County Health Department adopted specific design requirements for water heaters as follows: (1) Commercial grade, (2) Minimum of 50 gallon storage tank, (3) Recovery rate meets or exceeds the minimum hourly hot water demands of the food establishment as calculated by the Health Department, and (4) Specifications and installation meet National Sanitation Foundation (NSF) Standard 5. Our records indicate that your current hot water heater (AO Smith Model 100-230, 25,000 BTU input at 120F with 80F rise) does not meet these design requirements. THE CURRENT UNIT DOES NOT MEET STANDARD 5.
    Correction: This policy does not apply to an existing food establishments with a previously approved water heater that does not comply with the design requirements. Your facility falls into this category. However, the design requirements will apply to your facility in the event of a change of ownership, remodeling/renovation, or if it is determined that your facility exhibits a hot water deficiency in the future. A certified letter will be sent to your facility notifying you of this requirement, and a copy will be placed in your establishment file.

    Correction: Wall, floor, and ceiling surfaces shall be smooth, durable, easily cleanable, and non-absorbent for areas subject to moisture such as food preparation areas, walk-in refrigerators, warewashing areas, and toilet rooms. replace damaged or discolored tiles with non-absorbent and easily cleaned tiles.
  • Light Bulbs / Locations where Shielding is Required
    Observation: Fluorescent light fixture above aisle behind cook line has light tubes not covered by a protective shielding.
    Correction: Provide light bulbs that are shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, linens, or unwrapped single-service and single-use articles. Provide protective sheilding.
  • Hand Drying Provision / Individual, Disposable Towels
    Observation: Observed that paper towels were not provided at each handwashing sink. Men's room paper towel dispenser was empty.
    Correction: Provide paper towels at each hand sink to allow employees to properly dry their hands after handwashing.
  • Lighting Intensity/ Walk-ins & Dry Storage/ 10 foot candles
    Observation: Observed that inadequate lighting was provided in the walk-in refrigerators, walk-in freezers, and/or dry storage areas. Light measured by light meter in Kolpak WI-Cooler was 3-5 footcandles
    Correction: minimum required is 10 footcandles.
